Le flux de nicotine, un outil potentiellement puissant pour réguler l'apport de nicotine à partir des cigarettes électroniques : importance du flux de nicotine pour le taux d'apport de nicotine et effet subjectif
Les systèmes électroniques de distribution de nicotine (ENDS) chauffent et vaporisent un liquide contenant de la nicotine pour produire un aérosol qui peut délivrer de la nicotine au sang et au cerveau.
L'utilisation d'ENDS a augmenté rapidement au cours de la dernière décennie, en particulier chez les jeunes : plus de 20 % des lycéens américains sont des utilisateurs actuels d'ENDS, et il existe des preuves de dépendance à la nicotine dans cette population.
Une législation fédérale a été proposée qui limiterait la concentration de nicotine liquide ENDS pour rendre ENDS "beaucoup moins addictif et attrayant pour les jeunes".
Cependant, ces efforts et d'autres pour freiner la dépendance en limitant la concentration de nicotine liquide ont peu de chances de réussir car les émissions de nicotine des ENDS dépendent de plusieurs variables.
Pour atteindre les objectifs de santé publique visés, les réglementations ciblant la dépendance doivent se concentrer sur l'apport de nicotine, et non sur la concentration de nicotine.
Bien que l'administration de nicotine ne puisse pas être régulée directement, la vitesse à laquelle un ENDS émet de la nicotine, le "flux de nicotine", peut être régulée et, surtout, prédite sur la base de quelques variables de conception et de fonctionnement de l'appareil.
Cependant, à ce jour, il n'existe aucune preuve empirique démontrant la relation entre le flux et l'administration, ni entre le flux et les effets subjectifs qui soutiennent la dépendance à la nicotine.
Il est essentiel de combler cette lacune pour fournir un cadre efficace de réglementation des ENDS.
À l'Université américaine de Beyrouth, les enquêteurs évalueront la relation entre le flux de nicotine, la forme et les effets subjectifs.
Les participants utiliseront des dispositifs ENDS avec différents flux et formes de nicotine.
Les mesures de dépendance, telles que l'envie de fumer, le besoin impérieux et l'abstinence, seront évaluées.
Le résultat indiquera dans quelle mesure le flux/la forme de nicotine influence les effets subjectifs liés à la dépendance, à l'intensité des bouffées et à l'exposition aux substances toxiques.
En résumé, ce projet fournira les preuves empiriques nécessaires aux agences de santé publique pour utiliser le flux de nicotine comme une construction englobante et pratique pour réguler l'administration de nicotine à partir d'ENDS.

Description détaillée
Évaluer l'influence du flux de nicotine et de la forme de nicotine sur les effets subjectifs.
À l'AUB, les enquêteurs évalueront les effets subjectifs (par ex.
goût du produit, envie de nicotine) et topographie des bouffées pour 130 participants qui subiront 5 séances d'utilisation ENDS consistant en 2 épisodes (10 bouffées + 60min ad libitum) avec 2 flux (16 et 32μg/s) x 2 formes (protoné, base libre) et un état 0 nicotine.
De plus, les enquêteurs utiliseront un appareil de pointe pour prélever in situ une fraction de l'aérosol généré lors de chaque bouffée afin de vérifier le flux et la forme réels de nicotine et de mesurer l'exposition aux toxiques pulmonaires (carbonyles).
Les chercheurs émettent l'hypothèse que l'augmentation du flux de nicotine et de la nicotine protonée entraînera une plus grande réduction du besoin de nicotine, une intensité de bouffée et une exposition au carbonyle plus faibles.

Critère d'intégration:
- Doit être en bonne santé et avoir plus de 18 ans
- Doit être disposé à fournir un consentement éclairé, à assister au laboratoire et à s'abstenir de tabac/nicotine au besoin (les participants seront invités à s'abstenir de consommer de la nicotine/du tabac et/ou ENDS pendant ≥ 12h)
- Un double ENDS et un fumeur qui déclare une utilisation quotidienne d'ENDS (≥ 3 mg/ml de nicotine) ou de cigarettes (toute fréquence) ET une utilisation un jour (≥ 3 jours/semaine) d'ENDS (≥ 3 mg/ml de nicotine) ou de cigarettes (toute fréquence) fréquence) au cours des 3 derniers mois ou plus
Critère d'exclusion:
- Antécédents de maladie chronique ou de trouble psychiatrique non contrôlé
- Antécédents de maladie cardiovasculaire active ou hypotension artérielle, convulsions, utilisation régulière d'un médicament sur ordonnance (à l'exception des vitamines/contraceptifs oraux)
- Consommation de cocaïne, d'opioïdes, de benzodiazépines ou de méthamphétamines le mois dernier
- Individus qui déclarent consommer de la marijuana > 15/30 jours
- Les femmes seront exclues si elles allaitent ou si elles sont enceintes
- Participants ayant l'intention d'arrêter de fumer/de fumer de la nicotine dans les 30 prochains jours et orientés vers un traitement de sevrage

Les enquêteurs testeront les effets subjectifs du flux de nicotine (0μg/s) placebo.
Après une période d'observation d'une heure, les participants utiliseront Subox Mini C dans un combat dirigé de 10 bouffées (intervalle de 30 secondes entre les bouffées).
Une heure après le premier combat, les participants seront invités à tirer sur l'appareil pendant 60 minutes ad libitum.
Les mesures subjectives (c'est-à-dire la dépendance à la nicotine, les effets de la drogue, le goût du produit et l'état de manque) seront administrées 5 fois/session : 5 min avant et 5 min après le début de l'accès dirigé et 5 min avant, à mi-chemin et 5 min après le début de l'attaque. le combat ad libitum.

This item is drawn from the Electronic Cigarette Specific Effects Questionnaire, a modified/expanded version of the modified Cigarette Evaluation Questionnaire (mCEQ; Cappelleri et al., 2007) developed for ENDS.
Participants rate the item on a 0-10 scale immediately following each bout.

Importance vapor will be assessed by the Electronic Cigarette Specific Effects questionnaire and consists of 1 item scored 0 - 10 with 0 being not at all and 10 being extremely, with higher scores indicating vapor visibility mattered more to the participant.
This item is drawn from the Electronic Cigarette Specific Effects Questionnaire, a modified/expanded version of the modified Cigarette Evaluation Questionnaire (mCEQ; Cappelleri et al., 2007) developed for ENDS.
Participants rate the item on a 0-10 scale immediately following each bout.
Item wording: 'Is the vapor/cloud of the e-cig important to you?' (0 = not at all, 10 = extremely).

The Minnesota Nicotine Withdrawal Scale (MNWS; based on Hughes and Hatsukami, 1986 questionnaire) was measured using composite mean of MNWS items (irritability, anxiety, difficulty concentrating, restlessness, hunger/appetite, Impatient, depression, desire for sweets and craving) on a 0-10 scale (Not at all - Extremely), with higher scores indicating greater withdrawal severity.
