Tło:
MESA wywodzi się z NHLBI Task Force on Research in Epidemiology and Prevention, w której badanie subklinicznej choroby i jej progresji do choroby klinicznej zalecono jako główny przedmiot przyszłych badań populacji NHLBI. Następnie w 1995 r. Specjalny panel nacisku na podłużne badania kohortowe zdecydowanie zalecił badania oparte na pomiarach subklinicznych chorób oraz włączenie niedostatecznie reprezentowanych mniejszości do badań populacyjnych. Późniejszy Panel Specjalny ds. Wykorzystania EBCT i MRI serca w badaniach epidemiologicznych chorób sercowo-naczyniowych w 1996 roku zalecił włączenie MRI tętnicy szyjnej i serca oraz EBCT do wyjaśnienia progresji choroby z subklinicznej do klinicznej i identyfikacji subklinicznych cech choroby najsilniej związanych ze zwiększonym ryzykiem. Zapytania ofertowe zostały wydane w 1997 roku, a nagrody przyznano w 1999 roku.
Narracja projektowa:
Uczestnicy zostali zbadani na początku badania pod kątem subklinicznej choroby sercowo-naczyniowej za pomocą tomografii komputerowej (CT), rezonansu magnetycznego serca, ultrasonografii tętnicy szyjnej, poszerzenia tętnicy ramiennej zależnego od przepływu, tonometrii tętnicy promieniowej, pomiaru wskaźnika kostka-ramię; ustalone i domniemane laboratoryjne markery ryzyka; oraz cechy społeczno-ekonomiczne, psychologiczne, behawioralne i środowiskowe. Wybrane podstawowe elementy zostały powtórzone, a dodatkowe elementy, takie jak spirometria, fotografia siatkówki, genotypowanie, ocena funkcji poznawczych oraz, w podgrupach, TK aorty brzusznej, MRI tętnicy szyjnej, znakowanie MRI serca w celu pomiaru regionalnej funkcji mięśnia sercowego, zostały wprowadzone w pięciu kolejnych badaniach do 2018 r. . Przechowywane próbki krwi zostały przebadane pod kątem przypuszczalnych biochemicznych czynników ryzyka i przechowywane do badań kliniczno-kontrolnych. Wyekstrahowano DNA do badania genów kandydujących, skanowania całego genomu, ekspresji i innych badań -omicznych, a limfocyty poddano kriokonserwacji w celu ewentualnego unieśmiertelnienia.
MESA jest wyjątkowa pod względem składu czterech grup etnicznych, mając kohortę wolną od klinicznych CVD na początku badania i mającą wiele – aw niektórych przypadkach unikalnych – subklinicznych pomiarów CVD w czasie u tych samych osób. Dane zebrane z dużej liczby i różnorodnych badań pomocniczych MESA, w tym głównych badań pomocniczych dotyczących zanieczyszczenia powietrza, przewlekłych chorób płuc, genetyki i snu, dodatkowo przyczyniają się do jego wyjątkowości. Listę całej dokumentacji i protokołów danych fenotypowych można znaleźć na stronie internetowej MESA: www.mesa-nhlbi.org. Dane MESA są dostępne dla kwalifikujących się badaczy bezpośrednio z badania, a także za pośrednictwem dbGaP (http://www.ncbi.nlm.nih.gov/gap) i BioLINCC (https://biolincc.nhlbi.nih.gov). Dostępnych jest również wiele przechowywanych próbek biologicznych z badania, w tym DNA, surowica, osocze i mocz.
