MUSKEL ENERGI VERSUS MULLIGAN TEKNIKKER i BEHANDLING AF PATIENTER med CERVIKAL SPONDYLOSE
Denne undersøgelse vil have til formål at besvare følgende spørgsmål:
Hvilken teknik, MET eller MMT, er mere effektiv til at reducere smerte, øge bevægelsesområdet (ROM), forbedre proprioception og forbedre funktionsevnen hos personer med CS?
Det vil blive antaget, at:
- Alle deltagere vil overholde undersøgelsesinstruktioner og procedurer.
- Alle deltagere vil blive evalueret under standardiserede forhold.

Dette triple-blindede (undersøgeren, statistikeren og deltagerne vil blive blindet, mens terapeuten vil være opmærksom på behandlingsprocedurer) randomiserede kontrollerede forsøg (RCT) vil undersøge effektiviteten af muskelenergiteknikker (MET) sammenlignet med Mulligan mobiliseringsteknikker ( MMT) om cervikal rygsøjlesmerter, ROM og funktion hos deltagere med cervikal spondylose (CS). Undersøgelsen vil blive udført på den ambulante fysioterapiklinik på Horus University i New Damietta, Egypten.
Inklusionskriterier:
Deltagerne i begge grupper vil blive udvalgt efter følgende kriterier:
- Deltagere af begge køn i alderen 40 - 60 år, diagnosticeret med klinisk og medicinsk stabil CS.
- Alle deltagere vil blive vurderet og henvist af ortopædkirurger, inden undersøgelsen påbegyndes.
- Mekanisk nakkesmerter lokaliseret til livmoderhalsen.
- Nakkesmerter, der varer mere end 3 måneder, lokal ømhed i de øvre fibre i trapezius, begrænsning i cervikal ROM og muskelspasmer.
- Moderat smerteintensitet (45-74 mm på VAS) (Mahajan et al., 2012).
- Deltagere med moderat smerteintensitet NDI (30 - 48%).
- Alle deltagere er fri for patologiske tilstande, der kan påvirke resultaterne.
Eksklusionskriterier:
Deltagere, der opfylder et af følgende kriterier, vil blive udelukket fra undersøgelsen:
- Deltagere med systemiske sygdomme som leddegigt, stofskiftesygdomme eller osteoporose).
- Historie med traumer eller utilsigtede skader på halshvirvelsøjlen.
- Cervikal myelopati, inflammatorisk arthritis, tumorer, infektioner, der involverer den cervikale rygsøjle, eller vertebrobasilar arterieinsufficiens.
- Cervikal rygsøjleskade, svimmelhed, svimmelhed eller andre muskuloskeletale lidelser.

Inklusionskriterier:

Eksperimentel: Gruppe (A) Muskelenergiteknik (MET).
Disse deltagere vil modtage MET'er, ud over et standard fysioterapiprogram bestående af dybe nakkebøjer-styrkende øvelser, isometrisk øvelse og række bevægelsesøvelser (3 sessioner om ugen i 4 uger).

MET vil blive anvendt på tre nøglemuskler, der almindeligvis er forbundet med nakkesmerter: øvre trapezius, levator scapulae og anterior scalenus.

Eksperimentel: Gruppe (B) Mulligan Mobiliseringsteknik-gruppe (MMT).
Deltagerne vil modtage MMT-interventioner sideløbende med standardfysioterapiprogrammet dybe nakkebøjer-forstærkende øvelser, isometrisk øvelse og række bevægelsesøvelser 3 sessioner om ugen i 4 uger.

Deltagere i gruppe (B) vil modtage MMT ved siden af den traditionelle behandling.
MMT bruger en specifik teknik kaldet Sustained Natural Apophyseal Glides (SNAG'er) til at forbedre ledmobilisering.
