Yksityiskohtainen kuvaus
Tämän tutkimuksen tarkoituksena on arvioida silmäsilmityksen, lacripepin, sarveiskalvon (silmän etuosaa peittävän läpinäkyvä kudos) haavan paranemisen vaikutusta), kuten satunnaisen tai taisteluun liittyvän trauman jälkeen tai silmäleikkauksista, kuten fotorefraktiivinen keratectomy (PRK). Tämä tutkimus auttaa ymmärtämään, onko Lacripep tehokas hoito silmän pinnan paranemisessa ja visuaalisen palautumisen nopeuttamisessa vamman jälkeen.
Osallistujien on oltava aktiivisen palvelun jäsen, joka on kelvollinen hoitoon sotilaallisessa hoitolaitoksessa.

Sisällyttämiskriteerit:
- Mies tai nainen, mikä tahansa rodusta, 21 -vuotiaille ja vanhempille
- Aktiivisen palvelun sotilaspalvelun jäsenet, jotka ovat oikeutettuja hoitoon
- Likinäköisyyden haluaminen -4,00 -diopteriin (d) pallot asti ja ilman, että astigmatismi 0 --1,50 d ja -4,00 d -ilkeä pallomainen ekvivalentti molemmissa silmissä
- Vakaa taittuminen viimeisen vuoden ajan, kuten osoitetaan muutos molempien silmien ilmeisen taitekertaisen pallomaisen ekvivalentin muutoksella ≤ 0,50
- CDVA vähintään 20/20 molemmissa silmissä
- Valittu kahdenväliseen PRK: iin
- Pystyy täyttämään seurantavaatimukset jopa 6 kuukauden ajanjaksoon leikkauksen jälkeen
Poissulkemiskriteerit:
- Laskettu ablaation syvyys yli 75 mikronia tai muita olosuhteita kummassakin silmässä, mikä tutkijoiden arvioinnissa vaatii ennaltaehkäisevän MMC: n käyttöä
- Merkit epävakaasta tai edistyksellisestä likinäköisyydestä, keratokonuksesta tai keratokonuksesta epäillään kummassakin silmässä
- Epänormaalit sarveiskalvon topografiset havainnot kummassakin silmässä, esimerkiksi keratokonus, pellucid marginaalinen rappeutuminen
- Monovisioniin kohdistettu hoito
- Kuivasilmän merkkejä ja oireita, jotka perustuvat: (a) sarveiskalvon fluoreseiinin värjäytymisen kokonaispistemäärä ≥4 National Eye Institute (NEI)/teollisuuden työpaja -asteikolla kummassakin silmässä, (b) anethetoitu Schirmer -testitulokset ≤5 mm/5 minuuttia kummassakin silmässä, (c) oireiden pistemäärä ≤40 käyttämällä oireen arviointia kuivassa silmissä (Sande) -kyselyssä
- Naisten henkilöt, jotka ovat raskaana, imettävät tai aikovat tulla raskaaksi tutkimuksen aikana. Raskaus ja imetys ovat vasta -aiheita taitekerroksen yleensä, mukaan lukien PRK, osallistuminen tähän tutkimukseen vai ei.
- Jäännös-, toistuvat tai aktiiviset silmätaudit tai sarveiskalvon poikkeavuudet joko silmässä, kuten irisis, uveiitti, keratokonjunktiviitti sicca, herpeettinen keratiitti, yläsidoskonktiviitti, lagophthalmos, sarveiskalvon huijari, anterior -kellarin muistomerkki, toistuvat eroosit, glokki, glokki, merkittäviä kaihia

- Samanaikaiset ajankohtaiset tai systeemiset lääkkeet, jotka voivat heikentää paranemista, mukaan lukien kortikosteroidit, antimetaboliitit, isotretinoiini (Accutane®), amiodaronihydrokloridi (Cordarone®) ja/tai sumatriptaani (Imitrex®) (muut lääkkeet samassa perheessä kuin Imitrex saavat
- Sairaus (t), jotka tutkijan arvioinnissa voi heikentää paranemista, mukaan lukien, mutta rajoittumatta, kollageenin verisuonisairauksiin, autoimmuunisairauksiin, immuunikatosairauksiin ja silmän herpes zoster tai simplex.

Lacripepin teho
Aikaikkuna: Epiteelin paksuuden arviointi suoritetaan 4 ja 7 päivässä postoperatiivisesti sitten 1, 3 ja 6 kuukaudessa leikkauksen jälkeen.

Lakripepin tehokkuuden määrittämiseksi sarveiskalvon haavan paranemiseen, epiteelin paksuus mitataan käyttämällä optista koherenssitomografiaa (OCT).
OCT on ei-invasiivinen kuvantamistekniikka, joka käyttää kevyitä aaltoja kudosrakenteiden yksityiskohtaisten kuvien luomiseen.

Lacripepin turvallisuuden määrittämiseksi korjatun etäisyyden näköterveyden (CDVA) palautus mitataan korkean kontrastin näkökyvyn testillä.
CDVA: n palautus määritellään prosentuaaliseksi silmille, joilla ei ole enempää kuin 1 CDVA -linja, joka on menetetty preoperatiivisesta lähtötasosta.
