Sammenligning af kirurgiske procedurer for at reducere stressinkontinens i urinen (SISTEr)

Randomiseret klinisk afprøvning af Burch-modificerede Tanagho- og Autolog Fascia-slyngeprocedurer for kvinder med overvejende anstrengelsesurininkontinens
Det primære formål med dette kliniske forsøg er at sammenligne behandlingssuccesen for to kirurgiske procedurer, der ofte anvendes og har lignende helbredelsesrater, men som dog ikke er blevet sammenlignet direkte med hinanden i et stort, strengt udført randomiseret forsøg.
De sekundære formål med forsøget er at sammenligne andre resultater for de to kirurgiske procedurer, herunder livskvalitet, seksuel funktion, tilfredshed med behandlingsresultater, komplikationer og behov for anden behandling efter operationen.
Opfølgningen vil vare minimum to år og op til fire år.

- Fremherskende stressurininkontinens ved selvrapportering, undersøgelse og test;
- Urethral hypermobilitet;
- Berettiget til begge kirurgiske indgreb;
- Ambulant;
- Ikke gravid;
- >12 måneder efter fødslen;
- Ingen systemisk sygdom kendt for at påvirke blærefunktionen;
- Ingen nuværende kemoterapi eller strålebehandling;
- Ingen urethral divertikel, augmentation cytoplasty eller kunstig lukkemuskel;
- Ingen nylig bækkenoperation;
- Tilgængelig til opfølgning og i stand til at gennemføre undersøgelsesvurderinger;

Burch colposuspensionen
|
Burch colposuspension er en veletableret procedure til kirurgisk behandling af kvinder med anstrengelsesurininkontinens.
Den Burch-modificerede colposuspension suspenderer den forreste vaginalvæg (i niveau med blærehalsen) med permanente suturer bundet til det iliopektinale ligament.

Pubovaginal slynge, ved hjælp af autolog rectus fascia

Fascialslyngen er også en veletableret procedure til kirurgisk behandling af kvinder med anstrengelsesurininkontinens.
Den autologe slyngeprocedure placerer en høstet strimmel rectus fascia transvaginalt i niveau med det proksimale urinrør.
Fasciastrimlen er fastgjort overlegent til rectus fascia med permanente suturer
