Une étude de phase 2a, randomisée, contrôlée par placebo, en double aveugle à doses croissantes multiples chez des patients atteints de fibrose kystique porteurs de la mutation C-> T de 3849 + 10 Kb pour évaluer l'innocuité, la tolérabilité, la pharmacocinétique et l'efficacité préliminaire du SPL84

Le but de cet essai clinique est de savoir si le médicament SPL84 est sans danger pour les patients adultes atteints de mucoviscidose (FK). Il apprendra également si le médicament agit pour traiter la mucoviscidose avec une mutation spécifique.

Le but de cette étude de recherche est de :

  • tester l'innocuité et l'efficacité de doses multiples du médicament à l'étude, SPL84
  • tester comment plusieurs doses du médicament sont traitées par le corps

Les chercheurs compareront le médicament SPL84 à un placebo (une substance similaire qui ne contient aucun médicament) pour voir si le médicament SPL84 est sûr et s'il est efficace pour traiter la FK.

Les participants :

Prenez le médicament SPL84 ou un placebo par inhalation chaque semaine pendant 9 semaines. Visitez la clinique environ 14 fois sur 17,5 semaines pour des examens et des tests.

Critère d'intégration:

  • Diagnostic de mucoviscidose et deux mutations provoquant la mucoviscidose ; Mutation C->T de 3849+10 Kb sur un allèle du gène du régulateur de conductance transmembranaire (CFTR) de la CF (homozygote ou hétérozygote composé). Une documentation source provenant d’un laboratoire de génétique certifié est requise.
  • Indice de masse corporelle (IMC) ≥ 17 kg/m2.
  • FEV1 40 à 90 % prédit lors du dépistage.
  • Non-fumeurs ou vapoteurs pendant au moins 180 jours (6 mois) avant le dépistage, selon le rapport du participant.

Critère d'exclusion:

  • Utilisation de Kalydeco, Orkambi, Symdeko/Symkevi ou Trikafta/Kaftrio dans les 30 jours suivant la première dose avec intervention de l'étude.
  • Utilisation de tout médicament expérimental (autre que SPL84) ou dispositif dans les 30 jours suivant la première dose avec intervention de l'étude.
  • Utilisation de stéroïdes systémiques pendant 3 mois consécutifs au cours des 6 derniers mois précédant le dépistage, ou utilisation de stéroïdes systémiques au cours du dernier mois précédant le dépistage. Utilisation de stéroïdes inhalés supérieurs à 1 mg.
  • Utilisation de médicaments contre la FK, p. les antibiotiques inhalés, la dornase alfa (Pulmozyme), la solution saline hypertonique et la physiothérapie doivent suivre un régime stable pendant la période de 28 jours précédant le dépistage ; les participants prenant des antibiotiques inhalés à des fins prophylactiques doivent suivre un régime stable de ces médicaments pendant au moins 90 jours avant la première dose avec l'intervention de l'étude.
  • Toute infection aiguë, y compris les infections aiguës des voies respiratoires supérieures ou inférieures, une exacerbation pulmonaire, des changements dans le traitement d'une maladie pulmonaire ou toute maladie non liée à la mucoviscidose qui entraîne le début d'un nouveau traitement dans les 14 jours précédant la première dose avec intervention de l'étude.
  • Hémoptysie supérieure à 30 ml dans les 90 jours précédant le jour 1, ou hospitalisation pour hémoptysie dans les 6 mois suivant la première dose avec intervention de l'étude.
  • Maladie du foie caractérisée par une cirrhose cliniquement significative et/ou une hypertension portale documentée.
  • Antécédents de toute transplantation d'organe.
  • Infection documentée par la maladie à coronavirus (COVID-19) dans les 4 semaines précédant l'administration.
