연구 개요
상세 설명
이번 임상시험은 최대 4주간의 스크리닝 기간, 32주간의 치료 기간, 최종 임상시험용 제품 투여 후 20주간의 안전성 추적 기간을 포함한다.
이 임상 시험은 파킨슨병 진단을 받은 적격 참가자 60명을 등록하기 위해 공개됩니다. 환자 모집은 연구 팀에 의해 수행되며, 적격성 기준에 따라 적격 참가자가 확인되면 스크리닝 방문 일정이 잡힙니다. 정보에 입각한 동의서가 연구 참여자에게 제공되고 연구 절차 전에 서명됩니다. 정보에 입각한 동의서에는 임상 시험에 대한 정보가 포함되며 이 과정에서 몇 가지 측면을 고려해야 합니다.
사전 동의를 얻은 후 각 참가자는 다음 방문을 완료해야 합니다.
- 방문 1 - 스크리닝, 이 방문 동안 주임 조사관은 스크리닝된 참가자가 자격이 있는지 여부와 다음 방문 일정을 잡을 수 있는지 여부를 결정합니다. 일단, 주 조사관이 스크리닝된 피험자의 적격성을 평가하면(최대 28일) 적격 피험자를 동종이계 HB-adMSC 또는 위약으로 할당하기 위해 무작위화 프로세스가 수행됩니다. 무작위화는 적격 대상자에게만 적용됩니다. 연구 참가자가 스크리닝 과정에서 포함 및 제외 기준을 충족하지 않는 경우, 그/그녀는 스크리닝 실패(SF)로 간주되며 무작위 배정이 필요하지 않습니다.
- 방문 2 - 주입 1, (기준선): 이 방문은 참가자의 데이터 비교를 위한 시작점으로 사용됩니다. 이 방문 동안 적격 연구 참가자는 약물 노출 후 총 2시간 동안 활력 징후 모니터링과 함께 첫 번째 조사 제품 투여 또는 위약을 받게 됩니다. 다른 연구 평가는 이번 방문의 일부로 완료될 것입니다.

- 방문 6 - 주입 5: 초기 조사 제품 투여 후 약 16주에 이 방문을 완료해야 합니다. 다른 연구 평가는 이번 방문의 일부로 완료될 것입니다.
- 방문 7 - 주입 6: 초기 연구 제품 투여 후 약 20주에 이 방문을 완료해야 합니다. 다른 연구 평가는 이번 방문의 일부로 완료될 것입니다.
- 전화 통화 - 안전성 후속 조치: 초기 연구 제품 투여 후 약 24주 후에 활성 연구 참가자가 전화 통화 후속 조치를 완료합니다.
- 전화 통화 - 안전성 후속 조치: 초기 연구 제품 투여 후 약 32주 후에 활성 연구 참가자가 전화 통화 후속 조치를 완료합니다.
- 방문 8 - 연구 종료, 이 최종 방문 동안(0주 후 약 52주) 연구 평가의 전체 그룹이 동종이계 HB-adMSC 또는 위약 투여의 안전성 및 효능을 평가하기 위해 수행될 것입니다.

포함 기준:
연구 참가자는 다음 기준이 모두 적용되는 경우에만 이 연구에 포함될 수 있습니다.
- 45~80세의 남녀 참가자.
- 스크리닝 방문에서 연구 참여자는 7~28점 사이의 MDS-UPDRS 파트 II 점수를 가져야 합니다.
- 연구 참가자는 스크리닝 방문 동안 20에서 57 사이의 MDS-UPDRS 파트 III 점수를 가져야 합니다.
- Carbidopa/Levodopa 총 용량은 연구 참가자의 경우 하루 1200mg 미만이어야 합니다.
- 연구 참가자의 총 레보도파 등가 용량은 하루 1400mg 미만이어야 합니다.
- 연구 참여자는 연구 참여 최소 2년 전에 조기 및/또는 중등도 파킨슨병 진단을 받았어야 합니다.
- 연구 참여자는 읽고 이해하고 서면 동의를 제공할 수 있어야 합니다.
- 임상 시험 관련 절차를 수행하기 전에 자발적으로 서명한 정보에 입각한 동의서.
