Klinische proef voor de ziekte van Parkinson met behulp van allogene HB-adMSC's (vroege en matige PD)

Gedetailleerde beschrijving
Het onderzoek omvat een screeningperiode van maximaal 4 weken, een behandelingsperiode van 32 weken en een follow-upperiode van 20 weken na de laatste toediening van het onderzoeksproduct.
Deze klinische proef staat open voor inschrijving van 60 in aanmerking komende deelnemers met de diagnose van de ziekte van Parkinson. De werving van patiënten zal worden uitgevoerd door het onderzoeksteam. Als in aanmerking komende deelnemers worden geïdentificeerd op basis van geschiktheidscriteria, wordt een screeningbezoek gepland. Het geïnformeerde toestemmingsformulier zal worden overhandigd aan de studiedeelnemers en ondertekend vóór enige studieprocedures. Het formulier voor geïnformeerde toestemming bevat informatie over de klinische proef en sommige aspecten moeten tijdens dit proces in overweging worden genomen.
Nadat geïnformeerde toestemming is verkregen, moet elke deelnemer de volgende bezoeken afleggen.
- Bezoek 1 - Screening, tijdens dit bezoek neemt de hoofdonderzoeker de beslissing om te bepalen of de gescreende deelnemer in aanmerking komt en of het volgende bezoek kan worden ingepland. Zodra de hoofdonderzoeker de geschiktheid van de gescreende proefpersoon heeft beoordeeld (tot 28 dagen), zal een randomisatieproces worden uitgevoerd om de in aanmerking komende proefpersoon allogene HB-adMSC's of placebo toe te wijzen. Randomisatie is alleen van toepassing op in aanmerking komende proefpersonen. Als een studiedeelnemer tijdens het screeningproces niet voldoet aan de in- en exclusiecriteria, wordt hij/zij beschouwd als Screen Failure (SF) en is randomisatie niet vereist.
- Bezoek 2 - Infusie 1, (basislijn): dit bezoek wordt gebruikt als startpunt voor vergelijking van de gegevens van de deelnemers. Tijdens dit bezoek krijgen in aanmerking komende studiedeelnemers zijn/haar eerste toediening van het onderzoeksproduct of placebo met monitoring van vitale functies gedurende in totaal 2 uur na blootstelling aan het geneesmiddel. Andere studie-evaluaties zullen als onderdeel van dit bezoek worden afgerond.
- Bezoek 3 - Infusie 2: ongeveer 4 weken na de eerste toediening van het onderzoeksproduct moet dit bezoek worden afgerond. Andere studie-evaluaties zullen als onderdeel van dit bezoek worden afgerond.
- Bezoek 4 - Infusie 3: ongeveer 8 weken na de eerste toediening van het onderzoeksproduct moet dit bezoek worden afgerond. Andere studie-evaluaties zullen als onderdeel van dit bezoek worden afgerond.
- Bezoek 5 - Infusie 4: ongeveer 12 weken na de eerste toediening van het onderzoeksproduct moet dit bezoek worden afgerond. Andere studie-evaluaties zullen als onderdeel van dit bezoek worden afgerond.
- Bezoek 6 - Infusie 5: ongeveer 16 weken na de eerste toediening van het onderzoeksproduct moet dit bezoek worden afgerond. Andere studie-evaluaties zullen als onderdeel van dit bezoek worden afgerond.
- Bezoek 7 - Infusie 6: ongeveer 20 weken na de eerste toediening van het onderzoeksproduct moet dit bezoek worden afgerond. Andere studie-evaluaties zullen als onderdeel van dit bezoek worden afgerond.
- Telefoongesprek - Veiligheidsopvolging: ongeveer 24 weken na de eerste toediening van het onderzoeksproduct zullen actieve studiedeelnemers een telefonische follow-up voltooien.
