Radiorapia wewnątrzgałkowa i otrzewna siedliska MRI przewiduj pełną odpowiedź na chemoimmunoterapię neoadjuwantową w raku płaskonabłonkowym jamy ustnej
Tło : Neoadjuwantowa chemoimmunoterapia (NACI) wykazuje obietnicę w raku płaskonabłonkowym doustnym (OSCC), ale niezawodne nieinwazyjne biomarkery do przewidywania patologicznej pełnej odpowiedzi (PCR) pozostają rzadkie. Radiomics integrując heterogeniczność wewnątrzgałkowinową i otrzewną w MRI z wieloma sekwencjami może oferować nowe informacje na temat oceny odpowiedzi na leczenie.
Metody: Dane 212 pacjentów z OSCC po NACI zostały retrospektywnie zebrane i analizowane. Wśród tych pacjentów 56 (26,4%) osiągnęło PCR po Naci. Osiągnięto wewnątrzgałkowe i otrzewne obrazowanie siedlisk (HI) przy użyciu algorytmu grupowania K-MANS zastosowanych do obrazowania ważonego T1 (T1WI), obrazowania osiowego T2 z tłumieniem tłuszczu (T2WI) i kontrastowego obrazowania T1 z tłuszczą (T1C). Ponadto skonstruowano i porównywano modele hI wewnątrzgałkowe i otrzewne HI i porównano za pomocą krzywej charakterystyki operacyjnej odbiornika (ROC). Przeprowadzono pięciokrotną walidację krzyżową w celu złagodzenia przepełnienia modelu.

Kryteria włączenia: (a) histopatologicznie potwierdzone OSCC przy początkowej diagnozie; (b) brak leczenia przed pierwszym badaniem MRI; (c) otrzymanie NACI; (d) poddano badaniu MRI po Naci; (e) Przeszła interwencję chirurgiczną.

Kryteria wykluczenia:- (a) brak danych klinicznych i wyniki patologiczne pooperacyjne; (b) Artefakty obrazu MRI znacznie wpływają na jakość obrazu.
