Descrizione
Criterio di inclusione:
Cardiopatia ischemica definita come:
- Classe NYHA I o II per gli ultimi 3 mesi di calendario prima e al momento dell'iscrizione;
- uno o più precedenti infarti miocardici clinicamente documentati (onda Q o enzima positivo), ma non entro 3 mesi di calendario dall'arruolamento; e/o
- uno o più precedenti interventi chirurgici di innesto di bypass coronarico o interventi coronarici percutanei (angioplastica con palloncino e/o stent) ma non entro 3 mesi di calendario dall'arruolamento.
O
- Cardiopatia non ischemica inclusa la cardiomiopatia dilatativa caratterizzata da una bassa frazione di eiezione e aumento del volume ventricolare, con compliance ventricolare normale o aumentata
- Classe NYHA II per gli ultimi 3 mesi di calendario precedenti e al momento dell'iscrizione
E tutto quanto segue:
- Terapia farmacologica ottimale stabile.
- Una frazione di eiezione < o = 0,30 mediante metodi angiografici, radionuclidi o ecocardiografici entro un anno prima dell'arruolamento e misurata durante l'ecocardiogramma di arruolamento ottenuto entro 14 giorni prima della randomizzazione per confermare l'idoneità (consigliato)
- Durata del QRS a riposo > o = 130 ms sulla stampa di un elettrocardiogramma (ECG) corrente ottenuto entro 14 giorni prima della randomizzazione.
- Ritmo sinusale all'ECG (inclusi blocco di branca destra [RBBB] e blocco cardiaco di primo grado con PR < 250 ms.)
- Uomini e donne di età pari o superiore a 21 anni (nessun limite di età superiore)
Criteri di esclusione:
- Indicazione esistente per CRT
- Soggetti con un pacemaker impiantato
- Soggetti con un dispositivo ICD o CRT esistente
- Soggetti in classe NYHA I con cardiomiopatia non ischemica
- Soggetti in classe NYHA III o IV negli ultimi 3 mesi di calendario prima o al momento dell'iscrizione
- Intervento chirurgico di innesto di bypass coronarico o intervento coronarico percutaneo (angioplastica con palloncino e/o stent) negli ultimi 3 mesi di calendario prima dell'arruolamento
- Infarto del miocardio enzima-positivo negli ultimi 3 mesi di calendario prima dell'arruolamento
- Soggetti con evidenza angiografica di malattia coronarica che sono candidati alla rivascolarizzazione coronarica e che potrebbero essere sottoposti a intervento chirurgico di bypass coronarico o intervento coronarico percutaneo nel prossimo futuro
- Soggetti con blocco cardiaco di secondo o terzo grado
- Soggetti con danno cerebrale irreversibile da malattia cerebrale preesistente
- Donne che sono incinte o stanno pianificando una gravidanza durante il corso della sperimentazione. Le donne in età fertile devono avere un test di gravidanza negativo entro 7 giorni prima dell'arruolamento.
- Cardiomiopatia non ischemica reversibile come miocardite virale acuta o interruzione dell'alcol nelle cardiopatie indotte dall'alcol
- Soggetti con fibrillazione atriale cronica entro un mese prima dell'arruolamento
- Presenza di qualsiasi malattia, diversa dalla malattia cardiaca del soggetto, associata a una ridotta probabilità di sopravvivenza per la durata dello studio, ad esempio cancro, uremia (azoto ureico nel sangue [BUN] > 70 mg/dl o creatinina > 3,0 mg/dl ), insufficienza epatica, ecc.
- Soggetti che partecipano ad altri studi clinici
- Soggetti che non vogliono o non possono collaborare con il protocollo
- I soggetti che vivono a una tale distanza dalla clinica che viaggiano per le visite di follow-up sarebbero insolitamente difficili
- Soggetti che non prevedono di essere residenti nell'area per la durata prevista della sperimentazione
- Soggetti non disposti a firmare il consenso alla partecipazione