- Telefoongesprek - Veiligheidsopvolging: ongeveer 32 weken na de eerste toediening van het onderzoeksproduct zullen actieve studiedeelnemers een telefonische follow-up voltooien.
- Bezoek 8 - Einde van het onderzoek, tijdens dit laatste bezoek (ongeveer 52 weken na week 0) zal een volledige groep onderzoeksbeoordelingen worden uitgevoerd om de veiligheid en werkzaamheid van allogene HB-adMSC's of Placebo-toedieningen te evalueren.

Inclusiecriteria:
Een studiedeelnemer komt alleen in aanmerking voor opname in deze studie als alle volgende criteria van toepassing zijn:
- Mannelijke en vrouwelijke deelnemers van 45 - 80 jaar.
- Bij het screeningbezoek moeten de deelnemers aan de studie een MDS-UPDRS deel II-score tussen 7 en 28 hebben.
- Studiedeelnemers moeten tijdens het screeningbezoek een MDS-UPDRS deel III-score tussen 20 en 57 hebben.
- De totale dosering van Carbidopa/Levodopa moet minder zijn dan 1200 mg per dag voor deelnemers aan de studie.
- De totale Levodopa-equivalente dosis voor studiedeelnemers moet minder zijn dan 1400 mg per dag.
- Studiedeelnemer moet minimaal 2 jaar voorafgaand aan deelname aan de studie de diagnose van vroege en/of matige ziekte van Parkinson hebben gekregen.
- Studiedeelnemers moeten kunnen lezen, begrijpen en schriftelijke toestemming kunnen geven.
- Vrijwillig ondertekende geïnformeerde toestemming verkregen voordat procedures in verband met klinische onderzoeken worden uitgevoerd.
- Vrouwelijke studiedeelnemers mogen niet zwanger zijn of van plan zijn zwanger te worden tijdens deelname aan de studie en gedurende 6 maanden na de laatste toediening van het onderzoeksproduct.
- Mannelijke deelnemers als hun seksuele partners zwanger kunnen worden, moeten een anticonceptiemethode gebruiken tijdens deelname aan de studie en gedurende 6 maanden na de laatste toediening van het onderzochte product.
- Studiedeelnemer is in staat en bereid om te voldoen aan de vereisten van deze klinische studie.
Uitsluitingscriteria:
Een studiedeelnemer komt niet in aanmerking voor opname in deze klinische studie als een van de volgende criteria van toepassing is:
- Zwangerschap, borstvoeding. Vrouwen in de vruchtbare leeftijd die niet zwanger zijn maar geen effectieve anticonceptiemaatregelen nemen.
- Deelnemers aan de studie met gevorderde ziekte van Parkinson beschreven als ernstige handicap, rolstoelgebonden of bedlegerig.
- Studiedeelnemer heeft een actieve maligniteit, inclusief bewijs van cutaan basaal, plaveiselcelcarcinoom of melanoom.
- Studiedeelnemer heeft een alcoholverslaving of -afhankelijkheid gekend of heeft actueel middelengebruik of -misbruik.
Studiedeelnemer heeft 1 of meer significante gelijktijdige medische aandoeningen (geverifieerd door medische dossiers), waaronder de volgende:
- Slecht gecontroleerde diabetes mellitus (PCDM), gedefinieerd als een voorgeschiedenis van gebrekkige standaardbehandeling en/of preprandiale glucose >130 mg/dl tijdens het screeningsbezoek of postprandiale glucose >200 mg/dl.
- Medische geschiedenis van chronische nierziekte (CKD) diagnose en/of screeningresultaten van eGFR < 59 ml/min/1,73 m2.
- Aanwezigheid van New York Heart Association (NYHA) Klasse III/IV hartfalen tijdens screeningbezoek.
- Elke medische voorgeschiedenis van een myocardinfarct in een van de verschillende typen, zoals myocardinfarct met ST-elevatie (STEMI) of myocardinfarct zonder ST-elevatie (NSTEMI), coronaire spasmen of onstabiele angina pectoris.
