CAR-T療法は、白血病、リンパ腫、多発性骨髄腫など一部の難治性血液がんを管理するための比較的新しく革新的な治療戦略です(Efficace、2022)。 この画期的な治療法は、再発性非ホジキンリンパ腫、再発または難治性急性リンパ性白血病、再発性難治性多発性骨髄腫などの特定の悪性腫瘍に対する標準治療となっており、一部の研究では少なくとも80%の患者が治療に陽性反応を示しています(June、2018;Landry、2020)。 CAR T細胞療法は血液悪性腫瘍の治療に大きな進歩をもたらしましたが、同時に独自の毒性も伴います。 最も一般的な急性有害事象は、それぞれサイトカイン放出症候群(CRS)と免疫効果細胞関連神経毒性症候群(ICANS)として知られる神経毒性です(Neelapu、2017)。
最近の研究では、CAR-T療法の検査プロセス開始直後からリハビリテーション前準備(プレハビリテーション)に重点を置き、積極的治療中の機能維持を図り、特に神経学的後遺症が持続する場合はリハビリテーションを密にフォローアップすべきであると示唆されています(Obaisi、2022;Kiefer、2023)。 CAR-T療法を受ける血液がん患者の生活の質(QoL)と機能的能力を改善するための効果的な介入が緊急に必要とされています(Cenik、2020;Jeevanantham、2021)。 身体活動プログラムは、心理的症状、疲労、生活の質に影響を与えることが示されています(Vermaete、2013、2014)。 重要なことに、CAR-T療法を受けている血液悪性腫瘍患者を対象としたエビデンスは現在までに提示されていません。 したがって、CAR-T療法を受ける急速に増加するがん生存者のニーズに対応するため、これらの患者における多様ながんリハビリテーション実践へのアクセス拡大を探る研究が必要です。 最近の系統的レビューは、がんリハビリテーションを促進するための遠隔医療アプローチの高い可能性を示しました(Brick、2022;Chang、2022)。
ホームオートメーションテレマネジメント(HAT)。 HATシステムは、患者が個別化されたセルフケア計画に従うのを支援し、医療従事者がエビデンスに基づくガイドラインに沿って患者の治療とモニタリングを行うのを支援する遠隔医療プラットフォームです(Finkelstein、2012;Fleg、2020;Shero、2022)。 HATは慢性疾患ケアモデル(Grover、2014)に基づいており、患者の自己管理、包括的な患者-提供者間コミュニケーション、多職種連携ケアの調整をサポートします(Drouin、2015)。 HATシステムの主な目的は、慢性疾患管理の負担を最小限に抑え、外来ケアにおけるモニタリングを簡素化することです。 この目標を達成するため、HATシステムは(1)患者と医療提供者間の継続的な情報フィードバックループをサポートすること、(2)すべての日常的な反復作業を引き継ぐこと、(3)患者と臨床医の両方にリアルタイムの臨床意思決定支援を提供することが求められます。 HATは様々な慢性疾患で成功裏に導入・試験されています。
患者ユニットは患者の自宅で使用され、運動や食事処方を含む治療計画に従うよう患者を指導し、患者の進捗と症状をモニターし、医療提供者とコミュニケーションを取ります。 タッチスクリーンタブレット、VRヘッドセット、スマートフォンなど、ウェブ対応のあらゆるデバイスが患者ユニットとして機能します。 ホームユニットのインターフェースは厳格なユーザビリティテスト(Jeong、2015;Finkelstein、2021;Gabriel、2023)を経ており、コンピューター経験の全くない人でも非常に簡単に操作できます。 臨床医ユニット(ウェブ対応のあらゆるデバイス)は、個別化された治療計画の設定と更新、患者自己検査結果の分析、コンピューター生成アラートの確認に使用されます。 HATサーバーは、個別化されたアラート設定と患者自己検査データのリアルタイムモニタリングに基づくコンピューター化された意思決定支援を実装します。 本研究では、HATシステムは、個別化された治療的運動、栄養計画、行動カウンセリング、血液がん生存者の社会的、認知的、生殖的、職業的ニーズを対象とした調整された教育的エンパワーメントを含む多様ながんリハビリテーションプログラムの提供に焦点を当てます。
