Detaillierte Beschreibung
Patienten mit Typ-2-Diabetes haben ein erhöhtes Risiko für makrovaskuläre und mikrovaskuläre Erkrankungen, die beide durch die Kontrolle des erhöhten Blutdrucks bei Personen mit Bluthochdruck reduziert werden. Es wurde auch gezeigt, dass eine intensive glykämische Kontrolle mikrovaskuläre Erkrankungen reduziert, aber die Auswirkungen auf makrovaskuläre Erkrankungen bleiben ungewiss. In dieser Studie werden die Hypothesen untersucht, dass eine Blutdrucksenkung (mit einer ACE-Hemmer-Diuretika-Kombination) und eine intensive glykämische Kontrolle (mit einem Sulfonylharnstoff-basierten Regime) bei Hochrisikopersonen mit Typ-2-Diabetes (einschließlich hypertensiver und nicht hypertensiver Personen) reduziert werden das Auftreten sowohl makrovaskulärer als auch mikrovaskulärer Erkrankungen.
Die Studie ist eine 2 x 2 faktorielle, randomisierte, kontrollierte Studie, die 11.140 Erwachsene mit Typ-2-Diabetes mit erhöhtem Risiko für Gefäßerkrankungen einschließt. Nach 6-wöchiger Open-Label-Perindopril-Indapamid-Kombination wurden in Frage kommende Personen randomisiert einer Fortführung der Behandlung mit Perindopril-Indapamid oder einem entsprechenden Placebo und einem intensiven Gliclazid-MRT-basierten Glukosekontrollschema (mit dem Ziel eines HbA1c von 6,5 % oder weniger) oder den üblichen leitlinienbasierten Behandlungen zugeteilt Therapie. Primäre Endpunkte sind erstens die Kombination aus nicht-tödlichem Schlaganfall, nicht-tödlichem Myokardinfarkt oder kardiovaskulärem Tod und zweitens die Kombination aus neuer oder sich verschlechternder Nephropathie oder diabetischer Augenerkrankung. Diese primären Ergebnisse werden gemeinsam und getrennt analysiert. Die durchschnittliche Behandlungs- und Nachsorgedauer beträgt 5,5 bis 6 Jahre. Die Studie wird in 214 Zentren in Australasien, Asien, Europa und Nordamerika durchgeführt.
ADVANCE soll zuverlässigen Nachweis über das Nutzen-Risiko-Verhältnis einer blutdrucksenkenden Therapie und einer intensiven Glukosekontrolltherapie bei Hochrisiko-Diabetikern liefern, unabhängig vom anfänglichen Blutdruck oder Blutzuckerspiegel.

Einschlusskriterien:
- Eine Diagnose von Typ-2-Diabetes mellitus, die erstmals im Alter von 30 Jahren oder älter gestellt wurde
- Alter 55 Jahre oder älter bei Eintritt
- Fähigkeit zur informierten Einwilligung
Ein erheblich erhöhtes Risiko für Herz-Kreislauf-Erkrankungen, angezeigt durch:
- Eine schwere makrovaskuläre Erkrankung in der Vorgeschichte, definiert als eine der folgenden: Schlaganfall, Myokardinfarkt, Krankenhauseinweisung wegen transitorischer ischämischer Attacke, Krankenhauseinweisung wegen instabiler Angina pectoris, Koronararterien-Bypass-Transplantation, perkutane transluminale Koronarangioplastie (mit oder ohne Stent), periphere Revaskularisation (Angioplastie). oder Operation) oder Amputation infolge einer Gefäßerkrankung oder
- Eine Vorgeschichte schwerer mikrovaskulärer Erkrankungen, definiert als Nephropathie (Albumin:Kreatinin-Verhältnis > 300 ug/mg), retinale Photokoagulationstherapie, proliferative Retinopathie (neue Blutgefäße auf der Bandscheibe oder anderswo, Glaskörperblutung, präretinale Blutung oder fibröse Proliferationen). auf der Bandscheibe oder anderswo), Makulaödem (Netzhautverdickung innerhalb eines Bandscheibendurchmessers des Makulazentrums) oder Erblindung in einem der Augen (korrigierte Sehschärfe 6/60 oder schlechter, anhaltend für drei Monate oder länger), von denen nicht bekannt ist, dass sie auf nicht zurückzuführen sind -diabetische Ursachen oder
- Eine erste Diagnose von Typ-2-Diabetes, die 10 oder mehr Jahre vor der Einreise gestellt wurde, oder
- Ein weiterer wichtiger Risikofaktor für Gefäßerkrankungen, definiert als einer der folgenden: aktuelles tägliches Zigarettenrauchen, Gesamtcholesterin über 6,0 mmol/l (mit oder ohne cholesterinsenkende Behandlung), HDL-Cholesterin < 1,0 mmol/l, Mikroalbuminurie (Albumin:Kreatinin-Verhältnis 30 -300ug/mg) oder
- Alter 65 Jahre oder älter
Ausschlusskriterien:
- Eine definitive Kontraindikation für die Behandlung mit einem ACE-Hemmer oder einem Thiazid-ähnlichen Diuretikum
- Eine spezifische Indikation für die Behandlung mit einem anderen ACE-Hemmer als Perindopril 2-4 mg täglich (siehe auch Abschnitt 5.2.3) oder einem Thiazid-ähnlichen Diuretikum
- Eine definitive und spezifische Indikation für die Behandlung mit Gliclazid oder ein Hämoglobin-A1c-Kontrollziel von 6,5 % oder weniger
- Eine definitive Kontraindikation für die Behandlung mit Gliclazid oder ein Hämoglobin-A1c-Kontrollzielwert von 6,5 % oder weniger
- Eine definitive Indikation für eine Langzeit-Insulintherapie mit voller Dosis oder vor dem Schlafengehen
- Teilnahme an einer Studie innerhalb des Monats vor dem Registrierungsbesuch oder aktuelle Teilnahme an einer anderen Studie
Weitere mögliche Gründe für eine Nichtberechtigung sind:
- Hohe Wahrscheinlichkeit der Nichteinhaltung der Studienbehandlung oder Nachsorge
- Aktuelle klinische Instabilität (z. B. ein größeres zerebrales oder koronares Ereignis oder eine visusbedrohende Retinopathie oder ein Makulaödem innerhalb der letzten Wochen)
- Andere lebensbedrohliche nicht-vaskuläre Erkrankungen als Diabetes und ihre Komplikationen
- Mittelschwere oder schwere Demenz
- Schwerwiegende Behinderung, die wahrscheinlich den regelmäßigen Besuch der Studienkliniken verhindert
