Endoscopische echografie is een gevestigde onderzoeksmethode voor tumoren van het maagdarmkanaal en de pancreas. Aangezien beeldvorming op zichzelf beperkt is in de differentiële diagnose van tumoren, wordt EUS-geleide fijne naaldaspiratie gezien als een geldige aanvullende methode. Aangezien fijne-naald-aspiratie (FNA) voornamelijk gebaseerd is op cytologische diagnostiek, is deze methode beperkt, mede door het ontbreken van aanvullende immuunhistochemische diagnostiek. Hier zou het verkrijgen van kleine histologisch evalueerbare weefselmonsters (geperforeerde vaten) van voordeel zijn.
Een nieuw ponsnaaldapparaat genaamd Pro-core naald (Cook) (22 / 19 gauge) biedt de mogelijkheid om het aantal geldige extracties van histologisch evalueerbare weefsels te vergroten dankzij een betere gerichte precisie en manoeuvreerbaarheid in vergelijking met andere apparaten van dat soort (tru -naalden afsnijden, bijv.). Een kleine inkeping op het punt maakt het mogelijk om kleine weefselmonsters te verkrijgen, die door aspiratie in het apparaat worden gehouden.
Deze studie vergelijkt het verkrijgen van weefsel door Proc-core naald en conventionele aspiratie punctiesystemen.

Beschrijving
Inclusiecriteria:
- klinische indicatie voor endoscopische echografie met fijne naaldbiopten
- leeftijd ouder dan 18 jaar
Uitsluitingscriteria:
- moeilijke of onmogelijke benadering van gewenste structuren vanwege anatomie (postoperatieve anatomie, b.v.)
- cystische laesie, b.v. cystische alvleeskliertumoren
- coagulopathie
- ernstige algemene toestand van de patiënt
- andere contra-indicaties voor endoscopische echografie ondersteunden fijne naaldpunctie

Diagnostische nauwkeurigheid

Diagnostische nauwkeurigheid van Pro-core-naald (22 G) zal worden vergeleken met conventionele fijne naaldaspiratie (22 G). Daarom wordt EUS-FNA met beide naalden in willekeurige volgorde uitgevoerd in elke laesie. Voor de Pro-core-naald wordt een histologische/cytologische diagnose en kwaliteitsbeoordeling uitgevoerd door pathologen. Voor de Echotip-aspiratienaald wordt referentiecytologie-evaluatie uitgevoerd door cytologie-experts. De histopathologische diagnose na chirurgie of de klinische follow-up van minstens één jaar na EUS FNA is de huidige norm. |
