このプロジェクトは、多角的な教育的介入戦略を使用して、人工呼吸器(MV)を受けている成人患者に90~96%の目標酸素飽和度(SpO2)範囲を実施するための、複数の病院によるクラスター無作為化研究となる。 介入を受けるための病院レベルのランダム化は、適格な人工呼吸器患者の目標SpO2範囲の順守を強化するための複合教育とリアルタイム情報ツールの段階的展開のために順次行われる。 この段階的な展開は、参加しているすべての病院の ICU が最終的に介入を実施する一方向クロスオーバー設計における段階的なウェッジ アプローチによって実現され、介入実施のタイミングはランダムに順序付けされます。 介入は 4 つのクラスターを使用して行われ、クラスターごとに 1 つずつ、事前に決定された 4 つの時点で制御から介入に切り替わります。 各切り替えについて、各クラスターでのスタッフの教育/導入のための 2 週間の移行期間が設計に組み込まれており、この期間中に、導入を改善するために各参加病院にトレーニングが提供されます。 私たちは、病院および ICU レベルでの実施のために、標準化された教育と資料を地元の関係者に提供します。 移行期間中、クラスターは構造化された通常のケア介入(教育ベースの介入)または非構造化の通常のケアコントロール(現在の状態、教育ベースの介入なし)のいずれかを受けていると見なすことができないため、患者は移行期間中に病院で治療を受けます。期間は登録されず、分析にも含まれません。
