Die vorliegende Studie versucht, ein Krankenversicherungs-Navigationsprogramm mit Überlebenden von Krebs im Kindesalter zu evaluieren, die aus der Long-Term Follow-Up (LTFU)-Kohorte rekrutiert wurden. Überlebende von Krebs im Kindesalter sind ihr ganzes Leben lang mit gesundheitlichen Herausforderungen konfrontiert, die eine Überwachung und kontinuierliche Betreuung erfordern. Hinzu kommt die Tendenz unter Überlebenden im Kindesalter zu höheren Unterversicherungsquoten, ungedeckten Gesundheitsbedürfnissen und belastenden Kosten im Zusammenhang mit der Pflege. Diese belastenden Kosten tragen auch dazu bei, dass die Versorgung der Überlebenden nicht in Anspruch genommen wird. Dr. Park und ihre Kollegen veröffentlichten Ergebnisse, die darauf hindeuteten, dass LTFU-Überlebende Schwierigkeiten hatten, zu verstehen, wie sie ihre Versicherung nutzen sollten, und oft finanzielle Probleme hatten. Das Verstehen und Navigieren von Versicherungsleistungen in der aktuellen Landschaft ist für Krebsüberlebende von entscheidender Bedeutung, um die Gesundheitsversorgung zu erhalten und zu nutzen, die sie benötigen. Vor diesem Hintergrund schlagen die Studienforscher vor, die Wirksamkeit einer Versicherungsnavigationsintervention mit LTFU-Teilnehmern zu bewerten, die in einer synchronen und asynchronen Modalität durchgeführt wird.
Die Navigationsintervention wird von einem Krankenversicherungsnavigator über HIPAA-konforme Videokonferenzen für die synchrone Gruppe (HINT-S) und über eine vorab aufgezeichnete Videositzung für die asynchrone Gruppe (HINT-A) geliefert. Die Teilnehmer werden randomisiert entweder in die beiden Navigationsinterventionsarme (HINT-S und HINT-A; ungefähr 234 pro Interventionsarm) oder in den Arm mit verbesserter üblicher Versorgung (ungefähr 52 für den Kontrollarm) eingeteilt. Die Stichprobengröße pro Arm wurde gewählt, um eine Bewertung der Durchführbarkeits- und Akzeptanzziele zu ermöglichen und sinnvolle Unterschiede in den Ergebnissen zu untersuchen. Um die vorgeschlagenen primären und sekundären Ergebnisse zu bewerten, werden alle Studienteilnehmer eine Grundlinien- und 6-monatige, 12-monatige und 18-monatige Nachbeobachtungsumfrage nach dem Programm ausfüllen.

- in der Lage sind, eine informierte Einwilligung zu erteilen
- Zugang zu einem Smartphone, Computer oder Tablet mit Internetzugang haben
- eine in den USA ansässige Krankenversicherung haben
- aktuelle Teilnehmer der LTFU-Kohorte
- Zugang zum CCSS-Patientenportal haben.

Die Veränderung der selbstberichteten finanziellen Härte im Laufe der Zeit wird zwischen den Gruppen verglichen und anhand von 3 Skalen gemessen, die 3 Bereiche der finanziellen Härte abdecken: a) Verhalten, das den ungedeckten Gesundheitsversorgungsbedarf aufgrund von Gesundheitskosten im vergangenen Jahr mit 8 Ja bewertet /keine Gegenstände; mehr Ja-Werte = mehr Härte; b) Psychologische, die die Besorgnis aufgrund von Gesundheitskosten im vergangenen Jahr mit 6 Punkten bewertet, gemessen an einem 5-Punkte-Likert; eine höhere Punktzahl spiegelt größere Sorgen wider; c) Material, das die finanziellen Folgen der Gesundheitskosten im vergangenen Jahr mit 8 Ja/Nein-Items bewertet; mehr Ja-Ergebnisse = mehr Härte.

Die Inanspruchnahme der Gesundheitsversorgung wird anhand von 11 Punkten bewertet, bei denen die Patienten aufgefordert werden, Ja/Nein zu Gesundheitsversorgungsbesuchen zu melden (z. B. Besuche in der Grundversorgung im vergangenen Jahr, gewöhnlicher Versorgungsort, Erhalt jährlicher Impfungen). Eine größere Anzahl von Ja-Antworten spiegelt eine stärkere Nutzung des Gesundheitswesens wider. |
