Lacrípep para Estudo de Cura de Feridas da Córnea
Lacripep, um novo agente de investigação, para cicatrização de feridas na córnea e homeostase da superfície ocular

Descrição detalhada
O objetivo deste estudo é avaliar o efeito de um medicamento de olho -roo, lacrípep, na cicatrização de feridas na córnea (o tecido transparente que cobre a frente do olho), como visto após trauma incidental ou relacionado ao combate, ou de cirurgias oculares como a ceracectomia fotorefrativa (PRK). Este estudo ajudará a entender se o lacrípep é um tratamento eficaz na cicatrização da superfície ocular e acelerar a recuperação visual após a lesão.
Os participantes devem ser um membro do serviço ativo elegível para atendimento em uma instalação de tratamento militar.

Critérios de inclusão:
- Masculino ou feminino, de qualquer raça, 21 anos ou mais
- Membros do Serviço Militar dos EUA de serviço ativo elegíveis para atendimento
- Desejando correção refrativa da miopia até -4,00 dioptrias (d) Esfera com ou sem astigmatismo 0 a -1,50 d e até -4,00 d manifesto equivalente esférico em ambos os olhos
- Refração estável no ano passado, como demonstrado por uma mudança no equivalente esférico refrativo manifesto de ≤ 0,50 dioptrias (d) em ambos os olhos
- CDVA de pelo menos 20/20 em ambos os olhos
- Eleito para se submeter a PRK bilateral
- Capaz de atender aos requisitos de acompanhamento por até 6 meses após o pós-operatório
Critérios de exclusão:
- Profundidade de ablação calculada superior a 75 mícrons ou outras condições em qualquer olho, o que no julgamento dos investigadores exigirá o uso de MMC profilático
- Sinais de miopia instável ou progressiva, keratoconus ou keratoconus suspeito em qualquer olho
- Achados topográficos anormais da córnea em ambos os olhos, por exemplo, ceratoconus, degeneração marginal pelúcida
- Tratamento direcionado para monovisão
- Sinais e sintomas de olho seco com base em: (a) Pontuação total da coloração com fluoresceína da córnea ≥4 no National Eye Institute (NEI)/Escala de oficina do setor em qualquer um dos olhos, (b) Anetetized Schirmer Test Score ≤5 mm/5 minutos em qualquer um dos olhos, (C) Sintoma Score ≤ Eye ≤ Eye na avaliação dos sintomas (Laronening) (C) Sintoma ≤ 40 anos, usando a avaliação dos sintomas (5 minutos).
- As mulheres que estão grávidas, amamentando ou pretendem engravidar durante o estudo. A gravidez e a amamentação são contra -indicações para a cirurgia refrativa em geral, incluindo o PRK, seja participando deste estudo ou não.
- Residual, recurrent or active ocular diseases or corneal abnormalities in either eye such as iritis, uveitis, keratoconjunctivitis sicca, herpetic keratitis, vernal conjunctivitis, lagophthalmos, corneal scarring, anterior basement membrane disease, recurrent erosions, glaucoma, previous steroid responder, occludable chamber angles, visually significant cataracts
- Córnea anterior ou intra -ocular em qualquer olho para fins refrativos ou terapêuticos
- Medicamentos tópicos ou sistêmicos concomitantes que podem prejudicar a cicatrização, incluindo corticosteróides, antimetabólitos, isotretinoína (accutane®), cloridrato de amiodarona (Cordarone®) e/ou sumatriptano (IMitrex®) (outros medicamentos na mesma família como imitrex ainda serão permitidos)
- Condições médicas, que, no julgamento do investigador, podem prejudicar a cura, incluindo, entre outros, doenças vasculares colágenas, doenças autoimunes, doenças de imunodeficiência e herpes ocular Zoster ou simplex.
- Indivíduos não legalmente competentes para fornecer consentimento informado (por exemplo, indivíduos incapacitados, com deficiência cognitiva) não serão inscritos. Todos os sujeitos devem fornecer consentimento para participar do estudo

Eficácia do lacrípep
Prazo: A avaliação da espessura epitelial será realizada aos 4 e 7 dias após o operatório, depois de 1, 3 e 6 meses no pós -operatório.

Para determinar a eficácia do lacrípep na cicatrização da ferida na córnea, a espessura epitelial será medida usando a tomografia óptica de coerência (OCT).
OCT é uma técnica de imagem não invasiva que usa ondas leves para criar imagens detalhadas de estruturas de tecido.

A avaliação da espessura epitelial será realizada aos 4 e 7 dias após o operatório, depois de 1, 3 e 6 meses no pós -operatório.

Segurança de lacrípep
Prazo: Da inscrição até o final do tratamento aos 6 meses
|
Para determinar a segurança do lacrípep, o retorno da acuidade visual da distância corrigida (CDVA) será medido usando um teste de acuidade visual de alto contraste.
O retorno do CDVA será definido como porcentagem de olhos com não mais de 1 linha CDVA perdida da linha de base pré -operatória.
