Critérios de Inclusão:

  • Idade 18-65 de qualquer sexo, identidade de género, etnia e estatuto socioeconómico
  • Atualmente a vivenciar um episódio depressivo maior conforme definido pelos critérios DSM-5-TR e confirmado por um médico do estudo
  • Apresentando pelo menos gravidade moderada dos sintomas (MADRS ≥ 20)
  • Preenchendo critérios para depressão resistente ao tratamento (DRT), definida como ausência de resposta a pelo menos dois ensaios antidepressivos adequados
  • Ingénuo em neuromodulação (sem rTMS ou eletroconvulsoterapia no passado)

Experimental: Ketogenic Diet
Participants in this arm will follow a well-formulated ketogenic diet (low carbohydrate, moderate protein, high fat) for a 3-week dietary lead-in period prior to neuromodulation, and will continue the diet for a total of 12 weeks. The diet is designed to achieve and maintain nutritional ketosis (blood ketone levels of 0.5 to 3 mmol/L). Dietitian support will be provided through scheduled counseling and ongoing monitoring of finger-stick ketone and glucose testing.
Intermittent theta burst stimulation (iTBS), a form of repetitive transcranial magnetic stimulation (rTMS), is a non-invasive brain stimulation technique approved by the FDA and Health Canada for the treatment of TRD. In this study, participants will receive an accelerated course of imaging-guided, neuronavigated left dorsolateral prefrontal cortex (DLPFC) iTBS targeted based on functional connectivity with the subgenual anterior cingulate cortex (sgACC). Stimulation protocol consists of 1800 pulses per session, delivered at 110% of resting motor threshold, with 50-minute inter-session intervals, for 8 sessions per day over 5 consecutive days.

Nutritional monitoring will include dietary logs and metabolic assessments without targeted induction of ketosis. Participants will perform finger-stick glucose testing.

Os dados serão partilhados com investigadores qualificados para análises cientificamente sólidas, sujeitos à aprovação e execução de um acordo de utilização de dados de acordo com as políticas do Instituto de Investigação Sunnybrook. Os pedidos de acesso serão revistos para garantir mérito científico, viabilidade e proteção da confidencialidade dos participantes.

Prazo de Compartilhamento de IPD

O protocolo do estudo e o formulário de consentimento informado serão publicados antes do início do estudo. Os resultados do ensaio serão divulgados o mais rapidamente possível após a conclusão do estudo. Os DPI anonimizados estarão disponíveis a partir de 12 meses após a publicação dos resultados primários (ou 12 meses após a conclusão do estudo se não houver publicação).

Critérios de acesso de compartilhamento IPD

Os dados partilhados incluirão DIP anonimizados e documentos de suporte (protocolo do estudo, plano de análise estatística e dicionário de dados).

O acesso será limitado a investigadores com afiliação institucional adequada e aprovação ética documentada, que apresentem uma proposta metodologicamente sólida. Os pedidos serão revistos pelo Investigador Principal do estudo (ou pelo comité de acesso a dados designado).

Os utilizadores aprovados obterão acesso através de sistemas de partilha de dados seguros e aprovados institucionalmente. Todos os utilizadores devem assinar um acordo de utilização de dados que proíbe a reidentificação, a partilha subsequente e qualquer utilização fora da proposta aprovada.
