L'objectif de cet essai clinique est de tester si la combinaison d'un régime cétogène (RC) avec une stimulation théta burst intermittente accélérée et personnalisée (iTBS) produit de plus grandes réductions des symptômes dépressifs que l'iTBS combinée à un régime alimentaire sain standard chez des adultes souffrant de dépression résistante au traitement. Plus précisément, l'étude vise à déterminer si l'intervention combinée :

  1. Réduit les symptômes dépressifs
  2. Augmente les niveaux de cétones circulantes
  3. Est réalisable et tolérable pendant le traitement accéléré par iTBS

Les participants commenceront soit un RC soit un régime conforme au Guide alimentaire canadien (GAC) avec une période d'introduction diététique de 3 semaines, après quoi ils suivront un traitement d'iTBS accéléré et personnalisé tout en continuant leur régime attribué. Avant et après le traitement par iTBS, les participants effectueront des évaluations cliniques, fourniront des échantillons sanguins pour des tests métaboliques et subiront des IRM pour évaluer la connectivité cérébrale. Critères d'inclusion :

  • Âgé de 18 à 65 ans, de tout sexe, identité de genre, origine ethnique et statut socio-économique
  • Actuellement en épisode dépressif majeur selon les critères du DSM-5-TR et confirmé par un médecin de l'étude
  • Présentant une sévérité des symptômes au moins modérée (MADRS ≥ 20)
  • Répondant aux critères de dépression résistante au traitement (TRD), définie comme une non-réponse à au moins deux essais adéquats d'antidépresseurs
  • Naïf en neuromodulation (pas de rTMS ou d'électroconvulsivothérapie antérieure)
  • Capable de fournir un consentement éclairé
  • Disponible pour l'intervention de 15 semaines et prêt à suivre soit un régime cétogène, soit un régime aligné sur le Guide alimentaire canadien

Expérimental: Ketogenic Diet
Participants in this arm will follow a well-formulated ketogenic diet (low carbohydrate, moderate protein, high fat) for a 3-week dietary lead-in period prior to neuromodulation, and will continue the diet for a total of 12 weeks. The diet is designed to achieve and maintain nutritional ketosis (blood ketone levels of 0.5 to 3 mmol/L). Dietitian support will be provided through scheduled counseling and ongoing monitoring of finger-stick ketone and glucose testing.
Intermittent theta burst stimulation (iTBS), a form of repetitive transcranial magnetic stimulation (rTMS), is a non-invasive brain stimulation technique approved by the FDA and Health Canada for the treatment of TRD. In this study, participants will receive an accelerated course of imaging-guided, neuronavigated left dorsolateral prefrontal cortex (DLPFC) iTBS targeted based on functional connectivity with the subgenual anterior cingulate cortex (sgACC). Stimulation protocol consists of 1800 pulses per session, delivered at 110% of resting motor threshold, with 50-minute inter-session intervals, for 8 sessions per day over 5 consecutive days.

Dietitian counseling will be approximately matched in frequency and duration to the ketogenic diet arm. Nutritional monitoring will include dietary logs and metabolic assessments without targeted induction of ketosis. Participants will perform finger-stick glucose testing.

Les utilisateurs approuvés obtiendront l'accès via des systèmes de partage de données sécurisés et approuvés par l'institution. Tous les utilisateurs doivent signer un accord d'utilisation des données qui interdit la ré-identification, le partage ultérieur et toute utilisation en dehors de la proposition approuvée.
