De effecten van subanesthetische ketamine op ademhalingsstimulatie en transpulmonale druk bij mechanisch beademde ernstig zieke patiënten
Aantasting van de doorgankelijkheid van de luchtwegen is een veelvoorkomende oorzaak van het falen van de extubatie en opioïden en hypnotica kunnen de doorgankelijkheid van de luchtwegen nadelig beïnvloeden. Ketamine, een niet-competitieve antagonist van N-methyl-D-aspartaat (NMDA), activeert, in tegenstelling tot andere anesthetica, ademhalingsinspanning en bevordert bronchusverwijding. Bij subanesthetische plasmaconcentraties vermindert ketamine zowel de behoefte aan opioïden als propofol.
Het doel van deze farmacofysiologische interactiestudie is het evalueren van de effecten van ketamine op de ademhaling en elektro-encefalografie bij mechanisch beademde patiënten.

Het handhaven van de doorgankelijkheid van de bovenste luchtwegen bij verdoofde en verdoofde patiënten is een uitdaging, vooral wanneer patiënten klaar zijn om van mechanische beademing af te komen. Spontane ademhalingsproef (SBT) wordt gebruikt om het ontwenningsproces te versnellen, wat vaak de vermindering en/of stopzetting van sedativa en analgetica vereist. Bij sommige chirurgische patiënten kan het verminderen van deze medicijnen leiden tot pijngerelateerde agitatie en het onvermogen om SBT's uit te voeren, wat de behoefte aan mechanische beademing kan verlengen. Het gebruik van medicijnen met narcotische sparende effecten en die geen ademhalingsdepressie veroorzaken, kan het verminderen of stopzetten van middelen die de ademhalingsdrang onderdrukken en vervolgens extubatie vergemakkelijken, mogelijk maken.
Ketamine wordt al vele jaren gebruikt bij ernstig zieke patiënten voor sedatie en analgesie. Deze niet-competitieve antagonist van N-methyl-D-aspartaat (NMDA) wordt gebruikt als verdovings- en analgeticum en het is aangetoond dat het de opioïdenconsumptie vermindert en de ontwikkeling van opioïdentolerantie voorkomt. In tegenstelling tot andere anesthetica activeert ketamine de ademhalingsinspanning en bevordert het de bronchusverwijding. Bij subanesthetische plasmaconcentraties vermindert ketamine zowel de behoefte aan opioïden als propofol.
Het doel van deze farmacofysiologische interactiestudie is het evalueren van de effecten van ketamine in een subanesthetische dosis op ademhaling en elektro-encefalografie. De onderzoekers veronderstellen dat het druppelen van ketamine met een subanesthetische infusiesnelheid (lage dosis ketamine 5 - 10 mcg/kg/min) gepaard gaat met ademhalingsstimulerende effecten en de transpulmonale druk niet merkbaar verhoogt bij mechanisch beademde patiënten.
Het primaire resultaat is de ademhalingsfunctie, beoordeeld aan de hand van de piekinspiratoire flow, ademvolume, ademhalingsfrequentie, duty cycle en minuutventilatie gemeten 15 minuten voorafgaand aan de start van de ketamine-infusie (om als basislijn te dienen), bij 60 minuten ketamine-infusie van 5 mcg/ kg/min, bij nog eens 60 minuten infusie met 10mcg/kg/min, waarna de infusie gedurende 3 uur wordt gestopt voor een laatste reeks metingen.

Inclusiecriteria:
- Leeftijd ≥ 18 jaar opgenomen op de IC waar mechanische beademing nodig is
- Geschikt voor spontane ademhalingsproef
- Kandidaat heeft een lage dosis ketamine gekregen van het eerstelijns kritieke zorgteam
Uitsluitingscriteria:
- Slokdarm letsel
- Allergisch voor ketamine
- Bekende neurodegeneratieve aandoeningen
- Ernstige neurologische aandoeningen (verhoogde ICP)

EEG Beta-gamma Power

Electroencephalogram (EEG) power spectrum density was measured using four frontal electrodes with the SedLine monitor and analyzed using multitaper spectral methods.
Changes in beta-gamma power (19-44 Hz) were expressed in decibels (dB) relative to baseline using artifact-free ~3-minute segments at predefined time points.
The reported value represents the maximum increase in spectral power across the frequency range, capturing the strongest frequency-specific effect, which may be diluted by averaging across the band if central tendency is used.
This approach is standard in spectral analyses to detect peak effects and potential frequency shifts.
