반복적 경두개 자기 자극(rTMS)은 뇌 흥분성에 변화를 일으킬 수 있는 비침습적 뇌 자극 수단입니다. 일련의 일일 rTMS 세션에 이어 신경 회로 및 기타 원격 효과의 이러한 변조는 한 번에 몇 달 동안 PD의 일부 운동 및 신경 정신 증상을 도울 수 있습니다. 최근 FDA는 가짜 통제 시험과 그 안전성 프로파일에서 효능이 입증된 후 약물 불응성 우울증 치료제로 전전두엽 피질에 매일 rTMS를 승인했습니다. 포함 기준:
- 운동 장애에 대한 전문 지식을 갖춘 신경과 전문의가 확인한 UK Brain Bank Criteria에 따른 PD 진단.
- 파킨슨병의 정식 진단 후 최소 3년이 지났고 도파민 요법(최소한 레보도파 및/또는 도파민 작용제 요법)이 필요합니다.
- 모터 UPDRS의 최소 기준선 OFF 점수는 15점 이상입니다.
- 초기의 현저한 소뇌, 피라미드 또는 자율신경 기능장애와 같은 비정형 파킨슨증을 암시하는 기능의 결여; 핵상 시선 마비; 증상 발생 첫 해에 속함; 도파민 작용제를 시작하기 전에 환각.
- 지난 60일 동안 파킨슨증을 유발하는 신경이완제 또는 기타 약물의 병력이 없습니다.
- 현재 약물로 최적의 치료를 받고 있으며 치료하는 신경과 전문의의 관점에서 향후 6개월 동안 항 PD 약물 조정이 필요하지 않을 것입니다.
- 30일 동안 모든 약물을 안정적으로 복용합니다(최소 90일 동안 안정적이어야 하는 항우울제 제외).
- 등록 연구자의 관점에서 환자가 적절한 정보에 입각한 동의를 할 수 있는 치매가 없습니다. 또한 모든 환자는 스크리닝 MMSE에서 30점 만점에 최소 26점을 받아야 합니다.

- 자살 생각/시도로 인해 입원한 이력이 있는 환자.
- 지난 6개월 이내에 우울증으로 입원이 필요한 환자는 연구에 참여할 수 없습니다. 참여 피험자의 우울증이 연구 기간 동안 입원이 필요할 정도로 악화되거나 피험자가 심각한 자살 관념을 나타내면 연구에서 제외되고 치료를 위해 정신과 의사에게 의뢰됩니다.
