포함 기준:
- 혈액 악성 종양 또는 고형 종양 진단을 받은 성인 및 소아 환자(2세 이상).
- 항암화학요법 또는 골수이식(BMT) 후 호중구감소증(<500PN)이 발생하는 환자는 센터에 따라 경험적 광범위 항진균제 치료를 받을 수 있다.
- 매주 2회 혈중 갈락토만난이 있는 환자를 통제했습니다.
- 광범위한 스펙트럼의 경험적 항균 요법, 가능한 한 이 순간에 활성화되는 Pethema 프로토콜. 또한 4세대 또는 3세대 카바페네믹 또는 세팔로스포린 단독요법 또는 바이오요법도 검증될 예정이다.
- 화학 요법 또는 준비 요법 시작 이후 환자 포함.
- 세균 감염이 문서화되면 경험적 항진균제 치료를 시작하기 전에 치료 및 통제됩니다.
- 정보에 입각한 동의서에 서명했습니다.
- 가임 환자의 음성 임신 검사
제외 기준:
- Aspergillus에 대한 활성이 있는 트리아졸과 함께 항진균 예방을 사용하거나 이전의 침입자 진균 감염 또는 기타 이유로 인한 다른 체계적 항진균제를 사용합니다.
- 1일 100mg보다 높은 용량의 플루코나졸을 예방적으로 사용하십시오.
- 아졸에 대한 알레르기
- 열을 동반한 호중구감소증이 시작될 때 침입자 진균 감염이 있는 것.
- 플루코나졸에 강한 칸디다균의 격리에 높은 효과가 있다는 것은 센터의 의견으로는 경험적 플루코나졸의 사용이 고려되는 프로토콜에 포함하는 것이 적절하지 않다는 것입니다.
- 재생 불량성 빈혈 또는 유사한 뼈의 다른 결함으로 인한 호중구 감소증.

- 호중구감소증의 이 에피소드에서 Aspergillus, C.krusei 또는 C.gladiata에 의해 정착했거나 현재 에피소드에서 경계 문화의 결과가 부족한 경우 환자는 제외됩니다. 이러한 병원균에 대해 양성 결과를 나타내면 경험적 치료는 플루코나졸이 아닌 좋은 효과를 내는 항진균제(암포테리신, 카스포펑진 또는 보리코나졸)를 사용해야 합니다. 그러면 환자는 이 프로토콜을 따르지 않을 것입니다.
- 보리코나졸 및/또는 플루코나졸로 치료 중인 환자에게 필요하지 않은 약물을 받기 위해.
- 환자가 퀴놀론, 마크로라이드 등 또는 자극인자 G-CSF, GM-CSF 또는 이와 유사한 경구용 항균 예방요법을 받는 경우 제외되지 않습니다.

- 마지막으로 침입자 진균 감염의 중요한 위험이 있는 단기 체류의 호중구 감소증이 있습니다. 이 데이터는 환자가 연구에 포함되는 순간에는 자연스럽게 알 수 없습니다. 5일 미만 지속되는 경우 단기 체류의 호중구 감소증으로 정의합니다.
