TAVOITTEET: I. Selvitä, parantaako hyperfraktiointi ja/tai nopeutettu fraktiointi (jaettu kurssi tai samanaikainen tehoste) paikallista kontrollia verrattuna tavalliseen fraktiointiin sädehoitoon potilailla, joilla on pitkälle edennyt pään ja kaulan okasolusyöpä. II. Määritä näiden potilaiden taudista vapaa eloonjääminen ja kokonaiseloonjääminen eri sädehoidon fraktiointimenetelmillä. III. Määritä kunkin fraktiointiohjelman akuutti ja myöhäinen toksisuus. IV. Vertaa elämänlaatua kahdella hoito-ohjelmalla.
YHTEENVETO: Satunnaistettu tutkimus. Käsivarsi I: Sädehoito. Primaarisen kasvaimen ja siihen liittyvien ja riskialttiiden solmualueiden säteilytys lineaarisilla kiihdyttimillä, joiden fotonienergia on 1,25-6,0 MV (kaksoisenergiajärjestelyt voivat myös käyttää yli 6,0 MV:n sädettä), 4-25 MV:n elektroneja tai Co60:a. Normaali fraktiointi. Käsivarsi II: Sädehoito. Kohteet ja varusteet kuten Arm I:ssä. Hyperfraktiointi. Käsivarsi III: Sädehoito. Kohteet ja varusteet kuten käsivarressa I. Nopeutettu jaetun kurssin hyperfraktiointi. Käsivarsi IV: Sädehoito. Kohteet ja varusteet kuten käsivarressa I. Nopeutettu fraktiointi ja samanaikainen tehostus.
ARVIOTETTU KERTYMINEN: Yhteensä 1 080 potilasta (270/haara) kertyy 5,75 vuoden aikana. Jos jossakin haarassa havaitaan liiallista toksisuutta 324 ja 634 potilaan tulon jälkeen, tämä käsi voidaan sulkea.
