PROCAS: Prospektywne i wieloośrodkowe badanie obserwacyjne w celu określenia skuteczności klinicznej i bezpieczeństwa stosowania octanu kaspofunginy (CANCIDAS®) w leczeniu grzybicy inwazyjnej
Chcieliśmy określić skuteczność i bezpieczeństwo stosowania octanu kaspofunginy (CANCIDAS®) w leczeniu zakażeń grzybiczych najeźdźców (IFI), w szczególności inwazyjnej kandydozy (CI) u dorosłych pacjentów bez neutropenii oraz inwazyjnej aspergilozy (AI) u dorosłych pacjentów opornych na leczenie. na inne terapie lub ich nietolerancję (np. amfoterycyna B, preparaty lipidowe amfoterycyny B i/lub itrakonazol).

Kryteria przyjęcia:
- Kobieta i mężczyzna w wieku co najmniej 18 lat.
- Diagnoza AI lub CI. Będzie opierać się na historii klinicznej, oznakach i objawach, które przyjmuje pacjent, odkryciu badań radiologicznych i wynikach hodowli i/lub badaniach anatomopatologicznych pozytywnych dla Candida spp. lub Aspergillus spp.
- Pacjenci z AI muszą wykazywać oporność lub nietolerancję na inne terapie przeciwgrzybicze stosowane w szpitalu.
- Pacjenci z CI muszą być lub nie oporni lub nietolerować innych terapii przeciwgrzybiczych stosowanych w szpitalu.
Kryteria wyłączenia:
- Pacjenci z niepewną diagnozą w oparciu o co najmniej jedno z następujących kryteriów: dodatni wynik hodowli Candida w jedynej lokalizacji (mocz, ślina, koniec/prąd cewnika; zapalenie kości i szpiku lub zapalenie wsierdzia wywołane przez Candida bez odpowiedniego leczenia chirurgicznego; zakażenie materiału protetycznego przez Candida bez przejścia na emeryturę tego samego; pozytywny wynik posiewu krwi w kierunku Candida w wyglądzie protezy naczyniowej; alergiczna aspergiloza płuc; i aspergilloma lub aspergiloza oka bez odpowiedniego leczenia chirurgicznego).
- Alergia/nadwrażliwość na kaspofunginę.
- Niewydolność kilku wątrob (wskaźnik Chile-Pugh >9).
- Prognoza przeżycia <5 dni.
- Ciąża lub karmienie piersią.
- Poprzedni Udział pacjenta w tym badaniu.
- Obecność innej choroby lub jakiegokolwiek stanu, który może zakłócić interpretację wyników lub stworzyć ryzyko.
- Uprzedni udział pacjenta w innym badaniu klinicznym polegającym na podaniu badanego leku w ciągu 14 dni poprzedzających włączenie do badania lub podczas podawania kaspofunginy. Do badań możemy włączyć pacjentów przyjmowanych z lekami przeciwnowotworowymi zawsze, gdy wcześniej Koordynator Badacza wyrazi na to zgodę
