POZADÍ:
Publikovaná data z Veterans Administration Cooperative Study of Hypertension prokázala, že snížení morbidity a mortality lze dosáhnout léčbou mužů s fixním diastolickým krevním tlakem nad 105 mm Hg. Podobné trendy se objevily u pacientů s fixním diastolickým krevním tlakem mezi 90 a 104 mm Hg. Výsledky a současné trendy z jiných studií tato zjištění podpořily. Před zahájením programu pro detekci a sledování hypertenze (HDFP) však nebylo známo, zda přínosy z antihypertenzní léčby platí pro všechny hypertoniky v obecné populaci a zda by využití stávajících lékařských znalostí mohlo významně snížit morbiditu a mortalitu hypertenze v komunitách.
NHLBI si tuto potřebu uvědomila a zahájila pilotní aktivity HDFP s cílem charakterizovat významné provozní, socioekonomické a motivační faktory, které by ovlivnily přijetí antihypertenzní léčby u definovaných populací, v nichž by se kontrolované klinické hodnocení uskutečnilo, a získat základní informace nezbytné pro provedení klinické studie, která měla určit, zda praktický, intenzivní a antihypertenzní program může významně snížit morbiditu a mortalitu hypertoniků v běžné populaci.
Plánování pokusu, včetně vývoje protokolu a provozní příručky, začalo v roce 1971. Mezi únorem 1973 a květnem 1974 bylo ve 14 komunitách vyšetřeno 158 906 osob na vysoký krevní tlak. Celkem bylo randomizováno 10 940 hypertoniků.
Primární hypotéza testovaná ve studii byla, že intenzivní kontrola krevního tlaku ve stupňovité péči po dobu pěti let by mohla významně snížit úmrtnost ve srovnání s úmrtností v doporučené péči. Stupňovitá péče byla metoda léčby na klinikách HDFP, při které byla zpočátku podávána diuretika a další antihypertenziva byla přidávána časově strukturovaným, postupným způsobem, dokud nebylo dosaženo cílového krevního tlaku. Doporučená péče představovala doporučení soukromým lékařům a dalším komunitním zdrojům péče. Této studie se zúčastnilo 14 klinických center, koordinační centrum, EKG centrum, centrální laboratoř a monitorovací laboratoř.
Klinická fáze studie skončila v květnu 1982. Projekt byl prodloužen do roku 1983 s cílem pokračovat v dohledu nad úmrtností a kontrolou krevního tlaku.
DESIGNOVÝ PŘÍBĚH:
Studie byla randomizovaná, neslepá, studie s pevným vzorkem s jednou intervencí a kontrolními skupinami. Intervenční skupině se dostávalo stupňovité péče od klinik klinického hodnocení (viz Pozadí níže), zatímco těm v kontrolní skupině byli předáni jejich vlastní lékaři. Každá komunita přispěla jak účastníky postupné péče, tak doporučené péče, ale pro účely analýzy byly skupiny sloučeny do dvou skupin. Primárním cílovým parametrem byla mortalita. Účinky odstupňované vs. doporučené péče byly také hodnoceny na středních a sekundárních faktorech, včetně nozologických kódů specifických příčin mortality, nefatálního infarktu myokardu, cévní mozkové příhody, hypertenzní choroby srdce a abnormalit EKG.
