Stamcelletransplantation sammenlignet med standardkemoterapi til behandling af patienter med akut lymfatisk leukæmi i første remission
Fase III randomiseret forsøg med autolog og allogen stamcelletransplantation versus intensiv konventionel kemoterapi ved akut lymfoblastisk leukæmi i første remission
RATIONALE: Lægemidler, der bruges i kemoterapi, virker på forskellige måder for at stoppe kræftceller i at dele sig, så de holder op med at vokse eller dør. Kombination af kemoterapi med allogen eller autolog stamcelletransplantation kan give lægen mulighed for at give højere doser kemoterapi og dræbe flere kræftceller. Det vides endnu ikke, om stamcelletransplantation er mere effektiv end standard kemoterapi til behandling af akut lymfatisk leukæmi.

- Sammenlign de overordnede behandlingsresultater hos patienter behandlet med disse regimer.
- Bestem effekten af imatinibmesylat givet efter induktionsterapi hos Philadelphia (Ph) kromosompositive patienter i CR.
- Bestem fordelen ved allogen eller autolog SCT efter imatinibmesylat hos Ph-kromosompositive patienter.
- Bestem fordelen ved yderligere imatinibmesylat administreret efter allogen eller autolog SCT hos Ph-kromosompositive patienter.
- Bestem den minimale resterende sygdom hos Ph-kromosompositive patienter før og efter behandling med imatinibmesylat.

