Het primaire doel van het project is het bepalen van het percentage CTC-positieve patiënten en het totale CTC-aantal bij gevorderde NSCLC bij baseline, voordat de behandeling wordt gestart.
Om wijzigingen in het CTC-aantal te beoordelen als een vroege voorspeller van de respons op de behandeling, zal een prospectief observationeel onderzoek worden uitgevoerd, waarbij achtereenvolgens 150 patiënten zullen worden geïncludeerd, ongeacht het type of de lijn van de therapie. Deze zullen opnieuw worden geëvalueerd op ziektestatus en CTC-aantal, afhankelijk van het type en het behandelschema. behandeling.
Bovendien zal gezuiverde CTC worden verkregen via een geautomatiseerd platform en zal DNA-sequencing worden uitgevoerd om sensibiliserende mutaties bij gevorderde NSCLC-patiënten te beoordelen. De in CTC gedetecteerde EGFR-mutaties of het EML4-ALK-fusiegen zullen worden vergeleken met de mutaties die voor het tumorspecimen zijn gerapporteerd met behulp van standaardsequencing.
Tegelijkertijd zal de CTC-tellingstest worden geïntegreerd met mAb gericht op het EGFR- of EML4-ALK-fusiegen, om te testen of het testen van de quote van EGFR-positieve en ALK-positieve CTC een gevoeliger marker oplevert voor het beoordelen van farmacodynamische effecten bij patiënten vergeleken met totaal CTC telt.
Bovendien werd gerapporteerd dat NSCLC-cellen met EGFR-mutaties activatie van de PI3K (fosfatidylinositol 3-kinase)-AKT en MEK-ERK (extracellulair signaal-gereguleerde kinase) signaalroutes onder controle van EGFR vertonen, en blootstelling van dergelijke cellen aan EGFR. -TKI's blokkeren de signalering door beide routes en induceren apoptose 13. Gegevens verkregen met de CTC-test zullen worden gecorreleerd met algemene prognostische factoren.

Inclusiecriteria:
- Leeftijd 18-80 jaar, man of vrouw;
- Patiënten met NSCLC bevestigd door histopathologie of cytologie;
- Levensverwachting > 6 maanden;
