Hoved

  • For å bestemme om 2 skjemaer for kalsiumglukonat og magnesiumsulfatinfusjoner (gitt før og etter kjemoterapi eller like før kjemoterapi) kan forhindre eller lindre kronisk, kumulativ oksaliplatinindusert sensorisk nevrotoksisitet hos pasienter med tykktarms- eller rektalkreft som får adjuvant FOLFOX kjemoterapi med kalsium, leukovorin fluorouracil og oksaliplatin.

Sekundær

  • For å bestemme om disse 2 infusjonsplanene kan øke de kumulative oksaliplatindosene som kan leveres uten dosebegrensende kronisk nevrotoksisitet.
  • For å bestemme om disse to infusjonsplanene kan lindre akutt nevropati assosiert med oksaliplatin.

  • For å utforske om polymorfismer i GSTP1-, GSTM1-, ERCC2- og XRCC1-genene er assosiert med tidlig utbrudd av oksaliplatin-indusert nevrotoksisitet.

OVERSIKT: Dette er en multisenterstudie. Pasientene er stratifisert etter alder (< 65 år vs ≥ 65 år), kjønn, regime (FOLFOX4 vs modifisert FOLFOX6 vs annet) og sykdomsstadium (II vs III vs IV). Pasientene er randomisert til 1 av 3 behandlingsarmer.

  • Arm I: Pasienter får kalsiumglukonat IV og magnesiumsulfat IV i løpet av 30 minutter rett før og etter administrering av oksaliplatin (en del av FOLFOX-kjemoterapi som omfatter leukovorinksium, fluorouracil og oksaliplatin).
  • Arm II: Pasienter får placebo IV over 30 minutter rett før og etter administrasjon av oksaliplatin (en del av FOLFOX kjemoterapi).

I alle armer gjentas kursene hver 14. dag i 6 måneder i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

Blodprøver tas før det andre behandlingsforløpet for translasjonsforskning.

Pasienter fyller ut spørreskjemaer om bivirkninger, livskvalitet og kjemoterapi-indusert perifer nevropati med jevne mellomrom.

Etter avsluttet studiebehandling følges pasientene opp ved 3, 6, 12 og 18 måneder.

SYKDOMENS KARAKTERISTIKA:

  • Histologisk bekreftet adenokarsinom i tykktarmen eller endetarmen
  • Har gjennomgått kurativ reseksjon og anses å ha stadium II- eller III-sykdom eller fullstendig reseksjonert stadium IV-sykdom uten tegn på gjenværende tumor

  • Planlagt å motta 6 måneder med oksaliplatin-basert adjuvant kjemoterapi med 85 mg/m^2 oksaliplatin hver 2. uke (dette inkluderer for eksempel FOLFOX4 eller modifisert FOLFOX6)

    • Pasienter som får bevacizumab eller cetuximab i kombinasjon med FOLFOX som del av en klinisk utprøving eller klinisk praksis er kvalifisert

  • Sentral venøs tilgangslinje til stede, eller planlagt å ha en sentral linje plassert før oppstart av kjemoterapi og studiebehandling
  • Ingen tidligere behandling med nevrotoksisk kjemoterapi (f.eks. oksaliplatin, cisplatin, taxaner eller vinca-alkaloider)
  • Ingen samtidig digitalis-medisinering
  • Ingen samtidig behandling med antikonvulsiva karbamazepin (f.eks. Tegretol®), fenytoin (f.eks. Dilantin®), valproinsyre (f.eks. Depakene®), gabapentin (Neurontin®) eller pregabalin (Lyrica®)
  • Ingen samtidige nevrotrope midler, inkludert venlafaksin (Effexor), desvenlafaksin (Pristiq®), milnacipran (Savella®) eller duloksetin (Cymbalta)
  • Ingen samtidige trisykliske antidepressiva (som amitryptillin), eller andre midler som gis spesifikt for å forebygge eller behandle nevropati

Den oksaliplatininduserte sensoriske nevropatien som gjentatte ganger målt av European Organization for Research and Treatment of Cancer Quality of Life Questionnaire of Chemotherapy-indused periferal neuropathy (EORTC QLQ-CIPN20) sensorisk subskala under kjemoterapien. Dette er en multivariat gjentatt måling av CIPN med muligens variable sykluser for hver pasient. CIPN sensoriske underskala vil bli beregnet ved standard skåringsalgoritme og konvertert til 0-100 skala, hvor høyere skår representerer høyere livskvalitet.
