Descripción
Los criterios de inclusión para la inscripción en el estudio de trauma son los siguientes:
- Traumatismo cerrado sin traumatismo craneoencefálico aislado
- Ausencia de lesión cerebral traumática, definida como cabeza AIS <4 O motor GCS> 3 dentro de las 24 horas posteriores a la lesión
- Llegada al departamento de emergencias <= 6 horas desde el momento de la lesión
- Transfusión de sangre dentro de las 12 horas posteriores a la lesión.
- Déficit de base >=6 O presión arterial sistólica <90 mmHg dentro de los 60 minutos posteriores a la llegada al servicio de urgencias
- Médula espinal cervical total o parcialmente intacta
Todos los pacientes que cumplen con estos criterios se ingresan en la base de datos epidemiológicos y se evalúan según los criterios de exclusión específicos para establecer si se justifican extracciones de sangre en serie.
La presencia de cualquiera de los siguientes criterios de exclusión descalifica a un sujeto del estudio de muestreo de trauma.
- Edad < 16
- Supervivencia anticipada de <24 horas desde la lesión
- Supervivencia anticipada <28 días debido a una condición médica preexistente
- Incapacidad para obtener la primera extracción de sangre dentro de las primeras 12 horas después de la lesión
- Lesión cerebral traumática; es decir, GCS ≤8 después de la admisión en la UCI Y anormalidad en la tomografía computarizada del cerebro dentro de las 12 horas posteriores a la lesión
- Incapacidad para obtener el consentimiento informado
- Inmunosupresión preexistente en curso, p. receptor de trasplante
- Inmunosupresión preexistente en curso, p. corticosteroides crónicos en dosis altas (>20 mg/equivalentes de prednisona/día)
- Inmunosupresión preexistente en curso, p. tratamiento con fármacos oncolíticos en los últimos 14 días
- Inmunosupresión preexistente en curso, p. VIH positivo Y recuento de CD4 <200 células/mm3
- Posible requerimiento de inmunosupresión temprana - p. probabilidad significativa de requerir dosis altas de corticosteroides (p. lesión espinal)
- Disfunción orgánica preexistente significativa - pulmón: actualmente recibe oxigenoterapia domiciliaria, según lo documentado en los registros médicos
- Disfunción orgánica preexistente significativa - corazón: insuficiencia cardíaca congestiva, documentada en registros médicos
- Disfunción orgánica preexistente significativa - renal: insuficiencia renal crónica (creatinina >2)
- Disfunción orgánica preexistente significativa - hígado: cirrosis con hipertensión portal o encefalopatía
- Paciente lesionado mientras tomaba muestras de inscripción temporalmente en espera
