TAUSTA:
Veterans Administration Cooperative Study of Hypertension -tutkimuksen julkaistut tiedot osoittivat, että sairastuvuutta ja kuolleisuutta voidaan vähentää hoitamalla miehiä, joilla on kiinteä diastolinen verenpaine yli 105 mmHg. Samanlaisia suuntauksia esiintyi niillä, joiden diastolinen verenpaine oli kiinteä 90–104 mmHg. Muiden tutkimusten tulokset ja nykyiset suuntaukset tukivat näitä havaintoja. Ennen verenpainetaudin havaitsemis- ja seurantaohjelman (HDFP) aloittamista ei kuitenkaan tiedetty, hyötyykö verenpainelääkitys kaikista yleisen väestön verenpainepotilaista ja voisiko olemassa olevan lääketieteellisen tiedon hyödyntäminen vähentää merkittävästi sairastuvuutta ja kuolleisuutta korkea verenpaine yhteisöissä.
Tunnustettuaan tämän tarpeen NHLBI käynnisti HDFP:n pilottitoiminnot karakterisoidakseen merkittäviä toiminnallisia, sosioekonomisia ja motivoivia tekijöitä, jotka vaikuttaisivat verenpainetta alentavan hoidon hyväksymiseen määritellyissä populaatioissa, joissa kontrolloitu kliininen tutkimus tapahtuisi, ja hankkiakseen tarvittavat perustiedot kliinisen tutkimuksen sitoumus, jonka tarkoituksena oli määrittää, voisiko käytännöllinen, intensiivinen ja verenpainetta alentava ohjelma vähentää merkittävästi verenpainepotilaiden sairastuvuutta ja kuolleisuutta yleisessä väestössä.
Kokeen suunnittelu, mukaan lukien protokollan ja toimintaohjeen kehittäminen, aloitettiin vuonna 1971. Helmikuun 1973 ja toukokuun 1974 välisenä aikana 158 906 henkilöä tutkittiin korkean verenpaineen varalta 14 paikkakunnalla. Yhteensä 10 940 verenpainepotilasta satunnaistettiin.
Kokeessa testattu ensisijainen hypoteesi oli, että intensiivinen verenpaineen hallinta porrastetussa hoidossa viiden vuoden ajan voisi merkittävästi vähentää kuolleisuutta lähetehoidossa tehtyyn hoitoon verrattuna. Stepped-hoito oli HDFP-klinikoiden hoitomenetelmä, jossa annettiin aluksi diureettia ja lisää verenpainetta alentavia aineita lisättiin aikarakenteisesti, vaiheittain, kunnes tavoiteverenpaine saavutettiin. Lähetetty hoito edusti lähetettä yksityisille lääkäreille ja muille yhteiskunnallisille hoitolähteille. Tähän tutkimukseen osallistui 14 kliinistä keskusta, koordinoiva keskus, EKG-keskus, keskuslaboratorio ja seurantalaboratorio.
Kokeen kliininen vaihe päättyi toukokuussa 1982. Hanketta jatkettiin vuoteen 1983 kuolleisuuden ja verenpaineen hallinnan seurannan jatkamiseksi.
SUUNNITTELUN KARRATIIVINEN:
Tutkimus oli satunnaistettu, ei-sokea, kiinteä näytetutkimus, jossa oli yksi interventio- ja kontrolliryhmä. Interventioryhmä sai porrastettua hoitoa kliinisiltä klinikoilta (katso Tausta, alla), kun taas kontrolliryhmän jäsenet ohjattiin omien lääkäriensä luo. Jokainen yhteisö osallistui sekä porrashoitoon että lähetettyyn hoitoon osallistujia, mutta analysointia varten ryhmät yhdistettiin kahteen ryhmään. Ensisijainen päätetapahtuma oli kuolleisuus. Vaiheittaisen ja lähetetyn hoidon vaikutuksia arvioitiin myös väli- ja toissijaisiin tekijöihin, mukaan lukien nosologiset koodit erityisistä kuolleisuuden syistä, ei-fataalista sydäninfarktia, aivohalvausta, verenpainetautia ja EKG-poikkeavuuksia.