提案された多様な遠隔リハビリテーションモデルにより、リハビリテーション治療チームはウェブベースのケア管理ポータルを使用して個別化されたリハビリテーション計画を設定し、オンラインで患者の進捗をモニターできます。 自宅の患者は、行動カウンセリング、社会的支援、インタラクティブな教育とエンパワーメントと組み合わされたインタラクティブタッチスクリーン技術によって導かれる安全で効果的なパーソナライズされた運動と栄養計画に従います。 遠隔リハビリテーションシステムの設計は、心血管パラメータと運動パフォーマンスのリアルタイムモニタリングを可能にするクラウドベースのIoTアーキテクチャに基づいています。 運動中の患者の努力レベルは、運動の安全性と有効性をサポートする検証済みのAI駆動アルゴリズムによって自動的に識別されます。 このパイロット実現可能性プロジェクトの最終目標は、提案された患者中心のがん遠隔リハビリテーションモデルが疾患特異的生活の質、機能的・症状アウトカムに及ぼす影響の程度を確立し、マルチセンター研究でこのアプローチを評価する確定的な無作為化臨床試験のための十分なエビデンスを得ることです。
本研究は、CAR-T療法を受ける血液悪性腫瘍患者が多様ながん遠隔リハビリテーションに対して十分な受容性とアドヒアランスを示すかどうかを決定し、それが生活の質、フィットネス、患者報告アウトカムの陽性軌跡と関連するかどうかを明らかにします。 この試験は、CAR-T療法を受ける血液悪性腫瘍患者に対する患者中心の文化的に調整された遠隔医療アプローチによる多様ながんリハビリテーションの実現可能性を評価するための探索的逐次混合メソッドデザインです。
適格患者は、CAR-T療法開始前に4週間の多様なプレハビリテーションを受け、その後、特定のCAR-T治療レジメンに応じて最長4週間、化学療法、CAR-T注入、毒性モニタリング、回復を行いながら、CAR-T療法中に継続的なオンラインエンパワーメントと教育支援を受けます。 これに続き、治療完了と退院後に12週間の多様な遠隔リハビリテーションが行われます。 研究開始時に包括的評価が実施され、患者のフィットネスレベルと栄養状態に基づいて、4週間のプレハビリテーションのために個別にレジスタンス運動、有酸素運動、リラクゼーション運動、栄養計画が処方されます。 モバイルアプリは、外来化学療法、CART注入、毒性モニタリングのための入院を含むCAR-T治療中に、がん生存、早期離床、退院後セルフケアに関するインタラクティブな教育を提供します。 退院後、検証済みのAI駆動リアルタイム運動努力評価(Smiley、2024)を使用する遠隔リハビリテーションシステムを介して、12週間の多様なリハビリテーションプランが提供されます。 社会的支援はグループ遠隔カウンセリングセッションを通じて提供されます。 日常の運動と栄養記録の自動分析により、遠隔リハビリテーションチームは患者の低いアドヒアランスに気づき、遠隔リハビリテーションチームによる調整されたフィードバックを促します。 受容性、ユーザビリティ、実現可能性、アドヒアランスは、分析計画に記載されている通り、研究終了時に評価されます。 最後に、本研究は、遠隔医療プラットフォームを介して提供される多様なリハビリテーション前後の生活の質、フィットネス、体組成、がん症状、行動特性、ケア満足度を評価します。

除外基準:
- 不安定狭心症、管理不良の高血圧、最近の心筋梗塞、ペースメーカー、疼痛性または不安定性の骨転移、最近の骨骨折がある場合

実験的:マルチモーダルがんテレリハビリテーション
登録された各患者は、CAR-T療法の開始前に4週間の多様式プレハビリテーションを受けます。 CAR-T療法期間中、患者は化学療法、点滴投与、院内毒性モニタリング、回復期間中(期間は特定のCAR-T治療レジメンに依存)、がんセルフケア教育と早期離床を支援するインタラクティブアプリを使用します。 CAR-T手順の完了と退院後、患者は12週間の在宅多様式テレリハビリテーションを受けます。 ウェブベースのケア管理ポータルを使用した個別化リハビリテーションプラン。 在宅の患者は、インタラクティブタッチスクリーン技術と行動カウンセリング、社会的サポート、インタラクティブ教育およびエンパワーメントを組み合わせて指導された、安全で効果的な個別化運動・栄養計画に従います。 |
