Inflammation och värdens reaktion på skada
Syftet med denna studie är att hjälpa till att förbättra vår förståelse av den biologi som är involverad i kroppens svar på allvarliga trauman eller brännskador.
Värdens svar på trauma och brännskador är en samling fysiologiska och patofysiologiska processer som är kritiskt beroende av regleringen av det mänskliga medfödda immunsystemet, med särskild tonvikt på den inflammatoriska komponenten i det systemet.
Inget enskilt forskningscentrum eller en liten grupp av centra har kapacitet att avgränsa det integrerade svaret hos detta komplexa biologiska system, som involverar flera molekylära och genetiska interaktioner som varierar i tid.
Vårt förslag främjar identifieringen av viktiga dynamiska relationer som reglerar integrationen av detta komplexa biologiska system, med förväntningen att denna förståelse i slutändan kommer att påverka diagnosen, prognosen och behandlingen av den inlagda, allvarligt skadade patienten.

Detta storskaliga samarbetsprojekt ger möjlighet att skaffa den nödvändiga nya kunskapen direkt hos människor.
Kunskap kommer att förvärvas med hjälp av olika toppmoderna genomiska och proteomiska teknologier, en mycket komplex klinisk, proteomisk och genomisk databas, såväl som nyutvecklade, nya analytiska verktyg för att undersöka denna komplexa datamängd.
Våra analytiska möjligheter på genomisk och proteomisk nivå utvecklas nu snabbt och vår förmåga att länka dessa genomiska och proteomiska data till vägar och funktionella moduler kommer att hjälpa oss att närmare länka dessa cellulära data till immunologiska processer och i slutändan till det fenotypiska svaret (dvs. bana) i den skadade värden.
Som ett resultat kan potentiella insatser, oavsett om det sker genom vårt program eller andra finansieringsmekanismer, utformas mer effektivt.

Akut inlagda trubbiga traumapatienter
Beskrivning
Inklusionskriterier för inskrivning i traumastudien är följande:
- Trubbigt trauma utan isolerad huvudskada
- Frånvaro av traumatisk hjärnskada, definierad som antingen AIS-huvud <4 ELLER GCS-motor >3 inom 24 timmar efter skadan
- Akutmottagningens ankomst <=6 timmar från tidpunkten för skadan
- Blodtransfusion inom 12 timmar efter skadan
- Basunderskott >=6 ELLER systoliskt blodtryck <90 mmHg inom 60 minuter från akutmottagningens ankomst
- Helt eller delvis intakt cervikal ryggmärg
Alla patienter som uppfyller dessa kriterier läggs in i den epidemiologiska databasen och utvärderas för specifika uteslutningskriterier för att fastställa om serieblodtagningar är berättigade.
Närvaron av något av följande uteslutningskriterier diskvalificerar en försöksperson från traumaprovtagningsstudien.
- Ålder < 16
- Förväntad överlevnad på <24 timmar från skada
- Förväntad överlevnad <28 dagar på grund av redan existerande medicinskt tillstånd
- Oförmåga att ta första blodprov inom de första 12 timmarna efter skadan
- Traumatisk hjärnskada; d.v.s. GCS ≤8 efter intensivvårdsinläggning OCH avvikelse vid hjärndatortomografiskanning inom 12 timmar efter skadan
- Oförmåga att få informerat samtycke
- Redan existerande, pågående immunsuppression - t.ex. transplantationsmottagare
- Redan existerande, pågående immunsuppression - t.ex. kroniska högdos kortikosteroider (>20 mg/prednisonekvivalenter/dag)
- Redan existerande, pågående immunsuppression - t.ex. onkolytisk(a) läkemedelsbehandling inom de senaste 14 dagarna
- Redan existerande, pågående immunsuppression - t.ex. HIV-positiva OCH CD4-antal <200 celler/mm3
- Eventuellt krav på tidig immunsuppression - t.ex. betydande sannolikhet för att kräva höga doser kortikosteroider (t. ryggradsskada)
- Betydande redan existerande organdysfunktion - lunga: får för närvarande syrgasbehandling i hemmet, som dokumenterats i medicinska journaler
- Betydande redan existerande organdysfunktion - hjärta: kongestiv hjärtsvikt, som dokumenterats i medicinska journaler
- Signifikant redan existerande organdysfunktion - njurfunktion: kronisk njursvikt (kreatinin >2)
- Betydande redan existerande organdysfunktion - lever: cirros med portal hypertoni eller encefalopati
- Patient skadad under provtagning tillfälligt pausad
