うつ病の青少年におけるセルトラリンとラクトバチルス・クリスパトゥスの併用の有効性と安全性:多施設ランダム化比較試験
この臨床試験の目的は、12歳から18歳の大うつ病性障害を有する思春期患者を対象に、セルトラリンとLactobacillus crispatusを併用した際の有効性と安全性を評価することです。 主な研究課題は、うつ症状の軽減および感情症状の改善において、セルトラリンとLactobacillus crispatusの併用が、セルトラリンとプラセボ(プロバイオティクスを含まない外観類似物質)の併用よりも優れているかどうかを明らかにすることです。
比較群について:研究者は、セルトラリンとLactobacillus crispatusの併用と、セルトラリンとプラセボの併用を比較し、Lactobacillus crispatusの追加投与が、うつ病の思春期患者においてより大きな治療効果をもたらすかどうかを判定します。
参加者は以下のことを行います:1. セルトラリンとLactobacillus crispatus、またはセルトラリンとプラセボを8週間投与される;2. 4週間ごとに診療所を訪問し、臨床評価と安全性モニタリングを受ける

選定基準
- 年齢12歳から18歳まで;
- DSM-5における大うつ病性障害(MDD)の診断基準を満たし、キディ・スケジュール・フォー・アフェクティブ・ディスオーダーズ・アンド・スキゾフレニア(K-SADS-PL)面接により現在うつ病エピソードにあること;
- ベースライン時の小児うつ病評価尺度改訂版(CDRS-R)スコアが40以上;
- ベースライン時の臨床全般印象尺度(CGI-S)スコアが4以上。
除外基準
- DSM-5に基づく、不安障害以外の併存精神疾患の存在;
- 精神病症状を伴うMDD;
- ベースライン時のヤング躁病評価尺度(YMRS)スコアが13を超える;
- 神経疾患(例:てんかん、外傷性脳損傷)の既往歴、または重篤な身体疾患(例:甲状腺疾患、全身性エリテマトーデス、糖尿病、臨床的に有意な肺、肝、腎機能障害、または重度外傷)の存在;
- 異なる作用機序を持つ少なくとも2種類の抗うつ薬に対する無反応(各薬剤は適切な用量と期間で投与されたもの);
- セルトラリン不耐性の既往歴、または適切な治療コースでのセルトラリン無反応;
- 現在の高自殺リスク;
- アルコールまたは薬物乱用・依存の既往歴;
- 半減期の5倍以内に抗うつ薬の服用または投与;
- 登録前2週間以内のプロバイオティクス関連食品または医薬品の使用;
- 登録前1ヶ月以内の全身性抗菌薬または抗真菌薬の使用;
- 重度の食物アレルギーの既往歴、またはラクトバチルス・クリスパトゥス製剤またはその構成成分に対する既知の過敏症;
- 菜食主義またはその他の制限食パターンの遵守;
- 妊娠中または授乳中。
