Kryteria przyjęcia:
Choroba niedokrwienna serca zdefiniowana jako:
- NYHA klasa I lub II w ciągu ostatnich 3 miesięcy kalendarzowych przed iw momencie rejestracji;
- jeden lub więcej klinicznie udokumentowanych (zawał Q lub enzym dodatni) wcześniejszych zawałów mięśnia sercowego, ale nie w ciągu 3 miesięcy kalendarzowych od włączenia; i/lub
- co najmniej jedna wcześniejsza operacja pomostowania aortalno-wieńcowego lub przezskórna interwencja wieńcowa (angioplastyka balonowa i/lub stentowa), ale nie w ciągu 3 miesięcy kalendarzowych od rejestracji.
LUB
- Nieniedokrwienna choroba serca, w tym kardiomiopatia rozstrzeniowa charakteryzująca się niską frakcją wyrzutową i zwiększoną objętością komór, z prawidłową lub zwiększoną podatnością komór
- NYHA Class II w ciągu ostatnich 3 miesięcy kalendarzowych przed i w momencie rejestracji
ORAZ wszystkie poniższe:
- Stabilna optymalna terapia farmakologiczna.
- Frakcja wyrzutowa < lub = 0,30 metodami angiograficznymi, radionuklidowymi lub echokardiograficznymi w ciągu jednego roku przed włączeniem i mierzona podczas echokardiogramu rejestracyjnego uzyskanego w ciągu 14 dni przed randomizacją w celu potwierdzenia kwalifikacji (zalecane)
- Czas spoczynkowego zespołu QRS > lub = 130 ms na wydruku aktualnego elektrokardiogramu (EKG) uzyskanego w ciągu 14 dni przed randomizacją.
- Rytm zatokowy na podstawie EKG (w tym blok prawej odnogi pęczka Hisa [RBBB] i blok przedsionkowo-komorowy pierwszego stopnia z PR < 250 ms.)
- Mężczyźni i kobiety w wieku 21 lat lub starsi (bez górnej granicy wieku)
Kryteria wyłączenia:
- Istniejące wskazanie do CRT
- Osoby z wszczepionym rozrusznikiem serca
- Pacjenci z istniejącym urządzeniem ICD lub CRT
- Pacjenci w I klasie NYHA z kardiomiopatią inną niż niedokrwienna
- Przedmioty w klasie III lub IV NYHA w ciągu ostatnich 3 miesięcy kalendarzowych przed rejestracją lub w momencie rejestracji
- Operacja pomostowania aortalno-wieńcowego lub przezskórna interwencja wieńcowa (angioplastyka balonowa i/lub stentowa) w ciągu ostatnich 3 miesięcy kalendarzowych przed rejestracją
- Zawał mięśnia sercowego z dodatnim wynikiem testu enzymatycznego w ciągu ostatnich 3 miesięcy kalendarzowych przed włączeniem do badania
- Pacjenci z angiograficznymi dowodami choroby wieńcowej, którzy są kandydatami do rewaskularyzacji wieńcowej i prawdopodobnie zostaną poddani operacji pomostowania aortalno-wieńcowego lub przezskórnej interwencji wieńcowej w dającej się przewidzieć przyszłości
- Osoby z blokiem serca drugiego lub trzeciego stopnia
- Osoby z nieodwracalnym uszkodzeniem mózgu w wyniku istniejącej wcześniej choroby mózgu
- Kobiety, które są w ciąży lub planują zajść w ciążę w trakcie badania. Kobiety w wieku rozrodczym muszą mieć negatywny wynik testu ciążowego w ciągu 7 dni przed przyjęciem.
- Odwracalna kardiomiopatia niezwiązana z niedokrwieniem, taka jak ostre wirusowe zapalenie mięśnia sercowego lub odstawienie alkoholu w chorobie serca wywołanej alkoholem
- Pacjenci z przewlekłym migotaniem przedsionków w ciągu jednego miesiąca przed włączeniem
- Obecność jakiejkolwiek choroby innej niż choroba serca pacjenta, związanej ze zmniejszonym prawdopodobieństwem przeżycia w czasie trwania badania, np. nowotworu, mocznicy (azot mocznikowy we krwi [BUN] > 70 mg/dl lub kreatynina > 3,0 mg/dl) ), niewydolność wątroby itp.
- Osoby biorące udział w jakichkolwiek innych badaniach klinicznych
- Podmioty nie chcą lub nie mogą współpracować z protokołem
- Osobom mieszkającym w takiej odległości od kliniki, że dojazd na wizyty kontrolne byłby niezwykle utrudniony
- Osoby, które nie przewidują, że będą mieszkańcami danego obszaru w zaplanowanym czasie trwania badania
- Osoby, które nie chcą podpisać zgody na udział