- Medische voorgeschiedenis van ongecontroleerde hoge bloeddruk gedefinieerd als een gebrekkige standaardbehandeling en/of bloeddruk > 180/120 mm/Hg tijdens het screeningsbezoek.
- Medische voorgeschiedenis van erfelijke trombofilie, recente grote algemene operatie (binnen 12 maanden vóór de screening), verlamming van de onderste ledematen als gevolg van ruggenmergletsel, breuk van het bekken, heupen of dijbeen, kanker van de longen, hersenen, lymfatisch, gynaecologisch systeem ( eierstok of baarmoeder), of het maagdarmkanaal (zoals pancreas of maag).
- Geschiedenis van hersenchirurgie voor de ziekte van Parkinson.
- Studiedeelnemer heeft binnen 6 maanden vóór de eerste dosis van het onderzoeksproduct een andere stamcelbehandeling ondergaan dan stamcellen geproduceerd door Hope Biosciences.
- Een onderzoekstherapie of een goedgekeurde therapie voor onderzoeksgebruik ontvangen binnen 1 jaar voorafgaand aan de eerste dosis van het onderzoeksproduct, met uitzondering van COVID-19-vaccins.
Studiedeelnemer heeft tijdens de screening een laboratoriumafwijking, waaronder de volgende:
- Aantal witte bloedcellen < 3000/mm3
- Aantal bloedplaatjes < 80.000 mm3
- Absoluut aantal neutrofielen < 1500/mm3
- Alanineaminotransferase (ALAT) of aspartaataminotransferase (AST) 10 bovengrens van normaal (ULN) x 1,5
- De deelnemer aan het onderzoek heeft een andere laboratoriumafwijking of medische aandoening die naar de mening van de onderzoeker een veiligheidsrisico vormt of de proefpersoon ervan weerhoudt het onderzoek af te ronden.
- Het is onwaarschijnlijk dat de deelnemer aan het onderzoek het onderzoek afrondt of zich aan de onderzoeksprocedures houdt.
- Studiedeelnemer met bekende gelijktijdige acute of chronische virale hepatitis B- of C- of humaan immunodeficiëntievirus (HIV) -infectie.
- Studiedeelnemer heeft een eerder gediagnosticeerde psychiatrische aandoening die naar de mening van de onderzoeker de zelfbeoordelingen kan beïnvloeden.
- Studiedeelnemer met een systemische infectie die behandeling met antibiotica, antivirale middelen of antischimmelmiddelen vereist binnen 30 dagen voorafgaand aan de eerste dosis van het onderzoeksproduct.
- Mannelijke studiedeelnemers die van plan zijn sperma te doneren tijdens de studie of binnen 6 maanden na de laatste dosis. Vrouwelijke patiënten die van plan zijn eicellen te doneren of een in-vitrofertilisatiebehandeling te ondergaan tijdens het onderzoek of binnen 6 maanden na de laatste dosis.

The MDS-UPDRS scale refers to Movement Disorder Society - Unified Parkinson Disease Rating Scale, and it is a rating tool used to gauge the course of Parkinson's disease in patients. Part I tests "Nonmotor experiences of daily living". Non-Motor Aspects of Experiences of Daily Living (nM-EDL), including complex behaviors such as, cognitive impairment, hallucinations and psychosis, depressed mood, anxious mood, apathy, features of dopamine dysregulation syndrome, sleep problems, daytime sleepiness, pain and other sensations, urinary problems, constipation problems, light headedness on standing and fatigue. There are 13 items included in Part I. Part I score ranges from 0 - 52; 10 and below is mild, 22 and above is severe. Each item has 0-4 ratings:0 (normal), 1 (slight), 2 (mild), 3 (moderate), and 4 (severe). The total score ranges from 0 to 260, with 0 indicating no disability and 260 indicating total disability.