OVERSIGT: Dette er en randomiseret, multicenter undersøgelse. Patienterne er stratificeret efter alder (50 og under versus over 50), tid til at opnå fuldstændig remission (CR) (4 uger eller mindre versus mere end 4 uger) og Philadelphia (Ph) kromosomstatus (positiv vs negativ).
- Første induktionsterapi: Patienterne får daunorubicin (DNR) IV over 15-30 minutter og vincristin (VCR) IV over 3-5 minutter på dag 1, 8, 15 og 22; oral prednison (PRED) én gang dagligt på dag 1-28; og asparaginase (ASP) IV over 30 minutter eller intramuskulært på dag 17-28. Patienter med CNS-leukæmi modtager også methotrexat (MTX) intrathecalt (IT) via et Ommaya-reservoir ugentligt, indtil CSF er klar. Patienter uden CNS leukæmi ved præsentation modtager kun MTX IT på dag 23.
- Anden induktionsterapi: Begyndende umiddelbart efter den første induktionsterapi får patienterne cyclophosphamid (CTX) IV over 30 minutter på dag 1, 15 og 29; cytarabin (ARA-C) IV over 30 minutter på dag 1-4, 8-11, 15-18 og 22-25; og oral mercaptopurin (MP) én gang dagligt på dag 1-28. Patienter med CNS leukæmi undergår også samtidig kraniospinal bestråling. Patienter uden CNS-leukæmi ved præsentationen modtager MTX IT på dag 1, 8, 15 og 22. Patienter med Ph-kromosompositiv status får oralt imatinibmesylat én gang dagligt i mindst 28 dage (dag 1-28).
Patienter med Ph-kromosom-positiv status og CR efter anden induktionsterapi fortsætter til gruppe I til autolog eller allogen stamcelletransplantation (SCT). Patienter med Ph-kromosom-negativ status og CR efter anden induktionsterapi fortsætter til gruppe II.
Gruppe I (Ph-kromosompositive patienter):
- Autolog SCT: Patienter modtager højdosis konsoliderings-/mobiliseringskemoterapi omfattende ARA-C IV over 3 timer på dag 1-3 og mitoxantron IV umiddelbart efter ARA-C administration på dag 1 og 2. Patienterne får også filgrastim (G-CSF) subkutant (SC) én gang dagligt begyndende på dag 5 og fortsætter, indtil blodtallene er genoprettet.
Patienterne gennemgår derefter perifer blodstamcelleindsamling eller knoglemarvshøstning.
Patienter modtager præparativ behandling, der omfatter total kropsbestråling to gange dagligt (5-10 timers mellemrum) på dag -6 til -4 og højdosis etoposid (VP-16) IV over 4 timer på dag -3. Mandlige patienter gennemgår også strålebehandlingsboost til testiklerne på dag -6.
Patienter gennemgår autolog SCT på dag 0 og modtager sargramostim (GM-CSF) SC én gang dagligt, begyndende 6 timer efter afslutningen af SCT og fortsætter, indtil blodtallene genoprettes.
- Allogen SCT: Patienter modtager det præparative regime som ved autolog SCT og gennemgår derefter allogen SCT på dag 0. Patienter modtager GM-CSF som ved autolog SCT.
Post-SCT imatinibmesylatbehandling: Efter bedring fra autolog eller allogen SCT får patienterne oralt imatinibmesylat én gang dagligt. Behandling med imatinibmesylat fortsætter i fravær af sygdomsprogression eller uacceptabel toksicitet.
- Gruppe II (Ph-kromosom-negative patienter):
- Intensiveringsterapi: Begyndende 4 uger efter afslutningen af den anden induktionsterapi modtager patienter højdosis MTX IV over 2 timer på dag 1, 8 og 22; leucovorin calcium IV hver 6. time i 4 doser og derefter oralt hver 6. time i 12 doser begyndende 22-24 timer efter hver MTX-infusion; og ASP IV over 30 minutter på dag 2, 9 og 23.
Patienter, der er ≤ 50 år med en histokompatibel donor, fortsætter til allogen SCT og gennemgår allogen SCT som i gruppe I. Patienter, der er ≤ 50 år uden en passende donor, randomiseres til 1 ud af 2 behandlingsarme.
Arm I (konventionel konsoliderings-/vedligeholdelsesterapi):
- Konventionel konsolideringsterapi: Under forløb 1 får patienterne ARA-C IV over 30 minutter og VP-16 IV over 1 time på dag 1-5; VCR IV på dag 1, 8, 15 og 22; og oral dexamethason på dag 1-28. I løbet af forløb 2 (som begynder 4 uger efter påbegyndelse af forløb 1, eller når blodtallene kommer sig), får patienterne ARA-C og VP-16 som i forløb 1. I løbet af forløb 3 (som begynder 4 uger efter påbegyndelse af forløb 2, eller når blodtallene genoprettes), modtager patienter DNR IV på dag 1, 8, 15 og 22; CTX IV over 30 minutter på dag 29; ARA-C IV over 30 minutter på dag 31-34 og 38-41; og oral thioguanin på dag 29-42. I løbet af forløb 4 (som begynder 8 uger efter påbegyndelse af forløb 3, eller når blodtallene kommer sig), får patienterne behandling som i forløb 2.
- Vedligeholdelsesterapi: Begyndende 4 uger efter påbegyndelse af kursus 4 af konsolideringsterapi, eller når blodtallene kommer sig, får patienterne oral MP dagligt; MTX oralt eller IV én gang om ugen; VCR IV én gang hver 12. uge; og oral PRED i 5 dage hver 12. uge. Vedligeholdelsesterapi fortsætter i 2,5 år efter påbegyndelse af intensiveringsterapi.
- Arm II (autolog SCT): Patienter gennemgår autolog SCT som i gruppe I med undtagelse af højdosis konsoliderings-/mobiliseringskemoterapi.
Patienterne følges hver 6. måned i 2 år.
PROJEKTERET TILSLUTNING: Ca. 40 patienter om året vil blive opsamlet for gruppe I (Philadelphia [Ph] kromosom-positive patienter) i denne undersøgelse. Ca. 550 patienter vil blive opsamlet for gruppe II (Ph-kromosom-negative patienter) i denne undersøgelse inden for 5 år.

SYGDOMS KARAKTERISTIKA:
Histologisk bekræftet akut lymfatisk leukæmi (ALL)
Mere end 25% lymfoblaster i knoglemarv
- Patienter med myeloid antigenekspression OG utvetydig lymfoid immunfænotype er kvalificerede
Philadelphia (Ph) kromosomstatus bestemt af cytogenetik, fluorescens in situ hybridisering (FISH) og/eller RNA-analyse
- Patienter, der er fastslået til at være Ph-kromosom-negative af cytogenetik, men positive for BCR-ABL af FISH eller polymerase-kædereaktion, betragtes som Ph-kromosom-positive
- Patienter med Ph-kromosompositiv sygdom kan være op til 65 år
- Ingen myelodysplasi eller anden forudgående hæmatologisk lidelse
Patienter på 50 år og derunder skal være HLA-typebestemt under induktionsterapi af undersøgelsesbehandling ELLER give en skriftlig forklaring for ikke at gennemgå HLA-typning
- A- og B-tastning påkrævet
