Ketogén diéta és neuromoduláció kezelésre rezisztens depresszióban (ALIGN)
Kiegészítő alacsony szénhidráttartalmú ketogén diéta a képalkotásvezérelt neuromoduláció hatékonyságának fokozására kezelésre rezisztens depresszióban
Ennek a klinikai vizsgálatnak a célja annak tesztelése, hogy a ketogén diéta (KD) kombinálása személyre szabott, gyorsított intermitáló theta burst stimulációval (iTBS) nagyobb csökkenést eredményez-e a depressziós tünetekben, mint az iTBS standard egészséges diétával kombinálva kezelésrezisztens depresszióban szenvedő felnőttek esetén. A vizsgálat azt is célul tűzi ki, hogy meghatározza, hogy a résztvevők kivitelezhetően tudnak-e tartani egy ketogén diétát a gyorsított iTBS kezelés során, és hogy a diéta mérhető változásokat produkál-e a ketonszintekben.
Konkrétan a tanulmány célja annak meghatározása, hogy a kombinált intervenció:
- Csökkenti a depressziós tüneteket
- Növeli a keringő ketonszinteket
- Kivitelezhető és tolerálható a gyorsított iTBS kezelés során
A résztvevők vagy egy KD-t, vagy egy Kanadai Élelmiszer Útmutató által meghatározott diétát (CFGD) kezdenek egy 3 hetes diétás bevezető időszakkal, amelyet követően személyre szabott, gyorsított iTBS kezelésen vesznek részt, miközben folytatják a hozzárendelt diétájukat. Bevonási kritériumok:
- 18-65 éves kor, bármely nemű, nemi identitású, etnikumú és társadalmi-gazdasági státuszú
- Jelenlegi nagy depressziós epizód a DSM-5-TR kritériumai szerint, amelyet egy vizsgálati orvos megerősít
- Legalább közepes tüneti súlyosság (MADRS ≥ 20)
- Megfelel a kezelésrezisztens depresszió (TRD) kritériumainak, amelyet legalább két megfelelő antidepresszáns kezelésre adott nem-válasz definiál
- Nem volt korábban neuromodulációs kezelés (nincs múltbeli rTMS vagy elektrokonvulzív terápia)
- Képes tájékozott beleegyezés adására
- Elérhető a 15 hetes intervencióra és hajlandó követni egy ketogén vagy kanadai élelmiszer-útmutató szerinti étrendet
Kizárási kritériumok:
- Orvosi/pszichiátriai komorbiditások, amelyek akadályozzák a vizsgálatban való részvételt, vagy ahol a depresszió nem a fő aggodalomra ad okot adó pszichiátriai tünet
- Epilepszia, stroke vagy súlyos neurológiai állapotok, pszichózis vagy anyagfüggőség anamnézise az elmúlt 6 hónapban

Kísérleti: Ketogenic Diet
Participants in this arm will follow a well-formulated ketogenic diet (low carbohydrate, moderate protein, high fat) for a 3-week dietary lead-in period prior to neuromodulation, and will continue the diet for a total of 12 weeks.
The diet is designed to achieve and maintain nutritional ketosis (blood ketone levels of 0.5 to 3 mmol/L).
Dietitian support will be provided through scheduled counseling and ongoing monitoring of finger-stick ketone and glucose testing.

Intermittent theta burst stimulation (iTBS), a form of repetitive transcranial magnetic stimulation (rTMS), is a non-invasive brain stimulation technique approved by the FDA and Health Canada for the treatment of TRD.
In this study, participants will receive an accelerated course of imaging-guided, neuronavigated left dorsolateral prefrontal cortex (DLPFC) iTBS targeted based on functional connectivity with the subgenual anterior cingulate cortex (sgACC).
Stimulation protocol consists of 1800 pulses per session, delivered at 110% of resting motor threshold, with 50-minute inter-session intervals, for 8 sessions per day over 5 consecutive days.

The diet will emphasize balanced intake of vegetables, fruits, whole grains, and protein foods, without specific macronutrient restrictions.
Dietitian counseling will be approximately matched in frequency and duration to the ketogenic diet arm.
Nutritional monitoring will include dietary logs and metabolic assessments without targeted induction of ketosis.
Participants will perform finger-stick glucose testing.

A megosztott adatok magukban foglalják a személyazonosító adatoktól megtisztított egyéni betegadatokat (IPD) és a kiegészítő dokumentumokat (tanulmányi protokoll, statisztikai elemzési terv és adatszótár).
A hozzáférés azon kutatókra korlátozódik, akik megfelelő intézményi hovatartozással és dokumentált etikai engedéllyel rendelkeznek, és módszertanilag megalapozott javaslatot nyújtanak be. A kérelmeket a tanulmány fővizsgálója (vagy a kijelölt adathozzáférési bizottság) fogja felülvizsgálni.
A jóváhagyott felhasználók a biztonságos, intézményileg jóváhagyott adatmegosztó rendszereken keresztül kapnak hozzáférést.
