Beschreibung
Einschlusskriterien:
Ischämische Herzkrankheit definiert als:
- NYHA Klasse I oder II für die letzten 3 Kalendermonate vor und zum Zeitpunkt der Einschreibung;
- ein oder mehrere klinisch dokumentierte (Q-Wellen- oder Enzym-positiv) frühere Myokardinfarkte, jedoch nicht innerhalb von 3 Kalendermonaten nach Einschreibung; und/oder
- eine oder mehrere vorherige Koronararterien-Bypassoperationen oder perkutane Koronarinterventionen (Ballon- und/oder Stent-Angioplastie), jedoch nicht innerhalb von 3 Kalendermonaten nach der Einschreibung.
ODER
- Nicht-ischämische Herzerkrankung, einschließlich dilatativer Kardiomyopathie, gekennzeichnet durch eine niedrige Ejektionsfraktion und ein erhöhtes Kammervolumen bei normaler oder erhöhter ventrikulärer Compliance
- NYHA Klasse II für die letzten 3 Kalendermonate vor und zum Zeitpunkt der Einschreibung
UND alles Folgende:
- Stabile optimale pharmakologische Therapie.
- Eine Ejektionsfraktion < oder = 0,30 durch angiographische, radionuklidische oder echokardiographische Methoden innerhalb eines Jahres vor der Einschreibung und gemessen während des Einschreibungs-Echokardiogramms, das innerhalb von 14 Tagen vor der Randomisierung erstellt wurde, um die Eignung zu bestätigen (empfohlen)
- QRS-Ruhedauer > oder = 130 ms auf dem Ausdruck eines aktuellen Elektrokardiogramms (EKG), das innerhalb von 14 Tagen vor der Randomisierung erstellt wurde.
- Sinusrhythmus durch EKG (einschließlich Rechtsschenkelblock [RBBB] und Herzblock ersten Grades mit PR < 250 ms.)
- Männer und Frauen ab 21 Jahren (keine Obergrenze)
Ausschlusskriterien:
- Bestehende Indikation für CRT
- Probanden mit einem implantierten Herzschrittmacher
- Probanden mit einem vorhandenen ICD- oder CRT-Gerät
- Patienten der NYHA-Klasse I mit nicht-ischämischer Kardiomyopathie
- Probanden der NYHA-Klasse III oder IV in den letzten 3 Kalendermonaten vor oder zum Zeitpunkt der Einschreibung
- Koronararterien-Bypassoperation oder perkutane Koronarintervention (Ballon- und/oder Stent-Angioplastie) innerhalb der letzten 3 Kalendermonate vor der Einschreibung
- Enzym-positiver Myokardinfarkt innerhalb der letzten 3 Kalendermonate vor der Einschreibung
- Patienten mit angiographischem Nachweis einer Koronarerkrankung, die Kandidaten für eine koronare Revaskularisierung sind und sich wahrscheinlich in absehbarer Zeit einer Koronararterien-Bypass-Operation oder einer perkutanen Koronarintervention unterziehen werden
- Patienten mit Herzblock zweiten oder dritten Grades
- Personen mit irreversiblen Hirnschäden aufgrund einer vorbestehenden zerebralen Erkrankung
- Frauen, die während der Studie schwanger sind oder eine Schwangerschaft planen. Frauen im gebärfähigen Alter müssen innerhalb von 7 Tagen vor der Einschreibung einen negativen Schwangerschaftstest haben.
- Reversible nicht-ischämische Kardiomyopathie wie akute virale Myokarditis oder Absetzen von Alkohol bei alkoholinduzierter Herzerkrankung
- Probanden mit chronischem Vorhofflimmern innerhalb eines Monats vor der Einschreibung
- Vorhandensein einer anderen Krankheit als der Herzerkrankung des Probanden, die mit einer verringerten Überlebenswahrscheinlichkeit für die Dauer der Studie verbunden ist, z. B. Krebs, Urämie (Blut-Harnstoff-Stickstoff [BUN] > 70 mg/dl oder Kreatinin > 3,0 mg/dl ), Leberversagen usw.
- Probanden, die an anderen klinischen Studien teilnehmen
- Subjekte, die nicht bereit oder nicht in der Lage sind, mit dem Protokoll zu kooperieren
- Probanden, die so weit von der Klinik entfernt wohnen, dass Reisen für Nachsorgeuntersuchungen ungewöhnlich schwierig wären
- Probanden, die voraussichtlich nicht für die geplante Dauer der Studie in der Gegend wohnen
- Probanden, die nicht bereit sind, die Zustimmung zur Teilnahme zu unterzeichnen

MADIT-CRT war eine ereignisgesteuerte Studie, in der die Patienten auf Gesamtmortalität und Herzinsuffizienz-Ereignisse überwacht wurden.
