A légutak átjárhatóságának károsodása az extubációs sikertelenség gyakori oka, az opioidok és a hipnotikumok pedig hátrányosan befolyásolhatják a légutak átjárhatóságát. A ketamin, amely az N-metil-D-aszpartát (NMDA) nem kompetitív antagonistája, más érzéstelenítőkkel ellentétben aktiválja a légzést és elősegíti a hörgőtágulást. Szubanesztetikus plazmakoncentráció esetén a ketamin csökkenti mind az opioid-, mind a propofolszükségletet.

A felső légutak átjárhatóságának fenntartása szedált és érzéstelenített betegeknél különösen nagy kihívást jelent, ha a betegek készen állnak a gépi lélegeztetésről való leszoktatásra. A spontán légzési vizsgálatot (SBT) használják az elválasztási folyamat felgyorsítására, amely gyakran megköveteli a nyugtatók és fájdalomcsillapítók csökkentését és/vagy abbahagyását. Egyes sebészeti betegeknél ezeknek a gyógyszereknek a csökkentése fájdalommal járó izgatottsághoz és SBT-k elvégzésének képtelenségéhez vezethet, ami meghosszabbíthatja a gépi lélegeztetés szükségességét. A kábítószer-megtakarító hatású és légzésdepressziót nem okozó gyógyszerek alkalmazása lehetővé teheti a légzési késztetést elnyomó és az extubációt elősegítő szerek csökkentését vagy abbahagyását.
A ketamint évek óta alkalmazzák kritikus állapotú betegek szedációjára és fájdalomcsillapítására. Az N-metil-D-aszpartát (NMDA) nem kompetitív antagonistáját érzéstelenítőként és fájdalomcsillapítóként használják, és kimutatták, hogy csökkenti az opioidfogyasztást és megakadályozza az opioid tolerancia kialakulását. Más érzéstelenítőkkel ellentétben a ketamin aktiválja a légzést és elősegíti a hörgőtágulást. Szubanesztetikus plazmakoncentráció esetén a ketamin csökkenti mind az opioid-, mind a propofolszükségletet.
Ennek a farmakofiziológiai interakciós vizsgálatnak a célja a szubanesztetikus dózisú ketamin légzésre és elektroencefalográfiára gyakorolt hatásának értékelése. A kutatók azt feltételezik, hogy a ketamin csepegtetése szubanesztetikus infúziós sebességgel (alacsony dózisú ketamin 5-10 mcg/kg/perc) légzést stimuláló hatással jár, és nem növeli jelentősen a transzpulmonális nyomást mechanikusan lélegeztetett betegeknél.
Az elsődleges eredmény a légzésfunkció, amelyet a belégzési csúcsáramlás, a légzési térfogat, a légzésszám, a munkaciklus és a perc lélegeztetés alapján mérnek, 15 perccel a ketamin infúzió megkezdése előtt (kiindulási értékként), 60 perces ketamin infúziónál 5 mikrogramm/ kg/perc, további 60 perces infúzió után 10 mcg/kg/perc sebességgel, ekkor az infúziót 3 órára leállítják a végső mérési sorozathoz.

EEG Beta-gamma Power

Electroencephalogram (EEG) power spectrum density was measured using four frontal electrodes with the SedLine monitor and analyzed using multitaper spectral methods.
Changes in beta-gamma power (19-44 Hz) were expressed in decibels (dB) relative to baseline using artifact-free ~3-minute segments at predefined time points.
The reported value represents the maximum increase in spectral power across the frequency range, capturing the strongest frequency-specific effect, which may be diluted by averaging across the band if central tendency is used.
This approach is standard in spectral analyses to detect peak effects and potential frequency shifts.
Power spectra are continuous functions and peak values are the dominant signal components because they represent the strongest oscillatory activity the brain could achieve in a predefined frequency range.
