Dieta Cetogénica y Neuromodulación en la Depresión Resistente al Tratamiento (ALIGN)
Dieta cetogénica baja en carbohidratos como adyuvante para mejorar la neuromodulación guiada por imágenes en la depresión resistente al tratamiento
El objetivo de este ensayo clínico es evaluar si la combinación de una dieta cetogénica (KD) con estimulación theta burst intermitente acelerada y personalizada (iTBS) produce mayores reducciones en los síntomas depresivos que la iTBS combinada con una dieta saludable estándar en adultos con depresión resistente al tratamiento. El ensayo también pretende determinar si los participantes pueden seguir de manera factible una dieta cetogénica durante un curso de tratamiento acelerado con iTBS y si la dieta produce cambios medibles en los niveles de cetonas.
Específicamente, el estudio tiene como objetivo determinar si la intervención combinada:
- Reduce los síntomas depresivos
- Aumenta los niveles circulantes de cetonas
- Es factible y tolerable durante el tratamiento acelerado con iTBS
Los participantes comenzarán una dieta cetogénica (KD) o una dieta alineada con la Guía Alimentaria Canadiense (CFGD) con un período de introducción dietética de 3 semanas, después del cual se someterán a un curso de iTBS acelerada y personalizada mientras continúan con su dieta asignada. Antes y después del curso de tratamiento con iTBS, los participantes completarán evaluaciones clínicas, proporcionarán muestras de sangre para pruebas metabólicas y se someterán a escáneres de resonancia magnética para evaluar la conectividad cerebral. Los niveles de cetonas se medirán diariamente durante la intervención dietética de 15 semanas. Criterios de inclusión:
- Edad 18-65 de cualquier sexo, identidad de género, etnia y nivel socioeconómico
- Actualmente experimentando un episodio depresivo mayor según los criterios DSM-5-TR y confirmado por un médico del estudio
- Presentando al menos una gravedad moderada de síntomas (MADRS ≥ 20)
- Cumplir criterios para depresión resistente al tratamiento (TRD), definida como falta de respuesta a al menos dos ensayos antidepresivos adecuados
- Sin experiencia previa en neuromodulación (sin rTMS o terapia electroconvulsiva previas)
- Capaz de dar consentimiento informado
- Disponible para la intervención de 15 semanas y dispuesto a seguir una dieta cetogénica o alineada con la Guía Alimentaria Canadiense
Criterios de exclusión:
- Comorbilidades médicas/psiquiátricas que impidan la participación en el estudio o donde la depresión no sea el síntoma psiquiátrico principal de preocupación
- Antecedentes de epilepsia, ictus, o condiciones neurológicas mayores, psicosis, o dependencia de sustancias en los últimos 6 meses
- Discapacidad física o cognitiva que interfiera con la participación
- Mujeres embarazadas (auto-reporte o mediante análisis de sangre), en periodo de lactancia, o que planeen un embarazo durante el periodo del estudio IMC < 20 kg/m²
- Intentos de suicidio en los últimos 12 meses
- Intención suicida activa confirmada por el psiquiatra del estudio
- Trastorno alimentario activo en los últimos 12 meses
- Seguimiento actual de una dieta cetogénica (KD)
- Dieta habitual baja en carbohidratos en los últimos 6 meses
- Trastornos gastrointestinales o alergias alimentarias incompatibles con los protocolos dietéticos
- Consumo de alcohol >3 bebidas/día o >14/semana
- Uso de anticonvulsivos (se permitirán benzodiacepinas con dosis <2 equivalentes de lorazepam), agonistas GABA, o medicamentos que reduzcan la eficacia de la TMS
- Enfermedad médica grave
- Contraindicaciones para la resonancia magnética (MRI)
- Falta de disposición para realizar pruebas diarias de punción digital

Experimental: Ketogenic Diet
Participants in this arm will follow a well-formulated ketogenic diet (low carbohydrate, moderate protein, high fat) for a 3-week dietary lead-in period prior to neuromodulation, and will continue the diet for a total of 12 weeks.
The diet is designed to achieve and maintain nutritional ketosis (blood ketone levels of 0.5 to 3 mmol/L).
Dietitian support will be provided through scheduled counseling and ongoing monitoring of finger-stick ketone and glucose testing.

Intermittent theta burst stimulation (iTBS), a form of repetitive transcranial magnetic stimulation (rTMS), is a non-invasive brain stimulation technique approved by the FDA and Health Canada for the treatment of TRD.
In this study, participants will receive an accelerated course of imaging-guided, neuronavigated left dorsolateral prefrontal cortex (DLPFC) iTBS targeted based on functional connectivity with the subgenual anterior cingulate cortex (sgACC).
Stimulation protocol consists of 1800 pulses per session, delivered at 110% of resting motor threshold, with 50-minute inter-session intervals, for 8 sessions per day over 5 consecutive days.

Comparador activo: Canadian Food Guide-Aligned Diet
Participants in this arm will follow a Canadian Food Guide-aligned diet for a 3-week dietary lead-in period prior to neuromodulation and will continue the diet for a total of 12 weeks.
The diet will emphasize balanced intake of vegetables, fruits, whole grains, and protein foods, without specific macronutrient restrictions.
Dietitian counseling will be approximately matched in frequency and duration to the ketogenic diet arm.
Nutritional monitoring will include dietary logs and metabolic assessments without targeted induction of ketosis.
Participants will perform finger-stick glucose testing.

Criterios de acceso compartido de IPD
Los datos compartidos incluirán IPD anonimizados y documentos de apoyo (protocolo del estudio, plan de análisis estadístico y diccionario de datos).
El acceso estará limitado a investigadores con afiliación institucional apropiada y aprobación ética documentada, que presenten una propuesta metodológicamente sólida. Las solicitudes serán revisadas por el investigador principal del estudio (o el comité designado para el acceso a datos).
Los usuarios aprobados obtendrán acceso a través de sistemas seguros de intercambio de datos aprobados institucionalmente. Todos los usuarios deben firmar un acuerdo de uso de datos que prohíba la reidentificación, el intercambio posterior y cualquier uso fuera de la propuesta aprobada.
