这是一项 II 期研究。 主要终点是在接受立体定向放射结合治疗后第一年内给予替莫唑胺的第一年内发生新脑转移的新发转移患者的比例。 回顾性和前瞻性研究表明,50-60% 的长期幸存者会出现新的脑转移。 由于观察所有招募的患者至少一年以衡量主要结果很重要,因此传统的 2 期设计(例如 Simon 的两阶段优化设计或最小最大设计)在这种情况下不实用。 生存和 QOL 是次要终点。 QOL 将使用癌症治疗的功能评估 (FACT -BR) 进行测量。 它将在基线、第四周和每三个月进行一次,持续 24 个月。
该协议包括使用标准辐射剂量(15-24 Gy,基于 RTOG 9005)的放射外科手术。 只要在放射外科手术后两周内开始使用 Temodar,就可以在放射外科手术后对患者进行登记。
放射外科手术后两周内开始:TMZ 200mg/m2 天 1-5 重复 q28 天。 先前接受过化疗的患者将在第 1-5 天接受 150 mg/m2。
继续使用替莫唑胺,直至出现由全身进展或新转移所定义的疾病进展。 如果在没有新的 CNS 肿瘤或全身进展的情况下用放射外科治疗的病变进展,那么 TMZ 将继续。 因需要其他全身化疗的全身进展停用替莫唑胺。
在方案治疗期间,可以对非 CNS 部位进行姑息性放疗,但在患者出现全身性进展或出现新的转移之前,不会进行额外的全身化疗。

描述
纳入标准:
- 所有受试者必须有经组织学证实的恶性肿瘤病史。 除非主治医师判断诊断有疑问,否则不需要进行脑活检。
- 新诊断的脑转移(薄片对比后 MRI 发现四个或更少)。
- 对于既往未接受过化疗的患者,ECOG 体能状态小于或等于 2,对于既往接受过化疗的患者,ECOG 体能状态小于或等于 1。
- 年龄大于 18 岁
- 预期寿命大于 12 周
- 受试者获得书面知情同意书
足够的血液学、肾脏和肝脏功能,如在 14 天内(包括在内)在研究药物给药前进行的实验室值所证明的:
- 中性粒细胞绝对计数 (ANC) >= 1500/mm3
- 血小板计数 >= 100,000/mm3
- 血红蛋白 >= 9 克/分升
- BUN 和血清肌酐 <= 实验室正常值上限的 1.5 倍
- 总胆红素和直接胆红素 <= 实验室正常值上限的 2 倍或存在有记录的肝转移,总胆红素和直接胆红素 <= 正常值上限的 5 倍

排除标准:
- 患有小细胞肺癌和淋巴瘤的患者不符合资格。
- 薄层 MRI 显示有 4 个以上的转移灶。 请注意,如果放射外科手术前的诊断研究仅显示四个肿瘤,但伽玛刀治疗计划扫描显示多于四个肿瘤,如果所有肿瘤都可以用放射外科手术治疗,则患者仍符合该方案的条件。
- 研究药物给药前 4 周内接受化疗
- 治疗肿瘤内科医师认为除研究药物外还需要立即进行细胞毒性化疗的患者。 允许的药物包括抗激素药(即他莫昔芬)、赫赛汀和双膦酸盐。
- 放射治疗至大于或等于50%的骨髓。 在研究药物给药前 4 周内完成放射治疗以达到 >= 15% 的骨髓和在放射治疗前 2 周内完成 < 15% 的骨髓。
- 从先前治疗的所有活性毒性中恢复不充分
- 因非恶性全身性疾病而有不良医疗风险的受试者
- 频繁呕吐或可能干扰口服药物摄入的医疗状况(例如,部分肠梗阻)。
- 其他部位先前或并发的恶性肿瘤,或研究开始后 5 年内该部位的恶性肿瘤治疗开始,手术治愈的宫颈原位癌和皮肤基底细胞癌或鳞状细胞癌除外。
- 已知 HIV 阳性或与 AIDS 相关的疾病。
- 孕妇或哺乳期妇女。
- 未使用有效避孕方法的育龄妇女。 有生育能力的女性必须在服用研究药物前 24 小时进行血清妊娠试验阴性,并采取医学上批准的避孕措施。
- 不建议使用有效避孕方法的男性。
