Attualmente, il tasso di recidiva omolaterale del tumore al seno (IBTR) dopo chirurgia conservativa e radioterapia rimane rispettivamente dal 10% a 10 anni al 15% a 20 anni. L'IBTR è un predittore indipendente di scarsa sopravvivenza con un aumento del rischio di morte per cancro da 3 a 4,6.
In una popolazione eterogenea, il controllo locale rimane una sfida terapeutica importante per queste recidive, specialmente quelle considerate di prognosi migliore, vale a dire che si verificano tardivamente e di basso grado istologico. Per queste ricadute è possibile una de-escalation terapeutica per evitare una mastectomia mutilante e spesso traumatica.
Tuttavia, questo secondo intervento conservativo presenta un alto tasso di seconda recidiva locale (dal 19 al 50% a 5 anni) a causa dell'assenza di una re-irradiazione, resa impossibile dal problema della tolleranza dei tessuti precedentemente irradiati.
Studi retrospettivi o prospettici sull'irradiazione parziale del seno (PBI) in ambito adiuvante riportano risultati promettenti, sia in termini di tolleranza (risparmio di tessuto sano) che di controllo locale (dal 74% al 100% a 5 anni). Le tecniche utilizzate includono la brachiterapia, la radioterapia a fasci esterni e la radioterapia intraoperatoria (IORT).
IORT è ora oggetto di rinnovato interesse per il cancro al seno. Presenta i vantaggi della balistica di alta precisione sull'area operata e della conservazione del tessuto sano. Ad oggi, tuttavia, non sono stati pubblicati dati prospettici sull'indicazione di recidiva mammaria omolaterale.
Un progetto di Fase II prospettico, multicentrico, a braccio singolo valuterà la fattibilità di ripetuti interventi chirurgici di conservazione della mammella combinati con la reirradiazione mediante IORT dopo recidiva locale di carcinoma mammario.

- Recidiva mammaria non lobulare invasiva istologicamente provata
- Tempo dall'irradiazione dell'intero seno dopo la lumpectomia iniziale >5 anni
- Tumore unifocale
- Dimensione del tumore ricorrente ≤ 2 cm
- Dimensioni del seno adeguate per un secondo intervento chirurgico di conservazione del seno con risultati estetici accettabili
- - Mammografia mammaria bilaterale entro 90 giorni prima dell'ingresso nello studio
- RM al seno entro 90 giorni prima dell'ingresso nello studio
- Grado istologico I-II
- Tumore positivo per il recettore degli estrogeni (ER+)
- Tumore Cerb2-negativo
- N0
- M0
- - Precedente radioterapia erogata entro un programma di frazionamento standard
- Stato delle prestazioni (ECOG) 0-1
- Donne ≥ 50 anni - Assenza di qualsiasi condizione psicologica, familiare, sociologica o geografica che possa potenzialmente ostacolare la conformità con lo studio e il programma di follow-up
- Affiliato al regime di assicurazione sanitaria francese
- Modulo di consenso informato scritto e firmato.
Criteri di esclusione:
- Recidiva multifocale e/o multicentrica
- Carcinoma lobulare
- Tumore negativo al recettore degli estrogeni (ER-)
- Cerb2 (her2) sovraespresso - cancro al seno
- Ampia componente intraduttale (EIC) alla biopsia
- Invasione dei vasi linfatici alla biopsia
- Stato N1-3: recidiva linfonodale regionale citologica o istologicamente comprovata
- Stato M1: malattia metastatica
- cT4 (coinvolgimento cutaneo o muscolare) o morbo di Paget del capezzolo
- Precedente radioterapia erogata all'interno di un programma accelerato o ipo-frazione
- Precedenti tumori maligni diversi dal cancro della pelle non melanoma, a meno che il paziente non sia libero da malattia da almeno 5 anni
- Pazienti con un piccolo volume mammario, tecnicamente insoddisfacente per un secondo intervento conservativo o irradiazione mammaria intraoperatoria.
- Chemioterapia preoperatoria o terapia ormonale per recidiva locale
- Malattia del tessuto connettivo o sclerodermia, che controindica la radioterapia
- Mutazione nota del gene BRCA1/2 (non è richiesto il test genetico)

Sperimentale: chirurgia e radioterapia intraoperatoria
Chirurgia : La tumorectomia verrà eseguita secondo gli standard attuali, ottenendo margini "netti". Il controllo dei linfonodi ascellari dipenderà dalla gestione iniziale (clinica ed ecografica) di questi pazienti N0, scelti dalle équipe. Radioterapia intraoperatoria (IORT): Dopo l'escissione del tumore, verrà erogata la IORT. Una singola dose di 20 Gy per 50 kV di fotoni (Intrabeam™) sarà somministrata nel letto tumorale. L'aggiunta di IORT non modifica la procedura chirurgica. |
La tumorectomia verrà eseguita secondo gli standard attuali, ottenendo margini "netti".
Il controllo dei linfonodi ascellari dipenderà dalla gestione iniziale (clinica ed ecografica) di questi pazienti N0, scelti dalle équipe.
Dopo l'escissione del tumore, verrà erogata la IORT.
Una singola dose di 20 Gy per 50 kV di fotoni (Intrabeam™) sarà somministrata nel letto tumorale.