- 여성 연구 참가자는 연구 참여 기간 및 마지막 연구 제품 투여 후 6개월 동안 임신하지 않았거나 임신할 계획이 없어야 합니다.
- 성 파트너가 임신할 수 있는 남성 참가자는 연구 참여 기간과 조사 제품의 마지막 투여 후 6개월 동안 피임 방법을 사용해야 합니다.
- 연구 참가자는 이 임상 시험의 요구 사항을 준수할 수 있고 준수할 의향이 있습니다.
제외 기준:
연구 참가자는 다음 기준 중 하나라도 적용되는 경우 이 임상 시험에 포함될 자격이 없습니다.
- 임신, 수유. 임신하지 않았지만 효과적인 피임 조치를 취하지 않는 가임기 여성.
- 중증 장애, 휠체어에 묶인 상태 또는 침대에 누워 있는 상태로 설명된 진행성 파킨슨병을 가진 연구 참가자.
- 연구 참가자는 피부 기저, 편평 세포 암종 또는 흑색종의 증거를 포함하여 활동성 악성 종양이 있습니다.
- 연구 참가자는 알코올 중독 또는 의존성을 알고 있거나 현재 약물 사용 또는 남용이 있습니다.
연구 참가자는 다음을 포함하여 1개 이상의 중요한 동시 의학적 상태(의료 기록으로 확인됨)를 가지고 있습니다.
- 제대로 조절되지 않는 진성 당뇨병(PCDM)은 치료 기준 치료의 부족 및/또는 스크리닝 방문 동안의 식전 포도당 >130mg/dl 또는 식후 포도당 >200mg/dl의 이력으로 정의됩니다.
- 만성 신장 질환(CKD) 진단 병력 및/또는 eGFR < 59mL/min/1.73m2의 스크리닝 결과.
- 스크리닝 방문 중 뉴욕 심장 협회(NYHA) 클래스 III/IV 심부전의 존재.
- ST 상승 심근 경색(STEMI) 또는 비 ST 상승 심근 경색(NSTEMI), 관상 동맥 연축 또는 불안정 협심증과 같은 다양한 유형의 심근 경색 병력.
- 관리 치료 기준 부족 및/또는 스크리닝 방문 중 혈압 > 180/120 mm/Hg로 정의되는 조절되지 않는 고혈압의 병력.
- 유전성 혈전성향증 병력, 최근 대수술(검진 전 12개월 이내), 척수 손상으로 인한 하지 마비, 골반, 고관절 또는 대퇴골 골절, 폐암, 뇌암, 림프암, 부인과암( 난소 또는 자궁) 또는 위장관(예: 췌장 또는 위).
- 파킨슨병에 대한 뇌 수술의 역사.
- 연구 참가자는 희망 바이오사이언스에서 생산한 줄기 세포 이외의 첫 번째 연구 제품 투여 전 6개월 이내에 줄기 세포 치료를 받았습니다.
- COVID-19 백신 이외의 시험용 제품의 첫 투여 전 1년 이내에 시험용 요법 또는 시험용으로 승인된 요법을 받은 경우.
연구 참여자는 스크리닝 중에 다음을 포함하는 검사실 이상이 있습니다.
- 백혈구 수 < 3000/mm3
- 혈소판 수 < 80,000mm3
- 절대 호중구 수 < 1500/mm3
- ALT(Alanine aminotransferase) 또는 AST(aspartate aminotransferase) 10 정상 상한(ULN) x 1.5
- 연구 참가자는 연구자의 의견에 따라 안전 위험을 제기하거나 피험자가 연구를 완료하는 데 방해가 되는 다른 실험실 이상 또는 의학적 상태를 가지고 있습니다.
- 연구 참가자는 연구를 완료하거나 연구 절차를 준수할 가능성이 없습니다.
- 동시 급성 또는 만성 바이러스성 B형 간염 또는 C형 간염 또는 인간 면역결핍 바이러스(HIV) 감염이 알려진 연구 참여자.
- 연구 참가자는 조사관의 의견에 따라 자기 평가에 영향을 미칠 수 있는 이전에 진단된 정신 질환을 가지고 있습니다.
- 연구 제품의 첫 투여 전 30일 이내에 항생제, 항바이러스제 또는 항진균제로 치료가 필요한 전신 감염이 있는 연구 참여자.
