尿素サイクル障害は、体内でタンパク質がどのように分解されるかに影響を与える、まれな遺伝性疾患のグループです。 UCD は、タンパク質代謝の老廃物であるアンモニアを血流から除去する役割を担う 6 つの酵素または 2 つのミトコンドリア膜輸送体のいずれかの欠乏によって引き起こされます。 通常、アンモニアは尿素に変換され、尿の形で体外に排出されます。 しかし、UCD では、アンモニアが抑制されずに蓄積され、体から除去されません。 その後、血液を介して脳に到達し、不可逆的な脳損傷および/または死を引き起こします。
1つ(オルニチントランスカルバミラーゼ欠損症)を除くすべてのUCDは、劣性形質として遺伝します。 この研究の目的は、さまざまな UCDs を持つ個人の大規模なグループの長期分析を実行することです。 生化学的状態、成長、および認知機能が評価されます。 最も一般的に使用されている 2 つの治療形態である代替経路療法と移植の生存と認知の転帰が評価されます。 さらに、この研究では、将来の代謝の不均衡を予測できる生化学的変化を特定し、臨床症状が現れる前に修正できるようにします。
この観察研究には、2024 年まで資金が提供されます。 すべての参加者は、最初の研究訪問に参加します。これには、病歴および食事歴、身体検査および神経学的検査、心理検査、および血液検査が含まれます。 参加者はその後、それぞれ2〜3時間続く次の研究訪問で追跡されます. 新生児発症 UCD の個人は、2 歳までは 3 か月ごと、その後は 6 か月ごとに評価されます。 遅発性UCDの個人は、6か月ごとに評価されます。 心理テストは2年ごとに行われます。 心理テストは、テスト バッテリーに応じて、30 分 (年少の子供の場合) から最大 3 時間かかります。

- 病原性変異の検出、および/または肝臓におけるNAGS酵素活性の低下(対照の20%未満)、および/または高アンモニア血症および第一度近親者がNAGS欠損症の基準の少なくとも1つを満たすこととして定義されるNAGS欠損症の診断
- CPS I 欠乏症の診断。肝臓の CPS I 酵素活性の低下 (対照の 20% 未満)、および/または特定された病原性変異、および/または高アンモニア血症および第一度近親者が CPS I の基準の少なくとも 1 つを満たすと定義される欠乏
- 病原性突然変異の同定、および/または肝臓におけるOTC活性の制御の20%未満、および/または無作為尿サンプル中の尿中オロテートの上昇(20 uM/mMを超える)として定義されるOTC欠乏症の診断、または-アロプリノールチャレンジテスト後、および/または高アンモニア血症および第一度近親者がOTC欠乏症の基準の少なくとも1つを満たしている
- 血漿中のシトルリンの10倍以上の上昇、および/または培養皮膚線維芽細胞または他の適切な組織におけるAS酵素活性の低下として定義されるAS欠乏症(シトルリン血症)の診断、および/またはAS遺伝子、および/または高アンモニア血症および第一度近親者がAS欠損症の基準の少なくとも1つを満たす

- ARG欠乏症(高アルギニン血症)の診断は、血中のアルギニンレベルが5倍以上上昇している、および/または赤血球または他の適切な組織のアルギナーゼ酵素レベルが低下している、および/または病原性変異の同定として定義されていますARG 遺伝子、および/または高アンモニア血症および第一度近親者が ARG 欠損症の基準の少なくとも 1 つを満たす
