I dati pubblicati dal Veterans Administration Cooperative Study of Hypertension hanno dimostrato che la riduzione della morbilità e della mortalità potrebbe essere ottenuta trattando uomini con pressione arteriosa diastolica fissa superiore a 105 mm Hg. Tendenze simili si sono verificate per quelli con pressione arteriosa diastolica fissa tra 90 e 104 mm Hg. I risultati e le tendenze attuali di altri studi hanno supportato questi risultati. Tuttavia, prima dell'inizio dell'Hypertension Detection and Follow-up Program (HDFP), non era noto se i benefici della terapia antipertensiva fossero applicati a tutti gli ipertesi nella popolazione generale e se l'uso delle conoscenze mediche esistenti potesse ridurre significativamente la morbilità e la mortalità da ipertensione nelle comunità.
Riconoscendo questa esigenza, NHLBI ha avviato le attività pilota dell'HDFP per caratterizzare fattori operativi, socioeconomici e motivazionali significativi che avrebbero influenzato l'accettazione della terapia antipertensiva nelle popolazioni definite all'interno delle quali avrebbe avuto luogo la sperimentazione clinica controllata e per ottenere le informazioni di base necessarie per l'impresa della sperimentazione clinica, che doveva determinare se un programma pratico, intensivo e antipertensivo potesse ridurre significativamente la morbilità e la mortalità negli ipertesi nella popolazione generale.
La pianificazione del processo, compreso lo sviluppo di un protocollo e di un manuale operativo, iniziò nel 1971. Tra febbraio 1973 e maggio 1974, 158.906 persone sono state sottoposte a screening per l'ipertensione in 14 comunità. Sono stati randomizzati un totale di 10.940 partecipanti ipertesi.
L'ipotesi principale testata dallo studio era che il controllo intensivo della pressione arteriosa sotto cura graduale per cinque anni potesse ridurre significativamente la mortalità rispetto a quello sotto cura riferita. La cura graduale era il metodo di trattamento nelle cliniche HDFP in cui inizialmente veniva somministrato un diuretico e venivano aggiunti ulteriori agenti antipertensivi in modo graduale e strutturato nel tempo fino al raggiungimento della pressione arteriosa desiderata. L'assistenza rinviata rappresentava l'invio a medici privati e ad altre fonti di assistenza della comunità. Hanno partecipato a questo studio 14 centri clinici, un centro di coordinamento, un centro ECG, un laboratorio centrale e un laboratorio di monitoraggio.
La fase clinica della sperimentazione terminò nel maggio 1982. Il progetto è stato prorogato nel 1983 per continuare la sorveglianza della mortalità e il controllo della pressione arteriosa.
PROGETTAZIONE NARRATIVA:
Lo studio era uno studio randomizzato, non cieco, a campione fisso con un singolo intervento e gruppi di controllo. Il gruppo di intervento ha ricevuto cure graduali dalle cliniche della sperimentazione clinica (vedi Sfondo, sotto), mentre quelli del gruppo di controllo sono stati indirizzati ai propri medici. Ciascuna comunità ha contribuito sia ai partecipanti all'assistenza a gradini che a quelli indirizzati, ma per scopi di analisi, i gruppi sono stati raggruppati in due gruppi. L'endpoint primario era la mortalità. Gli effetti dell'assistenza graduale rispetto a quella riferita sono stati valutati anche su fattori intermedi e secondari, inclusi codici nosologici di cause specifiche di mortalità, infarto miocardico non fatale, ictus, cardiopatia ipertensiva e anomalie dell'ECG.
