DigiPall; 완화 치료에서 디지털 환자 보고 결과 및 바이오마커 모니터링에 대한 무작위 대조 시험

상세 설명
본 연구의 목적은 원격 증상 및 디지털 바이오마커 모니터링 시스템이 완화의료 중 진행성 암 환자 및 중증 질환 환자의 계획되지 않은 병원 재입원, 삶의 질 및 안정감에 미치는 영향을 테스트하는 것입니다.
암이나 기타 생명을 위협하는 질병을 앓고 있는 환자들은 통증과 기타 괴로운 증상을 겪는 경우가 많습니다. 완화의료는 환자의 삶의 질을 향상시키려는 전반적인 목표를 가지고 이러한 증상을 조기에 식별, 평가 및 치료함으로써 고통을 예방하고 완화하는 접근 방식입니다.
완화의료 병동은 스위스의 대형 병원에 설치되어 난치병 환자에게 단기 급성 치료를 제공합니다. 환자의 절반 이상이 집으로 퇴원한다. 그러나 안타깝게도 계획되지 않은 재입원이나 응급 방문이 자주 발생합니다. 계획되지 않은 재입원은 환자와 간병인에게 부담이 되며 비용도 더 많이 듭니다. 계획되지 않은 재입원 또는 병원 재입원의 최대 50%는 피할 수 있는 것으로 간주됩니다.
지정된 기간 내에 계획되지 않은 병원 재입원은 의료 분야에서 종종 적용되는 품질 벤치마크입니다. 환자가 특정 완화의료 병동에서 퇴원할 경우에는 우수한 전문직 간 외래환자 또는 홈케어 서비스가 가장 효과적이었습니다. 그러나 홈 케어 팀을 통한 면밀한 모니터링은 시간과 비용이 많이 들고 장거리로 인해 방해를 받으며 홈 케어 팀을 모든 곳에서 이용할 수는 없습니다. 더욱이, 많은 유럽 국가에서는 일차 진료 관행이 엄격하지 않으며 가정 방문에 대한 상환이 충분하지 않습니다. 따라서 특히 환자 수는 증가하고 간병인 수는 감소하는 점을 고려하여 이 환자 그룹을 모니터링하기 위한 새롭고 혁신적인 접근 방식이 필요합니다.
디지털 시대에는 셀프 트래킹이 더욱 유행하고 일일 화면 사용 시간이 늘어나고 있습니다. 증상과 웰빙에 관해서는 환자 스스로 모니터링하는 것이 최선의 선택입니다. 환자만이 자신의 감정을 적절하게 결정할 수 있기 때문입니다. 성능 상태와 성능 저하를 웨어러블 기기를 통해 원격으로 평가할 수 있습니다. 모바일 건강 기술은 활동과 건강 상태를 모니터링하는 쉽고 실행 가능한 방법입니다.
이 무작위 대조 연구는 표준 치료를 받는 두 그룹의 완화 환자와 모니터링 시스템을 추가로 받는 개입 그룹을 비교하는 것을 목표로 합니다. DigiPall 중재그룹에서는 웨어러블 디바이스를 통해 지속적으로 바이오마커 데이터를 수집하고 환자의 주관적 건강상태에 대한 질문에 대해 환자가 채팅을 통해 답변하게 됩니다. 답변을 바탕으로 자동화된 피드백이 제공됩니다. 이 피드백에는 새로운 처방이 포함되지 않으며 치료 의사와 아직 논의되지 않은 내용이 제안되지 않으며 오히려 환자에게 기존 치료 계획에 따른 옵션을 상기시켜 줍니다. 환자가 퇴원 후 제안된 치료 계획을 실행하지 못하는 것이 재입원을 피할 수 있는 주요 요인이라는 강력한 증거가 있습니다. 따라서 중재의 목표는 환자의 자기관리능력과 안정감을 강화하여 의료비 절감과 더불어 더 나은 결과를 가져오는 것이다.
이 접근법은 타당성 조사에서 호평을 받았으며 현재 연구 설계는 환자 자신의 의견을 바탕으로 개발되었습니다. 연구 기간 동안 환자 그룹의 조언을 정기적으로 상담받게 됩니다.

|
DigiPall은 센서가 장착된 웨어러블과 환자의 스마트폰에서 WhatsApp을 통해 통신하는 챗봇을 결합한 개입입니다.
환자는 스마트폰의 WhatsApp을 통해 주관적인 평가에 대해 매일 두 가지 질문을 받게 되며, 걸음수, 심박수, 순응도에 대한 측정값과 전날 환자의 답변에 따라 단어가 조금씩 다르게 표시됩니다.
