Descrizione dettagliata
Lo scopo di questo studio è di valutare l'effetto di un farmaco per occhiali, lacripep, sulla guarigione delle ferite nella cornea (il tessuto trasparente che copre la parte anteriore dell'occhio) come visto dopo un trauma accidentale o correlato al combattimento o da interventi chirurgici oculari come la cheratectomia fotorefrattiva (PRK). Questo studio contribuirà a capire se LaCripep è un trattamento efficace nella guarigione della superficie oculare e nel recupero visivo accelerato dopo lesioni.
I partecipanti devono essere un membro del servizio attivo idoneo per le cure in una struttura di trattamento militare.

Descrizione
Criteri di inclusione:
- Maschio o femmina, di qualsiasi razza, di età pari o superiore a 21 anni
- Membri del servizio militare statunitense attivo idoneo per le cure
- Desidendo la correzione refrattiva della miopia fino a -4,00 diottrie (d) sfera con o senza astigmatismo da 0 a -1,50 d e fino a -4,00 d manifest equivalente sferico in entrambi gli occhi
- Rifrazione stabile per l'anno passato, come dimostrato da un cambiamento nel manifest equivalente sferico refrattivo di ≤ 0,50 diottrie (d) in entrambi gli occhi
- CDVA di almeno 20/20 in entrambi gli occhi
- Eletto per sottoporsi a PRK bilaterale
- In grado di soddisfare i requisiti di follow-up per un massimo di 6 mesi dopo l'intervento
Criteri di esclusione:
- Profondità di ablazione calcolata superiore a 75 micron o altre condizioni in entrambi gli occhi che nel giudizio degli investigatori richiederanno l'uso di MMC profilattico
- Segni di miopia instabile o progressiva, cheratocono o cheratocono sospettano in entrambi gli occhi
- Risultati topografici corneali anormali in entrambi gli occhi, ad esempio cheratocono, degenerazione marginale pellucida
- Trattamento mirato alla monovisione
- Segni e sintomi dell'occhio secco in base a: (a) punteggio totale della fluoresceina corneale ≥4 nella scala del National Eye Institute (NEI)/Industria in entrambi gli occhi, (b) punteggio di test Schirmer anetetizzato ≤5 mm/5 minuti in occhio, (c) punteggio del sintomo ≤40 usando la valutazione dei sintomi nell'occhio secco (sandde) in caso di eye eye
- Soggetti femminili che sono incinta, allattando o intendono rimanere incinta durante lo studio. La gravidanza e l'allattamento al seno sono controindicazioni alla chirurgia refrattiva in generale, incluso il PRK, che partecipa a questo studio o meno.
- Malattie oculari residue, ricorrenti o attive o anomalie corneali in entrambi gli occhi come irite, uveite, cheratocongiuntivite siCCA, cheratite erpetica, congiuntivite in vernale, respontica del Lagoftalia, clugazione cornea, chiarezza corneale, culgazione corneale, chiodo corneale, chiaretta corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiodo corneale, chiaretta cornea, chiaretta di chicche da notte
- Precedente chirurgia corneale o intraoculare in occhio per scopi di rifrazione o terapeutica
- Farmaci topici o sistemici simultanei che possono compromettere la guarigione, tra cui corticosteroidi, antimetaboliti, isotretinoina (Accutane®), amiodarone hydrocloruro (Cordarone®) e/o sumatriptan (imitrex®) (altri farmaci nella stessa famiglia di Imitrex saranno ancora consentiti)
- Le condizioni mediche, che, nel giudizio dell'investigatore, possono compromettere la guarigione, incluse ma non limitate a malattie vascolari di collagene, malattie autoimmuni, malattie dell'immunodeficienza e herpes herpes oculare o simplex.
- Le persone non legalmente competenti a fornire il consenso informato (ad es. Individui incapaci, cognitivi) non saranno iscritti. Tutti i soggetti devono fornire il consenso per partecipare allo studio

Efficacia di lacripep
Lasso di tempo: La valutazione dello spessore epiteliale verrà eseguita a 4 e 7 giorni dopo l'intervento, quindi a 1, 3 e 6 mesi dopo l'intervento.

Per determinare l'efficacia del lacripep per la guarigione della ferita corneale, lo spessore epiteliale verrà misurato usando la tomografia a coerenza ottica (OCT).
OCT è una tecnica di imaging non invasiva che utilizza onde luminose per creare immagini dettagliate di strutture tissutali.

La valutazione dello spessore epiteliale verrà eseguita a 4 e 7 giorni dopo l'intervento, quindi a 1, 3 e 6 mesi dopo l'intervento.

Sicurezza di lacripep

Per determinare la sicurezza di lacripep, il ritorno dell'acuità visiva a distanza corretta (CDVA) sarà misurato utilizzando un test di acuità visiva ad alto contrasto.
Il rendimento di CDVA sarà definito come percentuale di occhi con non più di 1 linea CDVA persa rispetto alla linea di base preoperatoria.
