アテローム性動脈硬化症は、先進国における障害および死亡の大部分の原因となっています。
以前の研究では、突然の臨床イベントがプラーク組成およびプラーク炎症の程度と高度に相関することが示されています。
これらの結果は、臨床現場で無症候性の高リスクプラークを検出するために、プラーク炎症の非侵襲的代理マーカーを開発することの重要性を強調しています。
ダイナミック コントラスト増強 (DCE) 磁気共鳴画像法 (MRI) および 18F フルオロデオキシグルコース (FDG) 陽電子放出断層撮影法 (PET) とコンピューター断層撮影法 (CT) の組み合わせは、アテローム性動脈硬化症における代謝活動 (解糖/炎症) の特徴付けと定量化に有望であることが示されています。動物およびヒト被験者のプラーク中の新生血管 (DCE-MRI) およびマクロファージなどの炎症細胞 (18F-FDG PET) の存在を標的とすることによって。

目的 1 では、研究者はアテローム性動脈硬化症の DCE-MRI のための新しいデュアル イメージング シーケンスを開発し、検証します。研究者は、時間分解能は高いが空間分解能は低く、AIF 画像と高空間分解能/低時間分解能の血管壁画像を取得します。プラーク内の造影剤の取り込みの正確な定量化を可能にします。
このアプローチは、アテローム性動脈硬化症のウサギ モデルと人間の被験者の両方で従来のアプローチと比較されます。
従来の PET スキャナーの限られた空間分解能は、部分体積効果 (PVE) のため、アテローム硬化性プラークの 18F-FDG PET 定量化の精度に影響を与えます。
異なる画像モダリティで取得した高解像度の解剖学的画像を使用した事後 PVE 補正方法は、定量化を改善できますが、別の画像モダリティと PET の間で正確な位置合わせが必要になるため、困難です。
MR は、電離放射線を使用せずに高解像度の解剖学的画像を生成するため、この 2 番目のイメージング モダリティに最適です。
したがって、複合型 MR/PET スキャナーは、新しい PVE 補正方法の開発により適している可能性があります。
目的 2 の一環として、研究者は、アテローム性動脈硬化プラークの代謝活性の定量化を改善するために、組み合わせた MR-PET(FDG) イメージング アプローチを開発および検証します。
MR に基づく減衰補正は、CT に基づく減衰補正と比較されます。
改善された PVE 補正とプラーク イメージングのための最適な循環時間へのアプローチも、ウサギと人間の両方で検証されます。
最後に、改善された DCE-MRI および MR-PET(FDG) から得られた画像パラメータは、頸動脈内膜切除術 (CEA) を受ける患者で検証され、画像とプラーク炎症の組織学的マーカーとの関係を確立するという主要なエンドポイントがあります。
さらに、研究者は、血清バイオマーカーとの関係 (存在する場合) を評価し、探索的エンドポイントとして、イメージングとプラーク脆弱性マーカーの遺伝子発現との関係をリアルタイム PCR によって研究します。

- 糸球体濾過率 <30mg/ml (造影MRI用)

DCE-MRI
FDA 承認の造影剤を使用する場合と使用しない場合の磁気共鳴画像法 (MRI): MRI は、体の内部構造を詳細に視覚化するために使用される非侵襲的な画像化技術です。
MRI 装置は、常にオンになっている特大の磁石です。
この研究では、アテローム硬化性プラークに関する解剖学的および機能的 (造影 MRI) 情報を提供するために使用されます。

PET/CTとPET/MR
陽電子放出断層撮影法 (PET)/コンピューター断層撮影法 (CT): PET は、体内の機能プロセスの画像を生成する核医学画像技術です。
このシステムは、生物学的に活性な分子で体内に導入された陽電子放出放射性核種 (トレーサー) によって間接的に放出されるガンマ線のペアを検出します。
現在、PET イメージングは、CT スキャナーなどの解剖学的イメージングと組み合わせると最も有用です。そのため、PET スキャナーは、統合されたハイエンドのマルチ検出器列 CT スキャナーで利用できるようになりました。
2 つのスキャンは、同じセッション中にすぐに順番に実行でき、患者は 2 つのスキャン間で位置を変えずに実行できるため、PET 画像の異常領域を CT 画像の解剖学的構造と直接関連付けることができます。
