Raiing 무선 액와 온도계의 수술 전후 정확도
Raiing Medical의 새로운 무선 겨드랑이 온도계는 독점 시스템인 iThermonitor(WT701)를 사용하여 기존의 겨드랑이 탐침보다 더 나은 심부 온도 추정치를 제공합니다. 겨드랑 프로브가 4초마다 지속적으로 온도를 측정 및 기록하고 주변 온도와 팔 외전을 포함한 위치 변화를 보상하는 소프트웨어가 포함되어 있기 때문에 부분적으로 개선이 이루어집니다.
iThermonitor가 임상용으로 충분히 정확한지 여부는 아직 알려지지 않았습니다. 따라서 연구자들은 종종 몇 °C 범위에서 열적 동요를 경험하는 수술 전후 환자의 시스템을 평가할 것을 제안합니다. 특히, 연구자들은 수술 환자와 초기 회복 시간 동안 iThermonitor의 정밀도와 정확성을 결정할 것을 제안합니다. 이전 연구에서와 같이 연구자들은 대부분의 Raiing 온도가 기준 온도의 ±0.5°C 이내인 경우 임상 사용에 충분히 정확한 온도계를 고려할 것입니다.

상세 설명
연령, 신장, 체중, 성별, ASA 상태를 포함한 환자의 특성과 절차 및 수술 후 진단을 포함한 수술의 세부 사항이 기록됩니다. 수사관은 마취 유도 및 출현 시간을 기록합니다. 수술 중 10분 간격으로 조사자는 흡입된 휘발성 마취제 농도, 평균 동맥압, 열 관리 장치 유형 및 이전 10분 간격 동안의 소변 배출을 기록합니다.
전신 마취(유형에 대한 제한 없음)를 유도한 후 마취 전문의는 원위 식도에 온도 센서를 삽입합니다. 거리는 최대 심음(청진기를 사용하는 경우)으로 결정하거나 탐침을 0.48에 삽입합니다. 높이) - 4.4cm. 식도 프로브를 사용할 수 없는 경우 온도계를 비인두로 10~20cm 삽입할 수 있습니다. 수술 팀은 온도 센서가 있는 폴리 카테터를 방광에 배치합니다. 주변 온도는 열을 발생시키는 장비에서 멀리 떨어진 환자의 키에 위치한 전자 탐침에서 기록됩니다.
겨드랑이 온도는 iPhone과 페어링된 Raiing Medical 무선 모듈인 iThermonitor에 의해 기록됩니다. Raiing에서 제공하는 접착 패치는 마취 유도 전에 면도한 겨드랑이에 Railing 프로브를 단단히 배치하는 데 사용됩니다. 프로브를 삽입한 후 최대 5분 동안 또는 페어링된 iPhone에 표시된 온도가 안정될 때까지 환자에게 동측 팔을 내전하도록 요청합니다. 그 후 환자는 팔을 자유롭게 움직일 수 있습니다. 수술 중 모니터링되는 팔 위치가 기록됩니다(팔을 단단히 집어넣고, 팔을 옆으로 느슨하게 하고, 팔을 외전).

- 미국마취학회(ASA) 신체 상태가 1-3인 성인
- 1.5-4시간 동안 지속될 것으로 예상되는 복부 또는 골반 수술 일정을 잡아야 합니다.

제외 기준:
- 식도 또는 비인두 온도 모니터링이 실용적이지 않은 환자
- 양팔에 능동정맥주사가 필요한 환자

기준 온도와 iThermonitor 온도 간의 실제 차이로 정의되는 정확도. 식도 온도는 수술 중 핵심 온도 기준이 됩니다. 1차 결과 측정은 참조의 0.5°C 내에 있는 환자에 대한 iThermonitor 측정의 비율이 계산되고 이 비율(중앙값, 사분위수, 95% CI)이 환자 전체에 걸쳐 요약됩니다. 2차 결과 측정은 바이어스이며 반복 측정 Bland-Altman 분석에 의해 결정된 95% CI입니다. 각 환자에 대해 iThermometer와 기준 온도 간의 평균 차이가 계산됩니다. 환자 iThermonitor 측정값은 iThermonitor와 참조 사이의 Pearson 상관관계 및 95% CI를 평가하고 장치 대 참조의 편향(기울기)을 추정하기 위해 환자 참조 측정에서 회귀됩니다. |
