Radioterapia Intraoperatória Após Recidiva Local em Câncer de Mama (RE-IORT01)

Atualmente, a taxa de recorrência do tumor de mama ipsilateral (IBTR) após cirurgia conservadora da mama e radioterapia permanece em 10% em 10 anos a 15% em 20 anos, respectivamente. IBTR é um preditor independente de sobrevida pobre com um aumento de 3 a 4,6 no risco de morte relacionada ao câncer.
Numa população heterogénea, o controlo local continua a ser um grande desafio terapêutico para estas recidivas, especialmente aquelas consideradas de melhor prognóstico, nomeadamente tardias e de baixo grau histológico. O descalonamento terapêutico é possível para essas recaídas para evitar uma mastectomia mutilante e muitas vezes traumática.
No entanto, esta segunda cirurgia conservadora tem uma alta taxa de segunda recidiva local (19 a 50% em 5 anos) devido à ausência de re-irradiação, impossibilitada pelo problema de tolerância dos tecidos previamente irradiados.
Estudos retrospectivos ou prospectivos sobre irradiação parcial da mama (PBI) em ambiente adjuvante relatam resultados promissores, tanto em termos de tolerância (economizando tecido saudável) quanto de controle local (74% a 100% em 5 anos). As técnicas utilizadas incluem braquiterapia, radioterapia de feixe externo e radioterapia intraoperatória (IORT).
A IORT é agora objeto de interesse renovado no câncer de mama. Tem as vantagens da balística de alta precisão na área operada e da preservação do tecido saudável. Até o momento, nenhum dado prospectivo, no entanto, foi publicado na indicação de recorrência da mama ipsilateral.
Um projeto de Fase II prospectivo, multicêntrico e de braço único avaliará a viabilidade de cirurgia conservadora de mama repetida combinada com reirradiação usando IORT após recorrência local de carcinoma de mama.

Critério de inclusão:
- Recorrência de mama invasiva não lobular comprovada histologicamente
- Tempo desde a radiação de toda a mama após a mastectomia inicial > 5 anos
- tumor unifocal
- Tamanho do tumor recorrente ≤ 2 cm
- Tamanho adequado da mama para uma segunda cirurgia conservadora com resultado cosmético aceitável
- Mamografia de mama bilateral dentro de 90 dias antes da entrada no estudo
- RM de mama dentro de 90 dias antes da entrada no estudo
- Grau histológico I-II
- Tumor positivo para receptor de estrogênio (ER+)
- Tumor Cerb2-negativo
- N0
- M0
- Radioterapia prévia realizada dentro de um cronograma de fracionamento padrão
- Status de desempenho (ECOG) 0-1
- Mulheres ≥ 50 anos -Ausência de quaisquer condições psicológicas, familiares, sociológicas ou geográficas com potencial para dificultar o cumprimento do estudo e cronograma de acompanhamento
- Filiado ao regime de seguro de saúde francês
- Termo de consentimento informado escrito e assinado.
Critério de exclusão:
- Recorrência multifocal e/ou multicêntrica
- carcinoma lobular
- Tumor negativo para receptor de estrogênio (ER-)
- Cerb2 (her2) superexpresso - câncer de mama
- Extenso componente intraductal (EIC) na biópsia
- Invasão de vasos linfáticos na biópsia
- Status N1-3: Recorrência de linfonodo regional citológica ou histologicamente comprovada
- Status M1: doença metastática
- cT4 (envolvimento cutâneo ou muscular) ou doença de Paget do mamilo
- Radioterapia prévia administrada dentro de um cronograma acelerado ou de hipofração
- Malignidade anterior, exceto câncer de pele não melanoma, a menos que o paciente esteja livre de doença por pelo menos 5 anos
- Pacientes com volume mamário pequeno, tecnicamente insatisfatório para uma segunda cirurgia conservadora ou irradiação mamária intraoperatória.
- Quimioterapia pré-operatória ou terapia hormonal para recidiva local
- Doença do tecido conjuntivo ou esclerodermia, contraindicando radioterapia
- Mutação conhecida do gene BRCA1/2 (testes genéticos não são necessários)

Experimental: Cirurgia e Radioterapia Intraoperatória
Cirurgia : A tumorectomia será realizada de acordo com as normas vigentes, obtendo-se margens "limpas". O controle linfonodal axilar vai depender do manejo inicial (clínico e ultrassonográfico) desses pacientes N0, escolhido pelas equipes. Radioterapia Intraoperatória (IORT): Após a excisão do tumor, IORT será entregue. Uma dose única de fótons de 20 Gy por 50 kV (Intrabeam™) será administrada no leito tumoral. A adição de IORT não modifica o procedimento cirúrgico. |
