고화질 경두개 전기 자극에 의한 불안 장애에 대한 신경조절의 치료 효과
고화질 경두개 전기 자극(HD-tES)은 두피에 작은 전류를 적용하여 신경 흥분성을 변경하고 국소적인 뇌 활성화를 자극하는 비침습적 뇌 신경 조절 기술입니다. 이전 임상 시험에서는 우울증, 불안, 강박 장애, 외상 후 스트레스 장애와 같은 정신 건강 상태를 치료하기 위해 HD-tES를 사용하는 방법을 모색했습니다. 이 시험은 범불안장애(GAD) 환자의 불안 증상을 개선하는 데 있어 HD-tES의 효능과 안전성을 조사하여 불안 장애 치료제로서의 잠재력을 검증하는 것을 목표로 합니다.
참가자는 4가지 HD-tES 치료 그룹 중 하나에 무작위로 배정됩니다. (1) 오른쪽 배외측 전전두엽 피질(DLPFC)에 10분간 적용한 HD-tES 억제 파형(cDC+cTBS), 이어서 가짜 흥분성 파형(aDC+ iTBS) 자극은 왼쪽 DLPFC에 10분간 적용되었습니다. (2) 오른쪽 DLPFC에 10분 동안 적용된 가짜 억제 파형(cDC+cTBS) 자극, 이어서 왼쪽 DLPFC에 10분 동안 HD-tES 흥분성 파형(aDC+iTBS)이 적용되었습니다. (3) 오른쪽 DLPFC에 10분간 적용된 HD-tES 억제 파형(cDC+cTBS), 이어서 왼쪽 DLPFC에 10분간 적용된 HD-tES 흥분성 파형(aDC+iTBS). (4) 오른쪽 DLPFC에 10분 동안 적용된 가짜 억제 파형(cDC+cTBS) 자극, 이어서 왼쪽 DLPFC에 10분 동안 적용된 가짜 흥분성 파형(aDC+iTBS) 자극. 그룹 배정에 관계없이 참가자는 2주 동안 일주일에 5번의 세션, 하루에 1번 이하의 세션으로 치료 세션을 받게 됩니다. 각 세션은 약 20분 동안 진행됩니다. 평가는 치료 전, 치료 기간 동안 매주(1주차와 2주차 종료 시) 진행되며, 후속 평가는 치료 종료 후 1주 후에 실시된다.

포함 기준:
- 18세에서 65세 사이의 연령.
- DSM-5 기준에 따라 정신과 의사에 의해 범불안장애(GAD)로 진단되었습니다.
- 해밀턴 불안 평가 척도(HAM-A) 점수 ≥ 14.
- 해밀턴 우울증 평가 척도(HAM-D; 17개 항목 버전) 점수 ≤ 17.
- 등록 전 최소 6주 동안 지속적으로 안정적인 유형과 용량의 심리 상담 및/또는 약물 치료를 받아왔습니다. 또는 약물 치료 및/또는 심리 상담에 부적합하다고 간주되는 경우 또는 약물 치료 및/또는 심리 상담을 거부합니다.
제외 기준:
- 연구 전 2주 이내에 그리고 연구 기간 동안 rTMS 또는 기타 형태의 비침습적 뇌 자극 기술을 받은 경우.
- 심각한 신경 장애(예: 뇌졸중, 뇌종양, 간질, 기질성 뇌 질환) 또는 정신 장애(예: 정신분열증 및 기타 정신병 장애, 양극성 장애, 강박 장애, 기타 유형의 불안 장애, 약물 남용)가 있는 경우.
- 자율신경계 또는 중추신경계에 영향을 미칠 수 있는 심각하거나 불안정한 생리학적 상태(예: 급성 위장 질환, 심혈관 질환, 갑상선 질환).
- 심장 부정맥의 역사.
- 이식된 의료 전자 장치(예: 심장 박동기)의 존재.
- 머리 또는 목 부위에 금속 임플란트가 존재합니다.
- 전극 접촉 부위의 두피에 열린 상처.
- 의사의 평가에 근거하여 해당 개인을 포함하기에 부적합하게 만드는 두부 수술 또는 심각한 두부 외상의 병력.
- 상당한 자살 위험이 있는 개인(자살 위험에 대한 HAM-D 항목 3 점수 ≥ 3).
- 면역 장애(예: 전신성 홍반성 루푸스, 류마티스 관절염, 강직성 척추염, 염증성 장 질환)의 존재.
- 전기 자극에 대한 민감도가 비정상적이거나 높아져 이를 견딜 수 없는 개인.
- 임신(여성 참가자의 경우: 폐경기 이후이거나 수술로 불임 수술을 받아야 합니다. 가임기 여성은 임신 테스트 결과가 음성이어야 합니다. 임신할 수 있는 여성 참가자와 임신할 수 있는 여성 파트너가 있는 남성 파트너는 시험 기간 및 마지막 연구 개입 후 4개월 동안 경구 피임약, 이중 차단 방법 또는 자궁내 장치와 같은 효과적인 피임법을 사용하는 데 동의해야 합니다. 이 기간 동안 성행위를 삼가는 데 동의합니다. 비임신 여성은 양측 난소절제술을 받았거나 폐경기 이후인 여성으로 정의됩니다.
- 발작 역치를 낮추는 약물 복용.
