Este projeto será um estudo multi-hospitalar randomizado de cluster para implementar uma faixa alvo de saturação de oxigênio (SpO2) de 90-96% em pacientes adultos recebendo ventilação mecânica (VM) usando uma estratégia de intervenção educacional multimodal. A randomização em nível hospitalar para receber a intervenção ocorrerá sequencialmente para uma implementação faseada de educação multimodal e ferramentas de informática em tempo real para aumentar a adesão à faixa alvo de SpO2 em pacientes elegíveis ventilados mecanicamente. Esta implementação faseada é alcançada através de uma abordagem escalonada num design cruzado unidirecional em que todas as UTIs hospitalares participantes irão finalmente implementar a intervenção, e o momento da implementação da intervenção é ordenado aleatoriamente. A intervenção ocorrerá por meio de 4 clusters que passam de controle para intervenção em 4 momentos pré-determinados, 1 para cada cluster. Para cada mudança, o projeto incorpora um período de transição de 2 semanas para educação/implementação da equipe em cada cluster, durante o qual o treinamento será ministrado a cada hospital participante para melhorar a implementação. Forneceremos educação e materiais padronizados às partes interessadas locais para implementação em nível hospitalar e de UTI. Durante o período de transição, o cluster não pode ser considerado como recebendo a intervenção de cuidados habituais estruturados (intervenção baseada na educação) ou o controlo de cuidados habituais não estruturados (estado actual, sem intervenção baseada na educação) e, portanto, os pacientes tratados no hospital durante esse período período não serão inscritos ou incluídos na análise.
Definimos uma faixa alvo de SpO2 (90-96%), PaO2 (60-100 mmHg) com base em limites definidos em trabalhos publicados recentemente que incorporam opiniões de especialistas da área e incluem consenso entre os membros do COGG de cuidados intensivos que representam hospitais UCHealth em todo o Regiões Norte, Sul e Metropolitana, bem como os locais onde a intervenção é proposta para ocorrer.2,3 A faixa alvo de oxigênio será definida por uma saturação de oxigênio (SpO2) de 90-96% e, quando a gasometria arterial estiver disponível, uma pressão arterial de oxigênio (PaO2) de 60-100 mmHg. A intervenção começará quando os pacientes que necessitam de ventilação mecânica chegarem a uma UTI em um hospital participante e a intervenção continuará por qualquer período de ventilação mecânica invasiva necessária durante a internação na UTI. A meta do projeto é alcançar a adesão a >95% das horas elegíveis do paciente gastas na faixa alvo de SpO2, excluindo o tempo em FiO2 30% quando acima de SpO2 96% ou o tempo em FiO2 100% quando abaixo de SpO2 90%. Cada unidade hospitalar contribuirá com dados de pré-implementação (controle) e pós-implementação (intervenção), com o início do período de intervenção baseado em consenso definido pelo tempo randomizado no design de cunha escalonada.
Os pacientes serão ventilados com um pedido de ventilação mecânica padronizado em toda a UCHealth para atingir essas metas de SpO2 e PaO2. Esses conjuntos de pedidos incluíram os braços de PEEP/FiO2 baixa e alta da ARDSNetwork, bem como protocolos desenvolvidos para modos alternativos de ventilação, como APRV, que pode ser usado a critério das equipes do provedor.
As intervenções educacionais incluirão:
- Materiais padronizados para discutir a justificativa e os aspectos processuais da intervenção, incluindo detalhes sobre as modificações específicas relacionadas à meta de oxigênio na ordem de ventilação mecânica definida antes de iniciar a fase de intervenção.
- Terapeuta respiratório virtual (vRT) - feedback direcionado em tempo real sobre a adesão à faixa alvo de SpO2 como parte do conjunto de pedidos de ventilação mecânica incorporado no UCHealth EHR (EPIC).
