ЗАДНИЙ ПЛАН:
Опубликованные данные совместного исследования гипертонии Управления по делам ветеранов показали, что снижение заболеваемости и смертности может быть достигнуто путем лечения мужчин с фиксированным диастолическим артериальным давлением выше 105 мм рт. Аналогичные тенденции наблюдались у лиц с фиксированным диастолическим артериальным давлением от 90 до 104 мм рт. Результаты и текущие тенденции других исследований подтверждают эти выводы. Тем не менее, до начала Программы выявления гипертонии и последующего наблюдения (HDFP) не было известно, применимы ли преимущества антигипертензивной терапии ко всем гипертоникам в общей популяции и может ли использование существующих медицинских знаний значительно снизить заболеваемость и смертность от гипертонии. артериальная гипертензия в сообществах.

Планирование испытания, включая разработку протокола и руководства по эксплуатации, началось в 1971 году. С февраля 1973 г. по май 1974 г. 158 906 человек были обследованы на высокое кровяное давление в 14 населенных пунктах. Всего было рандомизировано 10 940 участников с артериальной гипертензией.
Основная гипотеза, проверенная в ходе исследования, заключалась в том, что интенсивный контроль артериального давления при поэтапном лечении в течение пяти лет может значительно снизить смертность по сравнению с лечением по направлению. Ступенчатая терапия была методом лечения в клиниках HDFP, при котором сначала давали диуретик, а дополнительные антигипертензивные препараты добавляли в структурированном во времени поэтапном режиме до тех пор, пока не было достигнуто целевое артериальное давление. Направленная помощь представляла собой направление к частным врачам и другим общественным источникам помощи. В исследовании приняли участие 14 клинических центров, координационный центр, центр ЭКГ, центральная лаборатория и лаборатория мониторинга.
Клиническая фаза испытаний завершилась в мае 1982 года. Проект был продлен до 1983 года, чтобы продолжить наблюдение за смертностью и контролем артериального давления.
ОПИСАНИЕ ДИЗАЙНА:
Исследование было рандомизированным, неслепым, с фиксированной выборкой, с одним вмешательством и контрольной группой. Группа вмешательства получала поэтапное лечение в клиниках клинических испытаний (см. Фон ниже), в то время как пациенты из контрольной группы были направлены к своим врачам. Каждое сообщество предоставило участников как для поэтапной помощи, так и для направленной помощи, но для целей анализа группы были объединены в две группы. Первичной конечной точкой была смертность. Эффекты ступенчатой и направленной помощи также оценивались по промежуточным и вторичным факторам, включая нозологические коды конкретных причин смертности, нефатальный инфаркт миокарда, инсульт, гипертоническую болезнь сердца и отклонения на ЭКГ.
