심장 재동기화 요법을 이용한 다기관 자동 제세동기 이식 시험
MADIT-CRT 시험은 이식형 심장 제세동기(ICD)와 심장 재동기화 요법(CRT-D)을 병용하는 것이 새로운 치료를 받는 피험자에서 사망 및 심부전(HF) 사건의 위험을 약 25% 감소시키는지 여부를 결정하기 위해 설계되었습니다. 비허혈성 또는 허혈성 심근병증이 있는 요크 심장 협회(NYHA) 기능적 클래스 II 및 허혈성 심근병증, 좌심실 기능 장애(박출률[EF] < 또는 = 0.30) 및 연장된 심실내 전도(QRS)가 있는 NYHA 기능적 클래스 I에 있는 피험자 기간 > 또는 = 130ms).

상세 설명
이 연구에서 피험자는 CRT-D 또는 ICD 단독으로 무작위 배정됩니다.
무작위화는 임상 센터 및 허혈 상태에 따라 계층화됩니다.
피험자의 약 60%는 무작위로 양심실 조율이 있는 CRT-D를 받고 40%는 ICD만 받게 됩니다.
심부전에 대한 최적의 약물 요법은 두 치료군 모두에서 필요합니다.
각 피험자에 대한 후속 조치 기간은 모든 피험자가 공통 연구 종료 날짜를 따르기 때문에 연구 시작 날짜에 따라 달라집니다.

다음과 같이 정의되는 허혈성 심장 질환:
- 등록 전과 등록 당시 지난 3개월 동안의 NYHA 클래스 I 또는 II,
- 하나 이상의 임상적으로 기록된(Q파 또는 효소 양성) 이전의 심근 경색, 그러나 등록 3개월 이내에는 아님; 및/또는
- 이전에 한 번 이상의 관상동맥 우회술 또는 경피적 관상동맥 중재술(풍선 및/또는 스텐트 혈관성형술)을 받았으나 등록 후 3개월 이내에는 하지 않았습니다.
또는
- 낮은 박출률과 증가된 심실 용적을 특징으로 하는 확장성 심근병증을 포함하는 비허혈성 심장 질환, 심실 순응도가 정상이거나 증가됨
- 등록 전과 등록 당시 지난 3개월 동안의 NYHA Class II
그리고 다음 모두:
- 안정적인 최적의 약물 요법.
- 등록 전 1년 이내에 혈관 조영술, 방사성 핵종 또는 심초음파 방법에 의한 박출률 < 또는 = 0.30 및 적격성을 확인하기 위해 무작위 배정 전 14일 이내에 획득한 등록 심초음파 검사 동안 측정(권장)
- 휴식 QRS 기간 > 또는 = 무작위화 전 14일 이내에 획득한 현재 심전도(ECG) 출력물에서 130ms.
- ECG에 의한 동리듬(우각차단[RBBB] 및 PR이 250ms 미만인 1도 심장차단 포함)
- 21세 이상 남녀(상위 구분 없음)
제외 기준:
- 기존 CRT 적응증
- 박동조율기를 이식한 피험자
- 기존 ICD 또는 CRT 장치가 있는 피험자
- 비허혈성 심근병증이 있는 NYHA 클래스 I 대상자
- 등록 전 또는 등록 시점에서 지난 3개월 동안 NYHA 클래스 III 또는 IV의 대상자
- 등록 전 지난 3개월 이내에 관상동맥 우회술 또는 경피적 관상동맥 중재술(풍선 및/또는 스텐트 혈관성형술)
- 등록 전 지난 3개월 이내에 효소 양성 심근 경색증
- 관상동맥 혈관재생술 대상자로서 관상동맥 우회로 이식 수술 또는 가까운 미래에 경피적 관상동맥 중재술을 받을 가능성이 있는 관상동맥 질환의 혈관 조영 증거가 있는 피험자
- 2도 또는 3도 심장 블록이 있는 피험자
- 기존의 뇌질환으로 비가역적인 뇌손상이 있는 피험자
- 시험 기간 동안 임신 중이거나 임신할 계획이 있는 여성. 가임 여성은 등록 전 7일 이내에 임신 테스트 결과가 음성이어야 합니다.
- 급성 바이러스성 심근염과 같은 가역적 비허혈성 심근병증 또는 알코올 유발성 심장 질환에서 알코올 중단
- 등록 전 1개월 이내의 만성 심방세동이 있는 피험자
- 피험자의 심장 질환 이외의 질병, 예를 들어 암, 요독증(혈액 요소 질소 [BUN] > 70 mg/dl 또는 크레아티닌 > 3.0 mg/dl ), 간부전 등
- 기타 임상시험에 참여하는 피험자
- 프로토콜에 협조하지 않거나 협조할 수 없는 피험자
- 후속 방문을 위해 이동하는 클리닉에서 멀리 떨어진 피험자는 비정상적으로 어려울 것입니다.
