Deze gerandomiseerde fase II-studie is bedoeld om de werkzaamheid en toxiciteit te bepalen van matig gehypofractioneerde conforme straling gecombineerd met S-1 voor plaveiselcelcarcinoom van de slokdarm.
Alle patiënten krijgen twee cycli van S-1 (40 mg/㎡, BID, po) op D1-14, D22-35, gecombineerd met thoracale radiotherapie van 60 Gy/24 fracties voor GTV en 40 Gy/16 fracties voor CTV. Het primaire eindpunt is progressievrije overleving, de tijd die verstrijkt vanaf de eerste dag van radiotherapie tot de datum waarop de ziekte voortschrijdt. Progressievrije overleving zal worden berekend en vergeleken met behulp van de Kaplan-Meier-methode. Toxiciteiten zullen worden beoordeeld volgens CTCAE v. 4.0.

Beschrijving
Inclusiecriteria:
- Histologisch bevestigd plaveiselcelcarcinoom van de slokdarm
- Inoperabel stadium II-IVa (UICC 2002; zesde editie), bevestigd door contrastversterkte computertomografie, endoscopische echografie, bariumoesofagram, emissiecomputertomografie en/of positronemissietomografie/computertomografie.
- Eastern Cooperative Oncology Group (ECOG) prestatiestatus 1-2
- Geschatte levensverwachting van minimaal 12 weken
- Charlson-comorbiditeitsindex.4
- Adequate beenmergfunctie: witte bloedcellen graad 0 of 1, absolute neutrofielen graad 0 of 1, bloedplaatjes graad 0, hemoglobine graad 0 of 1
- Adequate nierfunctie: creatininegraad 0 of 1
- Adequate leverfunctie: serumbilirubine graad 0 of 1, alanineaminotransferase en aspartaataminotransferase <2 maal de bovengrens van normaal
- Gewichtsverlies.15% gedurende 6 maanden voorafgaand aan de diagnose
- Geforceerd expiratoir volume seconde.1L
Uitsluitingscriteria:
- Voorgeschiedenis van maligniteit, behalve niet-melanoom huidcarcinoom of cervicaal carcinoom in situ
- Eerdere antitumortherapie, inclusief chirurgie, chemotherapie of radiotherapie
- Contra-indicatie voor chemotherapie of radiotherapie
- Kwaadaardige pleurale of pericardiale effusie
- Vrouwen in zwangerschap of lactatieperiode
- Vrouwen die de kans hebben op zwangerschap zonder anticonceptie
- Gewichtsverlies≥15% gedurende 3 maanden voorafgaand aan de diagnose
- In andere klinische onderzoeken binnen 30 dagen
- Verslaafd aan drugs of alcohol, AIDS-patiënten
- Oncontroleerbare aanvallen of psychotische patiënten zonder zelfbeheersing
- Ernstige allergie of eigenaardigheid
- Niet geschikt voor dit onderzoek, oordeelden de onderzoekers
